Case Report: Arthroscopic synovectomy and loose body removal for charcot knee in an adolescent with congenital insensitivity to pain with anhidrosis.

Charcot neuroarthropathy (CN) of the knee in adolescents with congenital insensitivity to pain with anhidrosis (CIPA) is exceptionally rare and presents unique diagnostic and therapeutic challenges. We present the case of a 13-year-old girl with genetically confirmed CIPA and a two-year history of recurrent knee swelling, effusion, and functional limitation. Imaging revealed intra-articular loose bodies and osteochondral defects in the left knee. The patient was treated with arthroscopic synovectomy and removal of a large loose body via standard anteromedial, anterolateral, and posteromedial p

Synovial Lipomatosis of Ankle with Sural Nerve Neuropathy: A Rare Case Report.

Synovial lipomatosis, also known as lipoma arborescens, is a rare benign condition characterized by villous fatty proliferation of the synovium with replacement of the subsynovial tissue by mature adipocytes. Although it predominantly affects the knee joint, involvement of the ankle is distinctly uncommon. We report the case of a 52-year-old woman who presented with progressive left ankle pain and swelling of 1 year duration, associated with mechanical symptoms and restriction of motion. Physical examination revealed a posterolateral ankle swelling with mild limitation of range of motion, whil

Rotating Hinge Revision Total Knee Arthroplasty With and Without Porous Cone Adjunct Fixation.

BACKGROUND: As the number of revision total knee arthroplasties (rTKAs) continues to rise, there is increasing interest in the use of contemporary rotating hinge prostheses. These devices often incorporate porous cones to fill bone defects and enhance long-term fixation. This study evaluated the clinical and functional outcomes and survivorship in rTKA patients utilizing a rotating hinge prosthesis with flexible titanium (FT) cones, porous tantalum (PT) cones, or no cones. METHODS: From December 2014 to July 2023, 251 patients who underwent rTKA with a rotating hinge prosthesis were identifie

Robotic-Assisted Medial Unicompartmental Knee Arthroplasty in an Outpatient Setting.

There were 63 outpatient medial unicompartmental knee arthroplasties (UKAs) performed by Mako robotic assistance by six surgeons. There were 40 men and 23 women who had a mean age of 65.1 years (range, 38 to 80). Their mean body mass index (BMI) was 29.6 (range 21.8 to 39.2), and 28 patients had a BMI over 35. Overall, patient perceptions of the procedure were excellent (mean 9.2 rating of 10), which was not different by sex or body mass index. There were no 30-day complications or readmissions. At one year, there was one case revised for maltracking, resulting in a revision rate of 1.6% (1 ou

Arthroscopic management of knee synovial chondromatosis: a systematic review of outcomes and recurrence.

BACKGROUND: Knee synovial chondromatosis (SC) is a rare joint disorder involving loose cartilaginous bodies, leading to pain, swelling, and impaired function. Arthroscopy has become a primary treatment option, but its efficacy and recurrence rates remain debated. This systematic review evaluates the effectiveness and safety of arthroscopic interventions, focusing on loose body removal, partial synovectomy, and total synovectomy. METHODS: A systematic search of PubMed and EMBASE (1985-2024) identified studies on arthroscopic treatment of knee SC, adhering to PRISMA guidelines. Inclusion criter

Short-term MRI Follow-up and Thin-layer PDWI Sequence without Fat Suppression for Detecting Cartilage Loose Bodies: A Case Report.

BACKGROUND: Osteochondritis Dissecans (OCD) is an idiopathic process and can progress from stable to cartilage fragmentation with the formation of loose bodies in the affected joint capsule. Loose bodies in the knee may wear out the articular cartilage, tendons, and ligaments, leading to a series of problems, such as joint locking, bouncing, joint effusion, and meniscus tear; therefore, early recognition and treatment of intraarticular loose bodies are important to achieve favorable long-term outcomes. CASE REPORT: A 49-year-old male presented with a 1-month history of right knee discomfort.

The Fate of Highly Porous Titanium Tibial Cones in Revision Total Knee Arthroplasty: A Multicenter 5-Year Minimum Follow-Up Study.

