Analysis of Complications Associated with Basivertebral Nerve Ablation for Vertebrogenic Low Back Pain: A Multicenter Retrospective Cohort Study of 1,145 Patients.

OBJECTIVE: Low back pain (LBP) is a leading cause of pain and disability worldwide. Recent studies demonstrate a vertebrogenic model involving the basivertebral nerve (BVN) as a cause of chronic LBP. Vertebral endplates are highly innervated by the BVN, and literature shows type 1 and 2 Modic changes at vertebral endplates are a source of vertebrogenic low back pain (VBP). As a result, intraosseous radiofrequency ablation of the BVN is a minimally invasive outpatient procedure used to treat VBP. We conducted a retrospective review of 1,145 patients that have undergone BVN ablation to assess fo

Pedicle Screw-Aortic Interaction: A Single Centre Experience and Proposal for a Morphology Based Management Algorithm.

OBJECTIVE: Pedicle screw (PS) fixation is widely used in thoracolumbar spine surgery. Although major vascular injury is uncommon, PS-aortic interactions may lead to severe complications, and optimal management remains poorly defined. This study proposes a morphology based algorithm to guide multidisciplinary management of PS-aortic interactions and reports a single centre experience. METHODS: All consecutive patients referred to a vascular surgery unit between 2020 and 2025 for PS interaction with the descending thoracic aorta were reviewed retrospectively. Pre-operative assessment was perfor

Integrated Network toxicology and Transcriptomics Uncover that the Involvement of NF-κB/NLRP3 Pyroptosis Axis in Mediating BaP-Induced Hepatotoxicity and Fibrosis.

Benzo[a]pyrene (BaP), a ubiquitous environmental pollutant, exerts hepatotoxicity primarily through its metabolite BPDE. This study combined network toxicology and experimental validation to delineate the mechanisms of BaP-induced liver injury and fibrosis. Network analysis identified ICAM-1 as a potential core target, and previous clinical data confirmed its upregulation in liver injury patients. In a rat model, BaP exposure induced dose-dependent hepatic injury and oxidative stress. Mechanistically, BaP activated the NF-κB/TNF-α signaling axis, upregulating ICAM-1 and initiating NLRP3 inflam

Epigenetic Reprogramming in Cadmium-Associated Liver Injury: Evidence, Mechanistic Insights, and Therapeutic Perspectives.

Chronic cadmium (Cd) exposure is associated with adverse liver outcomes, including metabolic dysfunction-associated steatotic liver disease (MASLD), inflammation, and fibrosis, yet the mechanisms underlying persistent hepatic injury remain incompletely resolved. This review critically examines population-based and experimental evidence for Cd-associated epigenetic alterations, focusing on DNA methylation, histone modifications, N6-methyladenosine (m6A) RNA modification, and non-coding RNA regulation, and evaluates the extent to which these changes represent epigenetic dysregulation or remodeli

Does the mini-open technique still have a role in the modern management of pediatric tibial eminence fractures?

BACKGROUND: Tibial eminence fractures are uncommon pediatric injuries that often require surgical fixation. Although arthroscopic fixation is widely adopted, the role of mini-open physeal-sparing techniques remains incompletely defined. This study evaluated the medium- and long-term clinical and radiographic outcomes of pediatric patients treated with this technique for displaced tibial eminence fractures. METHODS: A retrospective study was conducted on 26 consecutive patients (mean age 12.1 ± 2.3 years) treated between 2017 and 2019 with a mini-open physeal-sparing transosseous suture techni

Two Cases of Conservative Treatment for Fractured Trapezial Tubercles.

Among non-scaphoid carpal fractures, trapezium fractures are uncommon but rank as the third or fourth most frequent. A few international studies suggest that these fractures represent 1.5-3% of fractures of the hand. There are very few reports on the conservative treatment of patients with this type of injury owing to various complications. Herein, we present two cases of trapezial tubercle fractures managed conservatively with thumb spica casting. In Case 1, a 52-year-old male lost control and fell from his motorcycle, resulting in an injury to his right hand. On the following day, he present

Impact of operative timing for proximal humerus fracture dislocations on development of neurologic injury.

PURPOSE: Operative management of proximal humerus fractures with glenohumeral dislocation is often delayed until a shoulder specialist is available. This study aims to assess whether time from presentation to operative intervention impacts incidence of neurologic injury in the preoperative waiting period. METHODS: Retrospective review was performed for patients undergoing operative management of a proximal humerus fracture dislocation at a level 1 trauma center. Patient charts were reviewed for patient factors, injury characteristics, neurologic status, and postoperative complications. Statis

Abdominal aortic calcification and skeletal outcomes in hospitalized patients with surgically treated fragility fractures: a retrospective cohort study.

