The authors examine the diagnostic value of two non-invasive techniques--i.e. ultrasound and computerized teletermography--in the diagnosis of patellar enthesopathies. Such a pathology is quite frequent in young athletes, at various stages. The patients were grouped in two categories, according to their age when the trauma occurred, and to the peculiarities of both enthesopathic localization and imaging. The use of both techniques, either one prevailing over the other according to the different evolutive phases of enthesopathies, provides useful information as to detecting and determining the

A 26-year-old man incurred bilateral spontaneous rupture of the patellar ligament. This may be the eighth case in previously healthy individuals reported in the literature. Further review of other case reports suggested various etiological factors. Surgical repair generally produces excellent results.

Patellofemoral disorders represent a large portion of the average orthopedist's practice. Despite the improvements in patellofemoral radiographs and arthroscopic diagnostic techniques, these disorders are too frequently misunderstood and frustrating to treat. This report proposes a clinical classification for patellofemoral disorders that, it is hoped, will aid our understanding and improve our results of treatment. A major feature of this classification is the recognition that a developmental and familial abnormality, patellofemoral dysplasia, is the etiology for most patellofemoral disorders

The case of an 8-year-old boy with tuberculous arthritis of the right knee in whom patellar disease was the only radiological abnormality is presented. The patellar findings were a large lytic area with sclerotic borders containing a dense image corresponding to a sequestrum. Open biopsy was necessary for diagnosis, since the radiological abnormalities are consistent with other chronic infectious diseases.

Twenty-eight patients with chronic abnormalities of the knee were examined by 3-phase scintigraphy and ECT. The unobscured images produced by ECT provide additional information for evaluating the localisation, extent and activity of the lesions. The diagnosis of meniscus abnormalities in particular revealed pathognomonic patterns. The indications and value of ECT was compared with other diagnostic measures and particularly with invasive techniques (arthroscopy and arthrotomy).

We describe a lumbar facet syndrome in which disabling symptoms are associated with normal or near-normal plain radiographs. Local spinal fusion relieved symptoms in 12 patients; the excised facet joint surfaces showed some of the histological changes seen in chondromalacia patellae and in osteoarthritis of other large joints. The most frequent change was focal full-thickness cartilage necrosis or loss of cartilage with exposure of subchondral bone, but osteophyte formation was remarkably absent in all specimens. We suggest that there are both clinical and histological similarities between the

A case of hemihypertrophy associated with multiple anomalies of the skin, bone and visceral organs is presented. A 31-year-old female was admitted for evaluation of her skin conditions. Her family history is noncontributory, while her past history discloses operations for syndactyly of the right foot, tonsillar hypertrophy, anal prolapse and ovarial cyst. Erythemas of the face and the left upper extremity were noticed during the neonatal period and hypertrophy of the right side of the body started at age 2 months. On admission, hemihypertrophy was observed in the face, trunk and extremities. M

Fibular aplasia and/or hypoplasia is documented as a developmental field defect and the extent of the fibular developmental field is delineated. The term fibular a/hypoplasia denotes the clinical spectrum of fibular deficiency in different patients and also implies that aplasia can be present in one limb and hypoplasia in the other. Causal heterogeneity of fibular a/hypoplasia is demonstrated, thereby defining it as a developmental field defect. Most cases of fibular a/hypoplasia are isolated, sporadic events. An autosomal dominant form of isolated fibular a/hypoplasia with ankle joint anomaly

From a series of 72 patients treated with anterior advancement of the tibial tubercle for patellofemoral pain syndrome, secondary lowering of the patella was noted in eight. In all eight patients the results were poor following surgery, with severe disabling retro- and peripatellar pain and atrophy of the quadriceps muscle. We describe this complication and illustrate and discuss the biomechanical consequences. The shortening of the patellar ligament and lever arm of the ligament were determined from radiographs of the knee joints before and after surgery. A theoretical analysis was made of th

Thirty-nine school-age children with Down's syndrome (trisomy 21) were examined for orthopedic problems and evidence of joint laxity. Of these, 10.3% had required orthopedic surgery for feet and hip problems, and 13% had disorders not yet requiring surgery. Only two children could be regarded as showing unequivocally abnormal generalized joint laxity. Twenty-three of the children had one or more lax joints, and one had a dislocated hip without laxity of other joints. Six children had hypermobility of the patella, but this did not correlate with joint laxity. Fifteen children had no evidence of

