Patella baja after displacement of tibial tuberosity for patellofemoral disorders.

The change of patellar height after the displacement of tibial tuberosity and its effect on the improvement of clinical signs of knees was studied. The anterior displacement of tibial tuberosity was performed on fourteen knees and the anteromedial displacement on twenty knees with patellofemoral arthrosis of twenty-nine patients, four men and twenty-five women with the age at operation ranging from 47 to 79 years (average: 58.3). The anteromedial displacement of tibial tuberosity was performed on ten knees and medial displacement on fifteen knees with recurrent patellar dislocation (eleven kne

Scoring of patellofemoral disorders.

A new questionnaire was used to evaluate subjective symptoms and functional limitations in patellofemoral disorders. The questionnaire was completed independently by four groups of female subjects: controls (N = 17), and subjects with anterior knee pain (N = 16), patellar subluxation (N = 16), and patellar dislocation (N = 19). The questionnaire mean scores for the groups were 100, 83, 68, and 62 points, respectively (p < 0.0001). The items dealing with abnormal painful patellar movements (subluxations) (p < 0.0001), limp (p < 0.0001), pain (p < 0.0001), running (p < 0.0001), climbing stairs (

Role of arthroscopic lateral release in the treatment of patellofemoral disorders.

The results of 50 arthroscopic lateral releases are reported. The average follow-up period was 36 months with a range of 18-52 months. Satisfactory results were achieved in 71% of 21 patients with patellar pain alone and in 76% of those (Betz RR, Lonergan R, Patterson R. The percutaneous lateral retinacular release Orthopaedics 1982;5:57-62) with instability. Patients with patellofemoral osteoarthritis or patellar dislocation were excluded from the series. Postoperative hemarthrosis occurred in 10%. Unsatisfactory results could be related to incomplete release, severe chondromalacia, or insuff

In vivo tracking of the human patella.

The purpose of this study was to describe the dynamic, in vivo, three-dimensional tracking pattern of the patella for one normal male subject. Intracortical pins were inserted into the patella, tibia, and femur. The subject performed seated and squatting knee flexion/extension, and maximum voluntary quadriceps contractions. In addition, the vastus medialis oblique was subjected to maximal electrical stimulation. Motions of the markers attached to the intracortical pins were analyzed using an automated video system. Patellar and tibial motions were determined relative to a femoral reference sys

Patellar band for patellofemoral disorders: results and indications.

In patellofemoral disorders, some cases respond well to conservative management thus the authors' initial treatment is conservative. The Patellar Band (PB) was reported previously (Nakamura et al., 1987). Since then the indications for the band have been investigated. Sixty four patients treated by the PB without operative treatment were classified into eight groups. The Severity of Dysfunction (SOD) was assessed by three grades. The First Grade is dull pain after walking or running for a long distance, the Second Grade is sharp pain on climbing up and down stairs, the Third Grade is a feeling

Effects of variation on extensor elements and operative procedures in patellofemoral disorders.

The influence of both insertion and strength/elasticity of each extensor in patellofemoral disorders was fully investigated through a two-dimensional mathematical model analysis in a horizontal plane, in combination with experimental design theory for analyzing mutually correlated influences. In the model, patellofemoral joint profiles projected on a horizontal plane have been expressed as spline functions. Each muscle of the quadriceps has been represented as a string pulled by the respective force; fascias and tendons have been represented by springs. Nonlinear equations have been constructe

Lateral retinaculum release in adolescent patellofemoral disorders: its relationship to peripheral nerve injury in the lateral retinaculum.

Adolescent patellofemoral disorders which are associated with recognizable change in the articular cartilage of the patella are called chondromalacia patellae. This is a clinical syndrome characterized by persistent retropatellar pain, but not always associated with histopathological changes of the articular cartilage. When lateral retinacular release is performed in such patients, pain is frequently eased even though lateral release does not always cause an appreciable change in patellofemoral contact pressure. This suggests that pain, at times, may emanate from the peripatellar retinacular s

Changes in patellofemoral compressive force after anterior or anteromedial displacement of tibial tuberosity for chondromalacia patellae.

Patellofemoral compressive force was measured before and after anterior displacement of the tibial tuberosity for patellofemoral disorders on 30 knees in 28 patients. All patients selected for this experiment were those who obtained relief of pain in the activities of daily living. Twenty-five women and three men, with an average age of 54 years (range, 21-79 years), were followed an average of 38 months (range, 24-84 months). The maximum strength of knee extension was measured by a myodynamometer with the knee at 45 degrees flexion. Patellofemoral compressive force was calculated using a vect

Effectiveness of orthotic shoe inserts in the long-distance runner.

Five hundred questionnaires were distributed to long-distance runners who had used, or who were using orthotic shoe inserts for symptomatic relief of lower extremity complaints. Three hundred forty-seven (69.4%) responded (males, 71%; females, 29%). The mean age of the respondents was 36 years (range, 15 to 61). The average distance run per week was 39.6 miles (range, 5 to 98). The mean duration for use of the orthotic inserts was 23 months (range, 1 to 96). The predominant (63%) type of orthotic device used was flexible. The presumed diagnoses in the population studied were excessive pronatio

[An experimental study on the etiology of chondromalacia patellae].

