Triple positioning of tibial tubercle osteotomy for patellofemoral disorders.

BACKGROUND AND PURPOSE: Patellofemoral disorders are often associated with patellofemoral malalignment. Tibial tubercle transfer is an effective method to correct the patellofemoral malalignment. This study evaluated the long-term results of triple positioning of tibial tubercle osteotomy for refractory patellofemoral disorders with 10-year follow-up. PATIENTS AND METHODS: Fifty-six patients with 62 knees underwent triple positioning of tibial tubercle osteotomy for refractory patellofemoral disorders. All patients received standard rehabilitation protocol postoperatively. The average length

Patellofemoral disorders: role of computed tomography and magnetic resonance imaging in defining abnormal rotational lower limb alignment.

BACKGROUND: The contribution of lower limb rotational malalignment to patellofemoral pain and instability has been well recognized. The purpose of the present study is to review the role of computed tomography (CT) and magnetic resonance imaging (MRI) in assessment of abnormal rotational alignment of lower limb. EVIDENCE ACQUISITION: An analysis of all available literature in the English language through 2010 was performed to provide data on a comparison between MRI and CT-specifically, the techniques and normative values used to determine abnormal lower limb alignment. RESULTS: CT and MRI a

Radiologic assessment of patellofemoral pain in the athlete.

CONTEXT: Although disorders of the patellofemoral joint are common in the athlete, their management can be challenging and require a thorough physical examination and radiologic evaluation, including advanced magnetic resonance imaging techniques. EVIDENCE ACQUISITION: Relevant articles were searched under OVID and MEDLINE (1968 to 2010) using the keywords patellofemoral joint, patellofemoral pain or patella and radiography, imaging, or magnetic resonance imaging, and the referenced sources were reviewed for additional articles. The quality and validity of the studies were assessed on the bas

Magnetic resonance imaging assessment of tibial tuberosity-trochlear groove distance: normal values for males and females.

PURPOSE: The aim of this study was to report normal values of the tibial tuberosity-trochlear groove distance (TTTG) in males and females and assess the reliability of MRI in measuring TTTG. METHODS: Patients presenting with a suspected meniscus injury without any patellofemoral or ligamentous instability, and arthroscopically normal cruciate ligaments and patellofemoral joints were included in the study. K-PACS© was used for MRI analysis and was performed by three observers blinded to each others' measurements. RESULTS: One hundred patients (57 males, 43 females) were recruited from 2006-20

The epidemiology of patellofemoral disorders in adulthood: a review of routine general practice morbidity recording.

AIM: To describe the annual consultation prevalence of different patellofemoral disorders across the adult life span. BACKGROUND: The knee is the second most common site for musculoskeletal pain. Evidence from sports injury and orthopaedic settings suggests that patellofemoral disorders constitute a significant proportion of knee pain cases, but we have no evidence from general practice - the setting where most patellofemoral problems will present and be dealt with - of the prevalence of consultations for these problems in the different age groups. METHODS: Age- and gender-stratified annual

Acute patellar dislocation in adolescents: operative versus nonoperative treatment.

PURPOSE: The purpose of this prospective non-randomised study was to compare the efficacy of two opposed methods, operative and conservative. Our hypothesis was that if the method was selected correctly, on an individual basis, the results should be approximately equal. METHODS: The study included 37 adolescents aged between 12 and 16 years, with a mean follow up of 6.1 years. The presence of a significant loose body, confirmed by precise imaging, was the key for selecting operative or arthroscopic treatment. In both groups of patients, we evaluated functional knee scores and the incidence of

Patellofemoral disorders and instability.

Originally the main idea was to obtain a stable patella, i.e., to stabilize the "slipping patella". In the past many conditions like patella alta, ligamentous laxity, PF bone hypoplasia, weakness of the quadriceps muscle, genu valgum or genu recurvatum were thought to predispose to patellar instability. For a long period muscle exercises were instituted to strengthen the weak m.vastus medialis and to make vastus lateralis stronger. This pulls the patella laterally, especially during running or jumping, when lateral luxation of the patella occurs. Muscle imbalance as well as anatomical abnormal

Combined single photon emission computerised tomography and conventional computerised tomography (SPECT/CT) in patellofemoral disorders: a clinical review.

Patellofemoral disorders are common conditions seen in a knee clinic but can present a great diagnostic challenge to the orthopaedic surgeon. Combined single photon emission computerised tomography with conventional computer tomography (SPECT/CT) provides the clinician with precise anatomical and physiological information of the patellofemoral joint. We present a clinical review that highlights the value of SPECT/CT in patients with patellofemoral disorders, where other modalities such as radiographs, MRI, and conventional CT did not provide sufficient information. SPECT/CT has proven to be he

A single measure of patellar kinematics is an inadequate surrogate marker for patterns of three-dimensional kinematics in healthy knees.

Patellofemoral disorders, such as osteoarthritis and patellofemoral pain, are thought to be associated with abnormal patellar kinematics. However, assessments of three-dimensional patellar kinematics are time consuming and expensive. The aim of this study was to determine whether a single static measure of three-dimensional patellar kinematics provides a surrogate marker for three-dimensional patellar kinematics over a range of flexion angles. We assessed three-dimensional patellar kinematics (flexion, tilt and spin; lateral, anterior and proximal translation) at sequential static angles throu

[Clinical examination for patellofemoral problems].

