The treatment of patellar instability by lateral release.

A simple procedure of using a Smillie knife to release the lateral patellar retinaculum and the capsule in the unstable patella syndrome is described. The syndrome includes recurrent dislocation, recurrent subluxation and acute dislocation of the patella. Arthroscopy, to exclude any other pathology such as chondromalacia patellae, is performed before proceeding to closed lateral release. The results of this procedure are as good as those after more major surgical procedures. In a small percentage of patients a second operation such as tibial tubercle transfer or patellectomy may be necessary,

The medial patellar plica syndrome.

A retrospective review of all patients diagnosed at arthroscopy as having a medial patellar plica with no other intraarticular pathology was conducted. Characteristic clinical findings, including pseudolocking, were identified. The clinical response of 68 patients (78 knees) to shelf resection was noted at 3 and 6 months. The 6-month review demonstrated 60% to be totally relieved of their preoperative pain and 31% with occasional discomfort, but capable of full activities. Three patients were graded with poor results (4%) due to limitations in their work and sports, and four patients were lost

[Extraspinal erosions of tendon insertions in ankylosing spondylitis].

A study based on the clinical observation of 9 adults carrying the HLA B27 antigen. Seven of these patients had a typical ankylosing spondylitis with bilateral sacro-iliitis. The other two cases were considered to have a minor form of the disease, without sacro-iliitis. In various zones of tendinous or ligamentous insertion (on the calcaneal and ischial tuberosities, the anterior tibial tuberosity, the patella, the lateral femoral condyle, the metataŕso-phalangeal epiphyses of the index finger, the head of the humerus), the patients complained of pain and presented radiological images of more

Arthralgia in children.

Arthralgia is joint pain unaccompanied by obvious clinical signs of arthritis or trauma. In most children and adolescents, the affected joint is the knee, hip, ankle, or less commonly an arm joint. Causes of arthralgia include arthritis; systemic disease; tumor; infection; growing pains; transient synovitis of the hip; osteochondroses; ostochondritis dissecans; traction syndrome; chondromalacia of the patella and post-traumatic synovitis. Some pains can be diagnosed with confidence with history, examination, X-ray, and laboratory studies. Other pains are vague, but careful observation of wasti

The percutaneous lateral retinacular release.

A lateral retinacular release performed through a 2 ½-3 cm lateral parapatella incision can give rewarding results for patients with chondromalacia, degenerative joint disease, recurrent subluxation or recurrent dislocation of the patella. A retrospective review from August 1978 to December 1980 of 37 patients (42 knees) shows 83% excellent or satisfactory results, 17% unsatisfactory results with an average postoperative follow-up of 12.3 months. Preoperative pain was reduced in all but two patients; no patient was made worse by the procedure with respect to pain or function. In 17 knees with

The painful bipartite patella.

Two skeletally immature patients with bipartite patella, a lesion normally classified as a developmental variation, proved to have a symptomatic lesion with a traumatic etiology. In both patients, appropriate treatment resulted in complete subsidence of symptoms; one patient was treated with cast immobilization, while the other eventually required surgical resection. In addition, normal and bipartite patellar specimens from skeletally immature cadavers were histologically and morphologically assessed. These clinical and anatomic studies suggest that in some cases a bipartite patella may be a t

Conservative treatment of patellar dislocations. Influence of evident factors on the tendency to redislocation and the therapeutic result.

The factors predisposing to recurrent dislocation and the results of conservative treatment were investigated in 79 cases of patellar dislocation. Patella alta and subluxation were significantly more common in patients without than in those with a history of injury. No single predisposing factor caused more redislocation than any other. The tendency to redislocation was significantly greater in patients younger than 20 years of age than in those older than 20 years of age at the time of primary dislocation. Corroborating reports in a large literature on the disorder, the present experience val

Orthopedic disorders in patients with Down's syndrome.

We have contrasted 161 patients with Down's syndrome admitted to three acute care centers with 107 patients in an institutional setting, outlining their major orthopedic problems and suggesting methods of management based on acute care experience. Severe hallux valgus, metatarsus primus varus, subluxation an dislocation of the patella, severe pes planus, atlantoaxial subluxation, femoral head epiphysiolysis, and dislocation or subluxation of the hips remain the most critical problems against a background of other less common orthopedic abnormalities.

Anatomic separation of the suprapatellar pouch spares its involvement by rheumatoid synovitis in the knee.