BACKGROUND: The survivorship of tibial metaphyseal cones following revision total knee arthroplasty (rTKA) has been reported. However, literature comparing cone midterm survivorship (from five to 10 years after rTKA) between septic (infection history on the joint) and aseptic revisions is scarce. Therefore, we sought: (1) to assess the overall tibial cone survivorship rate after rTKA; and (2) to compare cone survivorship between septic and aseptic revisions. METHODS: A multicenter retrospective chart review of 231 patients who underwent rTKA using highly porous titanium tibial metaphyseal con

A Miniature Jumping Robot Using Froghopper's Direction-Changing Concept.

To improve the maneuverability and agility of jumping robots, a variety of steerable jumping mechanisms have been actively studied. The steering ability enables a robot to reach a particular target by altering its jumping direction. To make this possible, we propose a miniature steerable jumping robot based on froghopper's jumping principle: Moment cancellation is achieved via synchronous leg rotation, and a predictable jumping direction is achieved through an almost zero stiffness femoro-tibial joint. To satisfy these working principles, the robot is designed to have a four-bar shaped body st

Efficacy Analysis of Arthroscopic Treatment of Synovial Chondromatosis of the Knee: A Retrospective Study of More Than Five Years.

OBJECTIVE: The synovial chondromatosis is an exceptionally rare benign condition, predominantly found in the knee joint, and can result in pain, restricted mobility, and potential irreversible damage to the joint and cartilage. Despite the utilization of arthroscopic techniques in the surgical management of synovial chondromatosis, there remains a paucity of long-term assessment regarding its efficacy. The main objectives of this study include: (i) investigating the long-term efficacy of arthroscopic surgery in patients with knee synovial chondromatosis;(ii) identifying factors influencing fun

Effect of Cartilage Injuries on Medial Patellofemoral Ligament Reconstruction Outcomes at Midterm Follow-up.

BACKGROUND: Patellofemoral instability most commonly affects young patients and is often associated with concomitant chondral injury, the incidence of which increases with each dislocation event. The impact of these chondral injuries on the function and pain levels of patients who undergo isolated medial patellofemoral ligament (MPFL) reconstruction without bony realignment remains unclear. PURPOSE/HYPOTHESIS: The purpose of this study was to compare patient-reported outcome measures (PROMs) between individuals who underwent isolated MPFL reconstruction with concomitant cartilage restoration

Sex differences in preoperative pain intensity among patients undergoing total knee arthroplasty: A propensity score-matched cross-sectional study.

BACKGROUND: Although sex differences in knee osteoarthritis (OA) pain are well documented, previous studies have not adequately adjusted for structural and inflammatory confounders. Clarifying whether preoperative sex differences in pain persist after such adjustment is clinically important in advanced OA because it may influence surgical decision-making, preoperative counselling, and strategies to prevent persistent postoperative pain. OBJECTIVE: To compare preoperative pain intensity, pain sensitivity, psychosocial characteristics, and physical function between women and men with advanced k

Knee intra-articular loose body causing a compression common peroneal nerve neuropathy with foot drop.

We present a unique case of a man in his 60s with a knee intra-articular loose body causing a compression neuropathy on his common peroneal (fibular) nerve (CPN). This neuropathy caused a reduction in function of his leg's motor function causing a foot drop. Through a strong clinical interview and focused clinical examination, the neuropathy was localised, and the diagnosis was reinforced through nerve conduction studies and imaging. The intra-articular loose body was surgically excised after the careful isolation and decompression of the peroneal nerve. This resulted in a marked improvement i

Osteochondritis Dissecans of the Knee Associated With Mechanical Overload.

BACKGROUND: Osteochondritis dissecans (OCD) of the knee is a rare but potentially incapacitating disorder in which subchondral bone detaches, leading to an osteochondral fragment that can become unstable and progress into a loose body. The exact cause is unknown, although several biological and mechanical factors have been described. PURPOSE: To provide insight into epidemiological data of a large cohort of patients affected by OCD of the knee and to identify potential factors contributing to the cause of this disorder. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: A t

[Treatment of osteochondritis dissecans].

Osteochondritis dissecans (OD) is a rare condition with an incidence of 30/100,000. It especially affects male patients aged 10-20 years old. During the staged progression the osteochondral fragments can detach from their base. These can damage the adjacent articular cartilage, which can lead to premature osteoarthritis. Most commonly affected are the knee, ankle and elbow joints. The exact pathogenesis of OD has so far not been clearly confirmed. Several risk factors that can lead to the development of OD are discussed. These include repeated microtrauma and vascularization disorders that can