Patellofemoral pain and instability account for 25% of all sports-related knee problems. However, definitive analysis seems to be difficult. Descriptive findings like "anterior knee pain", "patellofemoral pain syndrome", or "chondromalacia" are used instead of diagnosis. One reason may be the complex pathomechanics of the patellofemoral joint which is suspended between the pelvis, the femur, and the tibia and subsequently is influenced by the whole lower extremity from the hip down to the foot. On the other hand, different signs and functional tests are in use with at times lack of quantificat

The reliability and validity of assessing medio-lateral patellar position: a systematic review.

Medio-lateral patellar position is regarded as a sign of patellofemoral pain syndrome and patellar instability. Its assessment is important in accurately performing patellofemoral therapeutic taping techniques. The purpose of this paper is to systematically review the literature to determine the reliability and validity of evaluating medio-lateral patellar position. An electronic database search was performed accessing AMED, British Nursing Index, CINAHL, the Cochrane database, EMBASE, Ovid Medline, Physiotherapy Evidence Database (PEDro), PubMed and Zetoc to July 2008. [Patellofemoral arthroplasty--results of a nation-wide survey in Germany and review of the literature].

BACKGROUND: Isolated patellofemoral arthroplasty is discussed controversially. The aim of this nation-wide survey in Germany and review of the literature was to determine the current status of patellofemoral arthroplasty. METHODS: A standardised questionnaire was sent to 744 German departments of orthopaedic surgery, traumatology and general surgery. In the first part, surgeons were asked general questions about their department size, case numbers of knee arthroplasties per year and non-endoprosthetic treatment of isolated patellofemoral disorders. If patellofemoral arthroplasty was conducted

Q-angle: an invaluable parameter for evaluation of anterior knee pain.

BACKGROUND: Patellofemoral pain syndrome is a descriptive term applied to patients with nonspecific anterior knee pain, and is the most common knee problem. The pain in most patellofemoral disorders is generalized to the anterior part of the knee. One important concept in patellofemoral joint function is the quadriceps angle (Q-angle). Theoretically, a higher Q-angle increases the lateral pull of the quadriceps femoris muscle on the patella and potentiates patellofemoral disorders. This study was undertaken to evaluate the relationship between the anterior knee pain and Q-angle. METHODS: This

Musculoskeletal injuries of the knee: are females at greater risk?

Research has shown that the incidence of anterior cruciate ligament injuries is higher in female athletes than in male athletes. And anecdotal evidence points to a higher incidence of patellofemoral disorders in females versus males. This article reviews what is known about the incidence of these injuries, the reasons for sex differences in injury rates, the mechanisms behind the injuries, and strategies to prevent them.

The influence of the lower patellar pole in the pathogenesis of chronic patellar tendinopathy.

Resection of the lower patellar pole provides good results in the treatment of jumper's knee. Therefore we hypothesized that the length of the lower patellar pole is increased in patients with chronic patellar tendinopathy. Cohort study, level of evidence 2. Between 2000 and 2005, 25 patients with chronic patellar tendinopathy underwent conservative and surgical treatment in our clinic. All of them had preoperative MRI were three independent examiners measured the Caton Index, the length and the ratio of the articular and non-articular patellar surface, tendon length and thickness and the thic

Patellofemoral disorders presenting as bilateral locked knees in achondroplasia.

Patellofemoral disorders in achondroplasia, to our knowledge, have not been reported in the literature. We describe an unusual case of bilateral locked knees in an achondroplastic patient secondary to intra-articular subluxating patellae and symmetrical chondral flap lesions of the femoral trochlea which were treated by arthroscopic resection. We suggest that abnormal kinematics may cause patellofemoral complications in achondroplasia.

Dimorphism and patellofemoral disorders.

Sex is defined as the classification of living things according to their chromosomal compliment. Gender is defined as a person's self-representation as a male or female or how social institutions respond to that person on the basis of his or her gender presentation. One frequently divides the topic or dimorphism into the biologic response inherent in their sex and the environmental response that might be better termed "gender differences." Clinicians have anecdotally agreed for years that patellofemoral disorders are more common in women. Given the difficulty in classifying patellofemoral diso

Special concerns of the female athlete.

The majority of injuries sustained by female athletes are due to participation in sports rather than their sex. Anatomic, hormonal, and functional differences should be understood when treating the female athlete. Screening should be done for eating disorders and the female athlete triad. Female athletes have increased rates of injury to the anterior cruciate ligament and patellofemoral disorders compared with their male counterparts.

Imaging of patellofemoral disorders.

Anterior knee pain is a common symptom, which may have a large variety of causes including patellofemoral pathologies. Patellofemoral maltracking refers to dynamic abnormality of patellofemoral alignment and has been measured using plain film, computed tomography (CT) and magnetic resonance imaging (MRI) using static and kinematic techniques. Patellar dislocation is usually transient, but specific conventional radiographic and MRI features may provide evidence of prior acute or chronic dislocation. In addition, chondromalacia patellae, osteochondritis dissecans, patellofemoral osteoarthritis,

Factors affecting the outcome of distal realignment for patellofemoral disorders of the knee.

This study correlated the risk factors with the clinical outcome of distal realignment for patellofemoral disorders in 48 patients with 53 knees with 25 to 96 months follow-up. The indications for surgery included pain and disability due to patellofemoral disorders with failure of at least 6 months of conservative treatments. The evaluations included pain scores, Lysholm functional scores and radiographs of the knee. The overall results were satisfactory in 47 knees (88.7%) and unsatisfactory in six knees (11.3%). There was no correlation of the clinical results with age, sex, body weight and
