This article reviews the variety of imaging modalities that are currently being used to evaluate the knee. Nuclear scintigraphy is discussed with emphasis on prosthesis abnormalities. Sonography is discussed with regard to the evaluation of popliteal masses. The uses of computed tomography, especially in the evaluation of the tibial plateau fracture, are discussed, and the role of fluoroscopy, computed tomography, and sonography in image-guided needle procedures are reviewed. Emphasis is placed on the role of MR imaging in knee imaging, with attention to internal derangements, bursal and capsu

OBJECTIVE: The theory of internal derangement of the temporomandibular joint involves the anterior displacement of the meniscus, which is thought to be the result of hyperactivity of the lateral pterygoid muscle. There is, however, a great deal of confusion with respect to the attachment of this muscle to the meniscus. This anatomic study reexamines the meniscal attachment of the lateral pterygoid muscle, in particular, the upper head of this muscle.
STUDY DESIGN: The anatomic relationship between the lateral pterygoid muscle and the meniscus was examined in the postmortem specimens of 25 mal

OBJECTIVES: To investigate the role of complement cascade induced damage and protection against it in acute arthritides compared to rheumatoid arthritis and other chronic joint derangements.
METHODS: C3c, C9, and protectin (CD59) were examined by avidin-biotin-peroxidase complex staining.
RESULTS: Marked deposits of C3c and C9 were found in synovial vasculature and intercellular matrix of the lining in rheumatoid arthritis and in acute arthritides (including bacterial, reactive, and osteoarthritis flare up). Furthermore, protectin was not visible in synovial lining cells and was relatively w

RATIONALE AND OBJECTIVES: We compared the performance of fast spin-echo (FSE) with conventional spin-echo (CSE) magnetic resonance (MR) imaging sequences in the detection of meniscal tears.
METHODS: Seventy-three patients underwent MR examination of the knee for suspected internal derangement. Each patient was scanned with a CSE sequence and one of two FSE sequences. The primary difference between the two FSE sequences consisted of the echo train length (ETL). Thirty-seven patients (group 1) were scanned with the FSE I sequence (ETL = 8), and 36 patients (group 2) were scanned with the FSE II

The inherent vulnerability of the knee to injury and degenerative change means that there is a major requirement for an imaging technique capable of assessing such damage accurately, rapidly and non-invasively. Over a relatively short time magnetic resonance imaging (MRI) has emerged as the technique of choice in the assessment of internal derangement of the knee. The present review describes the wide application of MRI to the knee and considers the position of MRI in relation to other diagnostic methods.

The temporomandibular joint (TMJ) status of 51 juvenile orthodontic patients was assessed with magnetic resonance imaging (MRI), clinical examination, and questionnaire data. The results of this study demonstrated that the prevalence of anterior displacement of the meniscus was 11.8% (6 of 51) as assessed by MRI. Clicking or pain in the TMJ area was found in 9.8% (5 of 51) of the subjects by clinical exam, and 19.8% (10 of 51) of the subjects had a history of pain or clicking of the TMJ. Three subjects had a positive MRI and a negative history and clinical examination. However, all subjects wi

A prospective study involving 190 patients presenting with symptoms suggesting internal derangements of the knee was performed to demonstrate the likelihood of symptoms and signs produced by medial patellar plica to mimic those of medial meniscal lesions. Arthroscopic examination was done in each patient by an independent examiner after the establishment of clinical diagnoses. Only patients with isolated medial patellar plica (Group A, n = 54) and isolated medial meniscal lesion (Group B, n = 19) were further assessed. Although isolated medial patellar plica can produce symptoms and signs simi

MRI has assumed a major role in investigating disorders of the knee, especially internal derangement. We correlated the MRI report and arthroscopic findings of 18 patients with suspected meniscal or ligament injury. The results are presented and compared with other series. A particularly important asset of MRI is its very high negative predictive value.

63 children underwent diagnostic arthroscopy of the knee carried out under general anaesthesia, 46 of them presenting an acute haemarthrosis, 17 because of recurrent pain of effusion that developed under remobilisation stress two or more weeks after the accident. Arthroscopy provides--in addition to radiographs and stability test--complete evaluation of internal derangement of the knee. Clinical diagnosis based on examination and radiographs alone was confirmed by arthroscopy in only 25% in our series. Several arthrotomies would have been performed following "safe" clinical diagnosis, but coul

Derangement of the caudal cruciate ligament was identified in 11 broiler fowls aged between 18 and 38 weeks. In some cases, the caudal cruciate ligament alone was ruptured, whereas, in others, the caudal meniscofemoral and meniscotibial ligaments and medial meniscus were also deranged. In some birds, ligaments were only partially ruptured but, in 5 cases, complete rupture had occurred in both knee joints. All birds showed severe lameness which, in 2 cases, could only be ascribed to caudal cruciate derangement. Degenerative changes were present in many of the ligaments which suggested that rupt

There four indications for arthroscopy in patients with ruptures of the anterior cruciate ligament: (1) assessment of the acutely injured knee, which will usually have a hemarthrosis; (2) assessment of the knee with late symptoms following ligament injury; (3) a preliminary to ligament reconstruction to assess the integrity of the menisci; (4) the assessment of patients who have symptoms following ligament reconstruction. Many patients achieve a good result after arthroscopic correction of meniscal lesions and other derangements without ligament reconstruction. Repair of all acutely ruptured a

The acceptance of arthrography of the knee as an accurate diagnostic procedure by both radiologists and orthopaedic surgeons in the United Kingdom has been slow compared to many other parts of the world. The value of arthrography in the diagnosis of meniscal lesions is discussed with reference to the first 1000 documented cases in the authors' series. It is considered that the results obtained from such early experience is more relevant to those about to start as an arthrographic service. Problems in clinical, arthrographic and arthroscopic diagnosis are discussed. Arthrography constitutes an

1. Anterior central detachments do satisfactorily as outlined. 2. However, if synovial meniscal detachments can be subluxed into the tibiofemoral junction, they should be sutured to their synovial ligamentous beds. 3. Both types of peripheral detachment coexist frequently with other intra-articular derangements, which are usually minor except when the anterior cruciate ligament is involved.

Despite our current knowledge and accumulated information, orthopedic surgeons often experience difficulties in the diagnosis of internal derangement of the knee. Too often, the admission diagnosis I.D.K. means "I don't know." Disorders of the patellofemoral joint contribute to this difficulty and may simulate torn menisci and loose bodies, as well as the instability which results from rotary displacement. Patients often have difficulty describing and locating their symptoms, and 2 or more pathologic lesions may be present in the same joint. Early lesions involving the articular cartilage are