UNLABELLED: Abdominal aortic calcification was associated with lower femoral neck BMD in unadjusted analyses, but its independent skeletal stratification value was limited in hospitalized patients with surgically treated fragility fractures. AAC was not independently associated with fracture severity or refracture risk in this cohort. PURPOSE: Abdominal aortic calcification (AAC) has been linked to lower bone mineral density (BMD) and fracture risk in community-based populations, but its relevance for skeletal stratification after surgically treated fragility fracture is unclear. We examined

Post-transfusion purpura: pathogenic mechanisms and relationship with immune thrombocytopenia: a critical analytical review.

Post-transfusion purpura (PTP) is a rare, life-threatening immune-mediated adverse transfusion event. It is characterized by severe thrombocytopenia developing 5-10 days after transfusion of platelet-containing blood products. The most notable pathological paradox is that alloantibodies directed against foreign human platelet antigen (HPA) can destroy the patient's autologous platelets that do not express the corresponding target antigen. PTP predominantly affects middle-aged women, and its major clinical manifestation is mucocutaneous bleeding, with intracranial hemorrhage being the leading c

Serum osteocalcin is associated with scintigraphic abnormalities in lame and clinically sound racehorses.

BACKGROUND: Musculoskeletal injury is a major welfare and economic challenge in racehorses, with stress-related bone injury contributing to training interruption and catastrophic failure. Advanced imaging modalities can detect early skeletal pathology but remain limited in routine screening environments. Circulating biomarkers of bone turnover may provide complementary indicators of skeletal remodelling but require validation against objective imaging findings. OBJECTIVES: Evaluate whether osteocalcin (OC) and C-terminal telopeptide of type I collagen (CTX), or the CTX/OC ratio, are associate

Fetuin-A improves knee osteoarthritis by regulating macrophage PD-1 expression to balance M1/M2 polarization.

Knee osteoarthritis (KOA) features cartilage injury, synovial inflammation and abnormal mineralization. Fetuin-A exerts immunomodulatory and anti-mineralization effects, yet its role in KOA remains unclear. This study aimed to clarify whether Fetuin-A alleviates KOA and its underlying mechanism. We established destabilization of the medial meniscus (DMM)-induced KOA mouse models divided into Sham, Model, Fetuin-A, and Fetuin-A+anti-programmed cell death protein 1 (PD-1) groups. LPS-stimulated bone marrow-derived macrophage conditioned medium (BMDM-CM) system was constructed, with BMDMs grouped

Long-term functional outcome after conservative management of a fingertip amputation with exposed bone, using a semi-occlusive dressing: a case report.

INTRODUCTION AND IMPORTANCE: Semi-occlusive dressings are established for selected fingertip amputations with exposed bone. Because this treatment is well described, the value of single-case reports depends on documenting the course, limitations, and long-term sequelae. CASE PRESENTATION: A 47-year-old right-hand-dominant man presented to primary care after a mandoline-slicer injury causing partial amputation of the right little fingertip, with an exposed distal phalanx and a preserved nail bed, clinically consistent with an Allen type II injury. A transparent, semi-occlusive polyurethane dre

Anterior fix-and-replace strategy using direct anterior and pararectus approaches for acetabular fractures in older patients.

BACKGROUND: Most reported fix-and-replace procedures use a posterior or lateral approach for total hip arthroplasty (THA) and a Kocher-Langenbeck, anterior intrapelvic, or ilioinguinal approach for acetabular fixation, which may require separate anterior and posterior exposures. Evidence on fully anterior, single-position workflows combining anterior acetabular fixation with THA remains limited. AIM: The aim of this study was to assess the outcomes of acute THA through the direct anterior approach (DAA) combined with pararectus in patients considered suitable for a fix-and-replace strategy.

Recurrent laryngeal nerve monitoring and postoperative airway complications following thyroid surgery: A multivariable national surgical quality improvement program analysis.

BACKGROUND: Recurrent laryngeal nerve injury during thyroid surgery can result in clinically significant airway compromise, particularly in patients with limited pulmonary reserve. Although intraoperative nerve monitoring is widely used to reduce nerve injury, its association with postoperative airway complications has not been well characterized. This study evaluates trends in intraoperative nerve monitoring and its relationship with postoperative airway compromise after thyroidectomy. METHODS: We performed a retrospective cohort study of adult patients who underwent thyroid surgery between
