Magnetic Resonance Imaging of Internal Derangements and Other Knee Pathologies in Adult Nigerians.

BACKGROUND: Knee joint pathologies/injuries are one of the most common musculoskeletal complaints in adults worldwide. The aetiologies of knee joint disorders are diverse. Magnetic resonance imaging (MRI) is a sophisticated method of detecting and characterizing knee pathologies. This study was conducted to document the clinical presentation and MRI patterns of knee joint abnormalities in a group of adults in Lagos, Nigeria, and to juxtapose it with reports from other climes. METHODOLOGY: A retrospective hospital-based analysis of the knee MRI of 158 adult Nigerians was conducted in a single

Bone-cartilage mismatch in the medial tibial plateau: A MRI study.

BACKGROUND: Bone-cartilage mismatch is a variation in which the surface curvature of the articular cartilage is incongruent with the curvature of the underlying subchondral bone. The purpose of this study is to investigate the prevalence of this variant in the medial tibial plateau (MTP) and examine potential association with clinical findings and intra-articular derangements using MRI. METHODS: A quantitative and qualitative retrospective analysis of 98 knee MRI studies was performed. Bone and cartilage depths of the MTP were measured to assess bone-cartilage morphology and classified into c

Patellotibial contusions: a rare cruciate ligament injury pattern.

OBJECTIVE: Patellotibial contusions are a recently described contusion pattern in anterior cruciate ligament (ACL) tears. The purposes or our study were to determine if patellotibial contusions are specific to ACL injuries, determine the internal derangements in patients demonstrating this contusion, and suggest a mechanism of injury. MATERIALS AND METHODS: We conducted a retrospective search of radiology reports over a 6-year period to identify patients with patellar edema described on magnetic resonance (MR) imaging. Only individuals with patellar and tibial edema were included in the study

Patellotibial contusions in anterior cruciate ligament tears.

Bone contusions are an important ancillary finding of many knee injuries. Not only are they a source of pain, they may suggest a mechanism of injury or a specific derangement of the knee joint. We have encountered a small number of patients being evaluated for anterior cruciate ligament (ACL) tears with unexplained patellar and tibial edema at magnetic resonance (MR) imaging. We present three individuals with contusions of the inferior patella with a corresponding contusion of the anteromedial tibial plateau. Internal derangements in these patients were similar to other individuals with acute

Anterior knee pain.

Anterior knee pain is a common complain in all ages athletes. It may be caused by a large variety of injuries. There is a continuum of diagnoses and most of the disorders are closely related. Repeated minor trauma and overuse play an important role for the development of lesions in Hoffa's pad, extensor mechanism, lateral and medial restrain structures or cartilage surface, however usually an increase or change of activity is referred. Although the direct relation of cartilage lesions, especially chondral, and pain is a subject of debate these lesions may be responsible of early osteoarthrosis

Synovial knee affected in multiple hemangiomatosis.

Synovial hemangioma is a vascular lesion of joint cavities. It is a rare cause of pain and spontaneous hemarthrosis in the paediatric age group. It is often seen as an internal derangement of the knee. We present the case of a 37-year-old woman with a history of recurrent effusions of this joint. The MRI showed a soft tissue mass affecting the Hoffa's fat pad and the synovial surrounding the patella. Other hemangiomas were detected in other sides of the body. Through arthroscopy, we eliminated all the pathological tissue of the knee. The patient remains asymptomatic after 1 year.

Internal derangements of the knee associated with patellofemoral joint degeneration.

The objective of the present study was to evaluate whether internal derangements of the knee joint are associated with patellofemoral joint (PFJ) degeneration. Data were collected prospectively from 1,000 consecutive knee arthroscopies. Chondral lesions were observed in 854 patients, and these patients were included in this study. Patients' details (age, sex, duration of symptoms, injuries, and possible mechanism of injury), operative details (types and number of portals, equipment used), intra-articular findings (articular, meniscal and synovial lesions, and stability characteristics) and pro

MR imaging of infrapatellar plica injury.

OBJECTIVE: Injury to the infrapatellar plica (ligamentum mucosum) has not been previously described in the radiology literature to our knowledge. This article shows the MR imaging appearance of injury to the infrapatellar plica. CONCLUSION: Injury to the infrapatellar plica is uncommon but should be considered as a potential source of knee pain, especially if no other evidence indicates internal derangement. MR imaging can reveal a typical appearance for infrapatellar plica injury.

Non perforated septum supra-patellaris mimicking a soft tissue tumour.

Plicae synoviales of the knee are well known to present as anterior knee pain but can also cause blocking, clicking, and swelling of the knee mimicking pathology such as meniscal tears or other internal derangements, and even mono-arthritis. Embryologically, the knee is divided into a medial and lateral compartment and a supra-patellar pouch by thin membranes of synovium. Incomplete involution of these membranes gives rise to a plica. When there is no involution at all, the membrane is called a septum. There are three frequent types of plicae/septa: a suprapatellar, a medial-patellar, and an i

Anatomy and MR imaging appearances of synovial plicae of the knee.