Synovectomy of the knee was performed in a 27-year-old female with seropositive rheumatoid arthritis of 6 years duration. At surgery the suprapatellar pouch was found to be separate from the rest of the knee joint, where direct access to articular collagenous tissues remained. The suprapatellar pouch, presumably separate from the knee cavity since birth, was free of disease. The rest of the joint lining, and synovium of the forefoot joints, that had been operated on at the same time, had gross and histological evidence of severe rheumatoid synovitis. The sparing of this suprapatellar pouch by

Experimentally induced thiamine deficiency in beagle dogs: clinical observations.

Twenty-three 2- to 5-month-old Beagle dogs were fed a purified thiamine-deficient ration (2 to 3 micrograms of thiamine/100 g of ration) at a rate of 40 to 70 g/kg of body weight/day depending on age. Eleven dogs were used as principles, 6 as pair-fed controls, and 6 as ad libitum-fed controls. Controls were treated once a week with an IM dose of 300 micrograms of thiamine hydrochloride/kg of body weight. Three stages of clinical disease occurred in the principals: (i) an initial short (18.0 +/- 7.9 days) stage of induction, during which the dogs usually grew suboptimally, but were otherwise h

Clinical characteristics of patellar disorders in young athletes.

The clinical histories of 50 young athletes who complained of patellar instability or pain were analyzed, and their physical findings were compared to those of 50 controls. The patients were divided into three groups, based on their symptoms. Analysis of their physical findings confirmed the validity of these divisions. Those who complained of frank dislocation of the patella exhibited the most pronounced stigmata of quadriceps dysplasia, including vastus medialis deficiency and infrapatellar fat pad enlargement, and had increased general ligamentous laxity, increased mean patellar mobility, a

Dynamic patellar brace: a new orthosis in the management of patellofemoral disorders. A preliminary report.

The diagnosis and treatment of patellofemoral dysfunction, especially subluxation of the patella, remains controversial. Many authors feel conservative treatment will give better long-term results than operative treatment. To aid in nonoperative treatment of patellofemoral conditions a dynamic patellar stabilizing brace has been developed. The brace applies an active, medially displacing force to the lateral border of the patella and maintains constant pressure during flexion, extension, and rotation of the knee. This brace consists of an elastic sleeve with a patellar cutout and two circumfer

Patella pain syndromes and chondromalacia patellae.

Patellar pain must be separated from other causes of internal derangement of the knee by a careful history and precise examination followed by appropriate investigations. Once the cause of the pain is determined, malalignment or malposition of the patella must be sought. The Merchant view of the patellar femoral joint is recommended in this regard to demonstrate patellar incongruence. The malalignment syndrome may or may not show the pathologic changes described as "chondromalacia" and respond particularly well after proximal patellar realignment. In the remaining cases, pain may be caused by

Patellar tendon rupture with underlying systemic disease.

Two patients with patellar tendon rupture and underlying systemic disease are described. One had systemic lupus erythematosus and the other had chronic renal failure and secondary hyperparathyroidism. There have been 13 other cases of patellar tendon rupture in patients with systemic disease reported in the literature and they are reviewed. Patellar tendon rupture has occurred spontaneously in more than one-half of the cases and has been associated with rupture of the contralateral extensor tendon mechanism in 87%. Although patellar tendon rupture almost always occurs at the tendinous insertio

[The patella and the femoro-patellar joint during rheumatoid polyarthritis].

The authors report 2 series of cases of rheumatoid arthritis, one prospective of 115 cases, the other retrospective of 72 cases, and note the frequency of clinical and radiological patellar and femoro-patellar involvement during this disease. Signs of active rheumatoid disease in the patella were present in 31 cases. The most common lesions of the femoro-patellar joint space are narrowing and lateral dislocation of the patella. The femoro-patellar lesions evolve in parallel to the femoro-tibial lesions, but dissociation is possible. The early detection of a femoro-patellar syndrome permits eff

Proximal "tube" realignment of the patella for chondromalacia patellae.

In young people complaining of patellar pain there is often an anatomic basis (an increased quadriceps angle or a high-riding patella). Forty-eight knees with one or other of these abnormalities underwent proximal quadriceps realignment. The results were excellent or good in 94%. Shaving of chondromalacic cartilage was also done in 11 knees but did not seem to improve the results, and is not now recommended except for "blister" lesions. Extensor mechanism dysplasia is an etiologically correct name for the disorder and therefore preferable to "chondromalacia patellae."