Chondromalacia patellae is one of the common causes of patellofemoral disorders in young subjects. It frequently occurs in patients with lateral subluxation of the patella. However, the etiology of this disease is still obscure. The purpose of this study was to elucidate the histological changes of articular cartilage and bone in chondromalacia patellae. Lateral subluxations of the patellae were produced experimentally in forty New Zealand white rabbits. The mean tilting angle of the patellae was 14.2 degrees just after surgery, 10.7 degrees after 6 weeks, and 7.1 degrees after 12 weeks respec

The role of radiography in the evaluation and treatment of common anarthrotic disorders of the patellofemoral joint.

Patellofemoral disorders are among the most common knee problems in the world. Although they are often considered as a group, pathologically the individual disorders are as variable as fingerprints. Treatment of resistant cases requires a detailed understanding of the disorder mechanism to be corrected. Many radiographic techniques, incorporating plain radiography, arthrography, CT, and MRI have been formulated to determine formats of chondromalacia, instability, and malalignment. Many of the techniques reviewed herein reveal distinct abnormalities but do not necessarily provide insight into t

Classification of patellofemoral disorders.

Patellofemoral disorders represent a large portion of the average orthopedist's practice. Despite the improvements in patellofemoral radiographs and arthroscopic diagnostic techniques, these disorders are too frequently misunderstood and frustrating to treat. This report proposes a clinical classification for patellofemoral disorders that, it is hoped, will aid our understanding and improve our results of treatment. A major feature of this classification is the recognition that a developmental and familial abnormality, patellofemoral dysplasia, is the etiology for most patellofemoral disorders

The quadriceps function in patellofemoral disorders. A radiographic and electromyographic study.

The influence of quadriceps contraction on patellar alignment was investigated in tangential views of 31 patients with patellofemoral complaints and compared with 25 controls. The congruence angle decreased significantly following quadriceps contraction in patients with patellar subluxation in contrast to those with chondromalacia and normals. Electromyography (EMG) of the vastus lateralis (VL) and vastus medialis obliquus (VMO) during maximal quadriceps contraction was obtained prior to and after 3 months of isometric quadriceps exercises in 15 knees with patellar subluxation and in 11 with i

Dynamic knee brace in the treatment of patellofemoral disorders.

The efficiency of a knee brace with a 0 degree-30 degrees range of motion was studied in 17 patients with anterior knee pain. Eighteen patients with identical symptoms served as controls. The brace was worn for 6 weeks. Follow-up was performed after 6 weeks and 3 and 12 months. After 6 weeks, 82% of the patients treated with the brace were classified as improved, compared with 44% without it, according to a strict evaluation system. However, after 12 months only 35% of the patients treated with the knee brace could be rated as improved. Despite this marked deterioration, the brace should be co

Patellofemoral pain--a prospective study.

This prospective study confirms that patellofemoral pain arises predominantly in a young population and is more common in females. Patellar malalignment and its progression to patellar subluxation can be managed conservatively with improvement in the majority of cases. Arthroscopic lateral release may be necessary for selected patients with patellar malalignment and subluxation, while more aggressive surgical therapy should be considered in patients with patellar compression syndrome and recurrent dislocation. Resection of symptomatic plicae gave satisfactory results in all patients. Chondroma

The Maquet principle. Anterior displacement of the tibial tubercle in the treatment of patellofemoral disorders: radiologic appearance.

Anterior displacement of the tibial tubercle is a well-accepted orthopedic procedure in the treatment of certain patellofemoral disorders. The radiologic appearance of surgical procedures utilizing the Maquet principle has not been described in the radiologic literature. Familiarity with the physiologic and biochemical basis for the procedure and its postoperative appearance is necessary for appropriate roentgenographic evaluation and the radiographic recognition of complications.

Patellofemoral disorders: physical and radiographic evaluation. Part I: Physical examination.

Patellofemoral disorders present a diagnostic and therapeutic challenge to the orthopedic surgeon. By the sequential application of such basic diagnostic tools as inspection, palpation, and various measurements to a patient in standing, sitting, supine, and prone positions, a reproducible, comprehensive physical examination of the patellofemoral joint and entire lower extremity may be achieved.

Patellofemoral disorders: physical and radiographic evaluation. Part II: Radiographic examination.

Evaluation of the patient with patellofemoral complaints requires a comprehensive physical and radiologic investigation. The tangential view is most helpful in the evaluation of such patellofemoral disorders as chondromalacia, lateral patellar compression syndrome, and recurrent subluxation or dislocation. Commonly utilized tangential views include those described by Hughston, Merchant, Laurin, and Ficat. Multiple calculations available for quantitation of abnormal tangential views include the sulcus angle, congruence angle, lateral patellofemoral angle, patella index, and patellofemoral index

The isolated lateral retinacular release in the treatment of patellofemoral disorders.

Isolated lateral retinacular release was performed on 102 knees in 76 patients. Diagnoses included chondromalacia patellae, patellar compression syndrome, and subluxation and dislocation of the patella. Patients reported that their symptoms improved after isolated lateral retinacular release in 95 of 102 knees. At follow-up evaluation (mean, 14.5 months after surgery), independent observers found no recurrence of dislocation in the 23 knees having had frank dislocations before surgery. In 71% of the knees with limitation of activity before surgery, unlimited activity was tolerated after isolat