Synovial plicae are normal anatomic structures of the knee that sometimes become symptomatic. Magnetic resonance (MR) imaging and MR arthrography are useful tools in the evaluation of synovial plicae and allow differentiation of these entities from other causes of knee pain. At MR imaging, synovial plicae appear as bands of low signal intensity within the high-signal-intensity joint fluid. Gradient-echo T2-weighted and fat-suppressed T2-weighted or proton density-weighted MR images are optimal for the evaluation of plicae. Plica syndrome, the painful impairment of knee function in which the on

Acute lateral patellar dislocation at MR imaging: injury patterns of medial patellar soft-tissue restraints and osteochondral injuries of the inferomedial patella.

PURPOSE: To assess magnetic resonance (MR) imaging findings after acute lateral patellar dislocation (LPD) with emphasis on the medial patella restraints and to describe a medial patellar impaction deformity. MATERIALS AND METHODS: Knee MR images obtained within 8 weeks after LPD were evaluated for medial retinacular and medial patellofemoral ligament (MPFL) disruption, vastus medialis obliquus (VMO) edema and/or elevation, and other derangements. One hundred patients with no evidence of prior LPD were evaluated as controls. The Student t test was used for statistical comparisons. RESULTS: E

Serial magnetic resonance imaging evaluation of operative site after fixation of patellar tendon graft with bioabsorbable interference screws in anterior cruciate ligament reconstruction.

Magnetic resonance imaging (MRI) is accepted as the imaging procedure of choice for showing internal derangement of the knee. In contrast to metal implants, bioabsorbable interference screws do not produce an artifact and provide an opportunity to expand the evaluation of the postoperative anterior cruciate ligament (ACL) ligament repair. There is the potential to evaluate the implant, the graft, the adjacent tissue, and the surgically created bone tunnels. The purpose of this study was to evaluate with MRI the postoperative site of ACL patellar tendon autografts in which bioabsorbable screws

Patellofemoral joint abnormalities in athletes: evaluation by kinematic magnetic resonance imaging.

The patellofemoral joint is one of the most common sources of painful symptoms encountered by athletes and is one of the most frequently injured joints. Patellar incongruency is the primary pathologic condition that affects the patellofemoral joint and has been reported to be associated with patellar subluxation, patellar dislocation, chondromalacia, and arthrosis. The diagnosis of patellofemoral incongruency by physical examination alone is extremely difficult because the clinical signs may stimulate other types of internal derangements of the knee, and there is a high incidence of combined a

Patellofemoral arthralgia, overuse syndromes of the knee, and chondromalacia patella.

Patellofemoral arthralgia is a very common syndrome affecting athletes. Most often, examination fails to define true pathology. Conservative treatment, an active exercise program, and sports may be undertaken without harm to the knee. The patellofemoral arthralgia syndrome must be differentiated from true chondromalacia patella, where there is actual degeneration of the patella's articular cartilage, and from other sources of internal derangement such as meniscal disease or osteochondral lesions. Careful attention to the history of onset, and provoking activities such as climbing stairs, kneel

Patella pain syndromes and chondromalacia patellae.

Patellar pain must be separated from other causes of internal derangement of the knee by a careful history and precise examination followed by appropriate investigations. Once the cause of the pain is determined, malalignment or malposition of the patella must be sought. The Merchant view of the patellar femoral joint is recommended in this regard to demonstrate patellar incongruence. The malalignment syndrome may or may not show the pathologic changes described as "chondromalacia" and respond particularly well after proximal patellar realignment. In the remaining cases, pain may be caused by

The place of arthroscopy in the diagnosis and treatment of internal derangement of the knee: an analysis of 1000 cases.

An analysis of 1,000 consecutive arthroscopies of the knee, representing almost 10 years' experience, has made it possible to identify the patient in whom arthroscopy is deemed essential, very useful, or in some instances, superfluous. In the latter category are the young, often teenagers, with pain as their main symptom and no positive physical findings. In osteochondritis dissecans, added information beyond that supplied by X-rays was obtained infrequently, and routine arthroscopy for chondromalacia of the patella is probably unnecessary. Pain and other symptoms in the older age group are mo

The arthroscope in the diagnosis of disorders of the patellofemoral joint.

Despite our current knowledge and accumulated information, orthopedic surgeons often experience difficulties in the diagnosis of internal derangement of the knee. Too often, the admission diagnosis I.D.K. means "I don't know." Disorders of the patellofemoral joint contribute to this difficulty and may simulate torn menisci and loose bodies, as well as the instability which results from rotary displacement. Patients often have difficulty describing and locating their symptoms, and 2 or more pathologic lesions may be present in the same joint. Early lesions involving the articular cartilage are

Arthritis of the canine stifle joint.

A survey of cadaver material was undertaken in order to determine the prevalence of arthritis of the canine stifle joint. One hundred and fifty unselected cadavers were obtained from veterinary practices for this purpose, and their stifle joints were radiographed and dissected to discover abnormalities. Thirty-one dogs (20 per cent) had stifle arthritis and in all except one the lesions were characteristic of osteoarthritis. In the exceptional case infection was the cause. A mechanical derangement was present in 20 of the 54 affected joints, in the form of rupture of the anterior cruciate liga