Dysplasia epiphysealis hemimelica - an epiphyseal developmental disturbance of the skeleton - is combined with exostose-like, tumor-simulating cartilaginous hypertrophy of bone tissue, mainly located at the epiphyses of the lower extremities and at the tarsal bones. In the case of multiple involvement the unilateral type prevails (hemimele type); within the epiphyses it is mostly the medial part which is undergoing changes. Rare sites are patella, head of the hip bone, shoulder joint, carpalia, more commonly affected are talus as well as other tarsal bones, distal femoral epiphysis and proxima

Surgical reinforcement of the knee extensor mechanism produces improvement in gait in children with cerebral palsy who walk with a crouch gait with flexion of the hips and knees. In these patients, the defect is related to excessive length of the patellar tendon as shown by the presence of greater passive extension than active extension of the knee, and a high position of the patella on lateral radiographs of the knee. Surgical treatment combines shortening of the patellar tendon with lengthening of the hamstrings and tenotomy of the rectus femoris. In a series of eighteen patients, the author

A 34-year-old Nigerian man had successive ruptures of the ligamenta patellarum repaired 2 years after the first injury. One knee was injured at a soccer match, the other 5 months later subsequent to a minor fall. There was no evidence of systemic disease. In spite of delay in effecting treatment, the outcome was satisfactory.

Six boys with fragmentation of the proximal pole of the patella are reported; the condition was bilateral in one. Four of the six presented with symptoms of Osgood-Schlatter's disease or Larsen-Johansson disease of the same or of the contralateral knee, but they had no symptoms or signs relating to the proximal pole of the patella; one also had features suggesting minimal chondromalacia patellae. Two boys had no objective abnormality in either knee. It is suggested that the fragmentation may be a further form of traction osteochondritis of the attachments of the quadriceps mechanism. Attention

Patellofemoral arthralgia is a very common syndrome affecting athletes. Most often, examination fails to define true pathology. Conservative treatment, an active exercise program, and sports may be undertaken without harm to the knee. The patellofemoral arthralgia syndrome must be differentiated from true chondromalacia patella, where there is actual degeneration of the patella's articular cartilage, and from other sources of internal derangement such as meniscal disease or osteochondral lesions. Careful attention to the history of onset, and provoking activities such as climbing stairs, kneel

The mechanism of patellar pain is still not fully understood. Many operative procedures have been proposed, but so far no single technique has been successful. Under the impression that some of the pain-relieving effect by operative treatment is due to partial denervation of the patella, a medial denervation of the patella was performed in 17 patients (20 knees), mean age 24 years, with patellar pain syndrome. At the follow-up only 11 knees were free of symptoms or had improved, and the authors conclude that though comparable with other surgical methods, medial neurotomy is not sufficient to e

We present a 9-year-old boy, the product of a consanguineous marriage, with proteinuria, edema, and microscopic hematuria with mild renal impairment since age 2 yr. Renal biopsy showed the histopathologic electron microscopic changes seen in hereditary osteo-onchyodysplasia (HOOD) [Hoyer et al, 1972; Bennet et al, 1973; DelPozo and Lapp 1970; Vernier et al, 1974; Morita et al, 1973] or "nail-patella syndrome." Neither the patient nor his relatives had bone or nail abnormalities; however, 2 other sibs had died of complications of end-stage renal disease at age 6 and 9, leading us to suspect aut

A variety of names has been given to disorders of the inferior pole of the patella occurring in young athletic individuals and several different causes have been proposed for these disorders. Occasionally a direct blow will cause fracture of the inferior pole of the patella, but the only other mechanism which seems to be responsible is subluxation or dislocation. Ten cases of distal patellar pole fracture secondary to dislocation or subluxation of the patella are reported. From these cases and an extensive review of the literature it is concluded that patellar subluxation or dislocation is the

Frequency of rare skeletal lesions in Paget's disease is compared with normal localisations. Radiological morphology of rare localisations of Paget's disease of the scapula, of the lower arm bones, of the hand bones, of the patella and the calcaneus are described. Thickening of trabeculae in the spongy bone and porosity of compact bone, deformation and enlargement of the bones are typical; this has to be differentiated and related to a small number of other bone diseases.
