Synovial Chondromatosis in a Young Athlete: A Report of a Rare Case.

Synovial chondromatosis is a rare and benign disorder that involves the synovial lining of joints, synovial sheaths and bursae. The synovial layer of the joint is affected by a metaplastic process which in turn converts it into cartilagenous tissue. Eventually, it gets dislodged and transformed into a loose body in the large joints. We report the case of a 24-year-old young athlete who presented with complaints of painful movements and restriction of joint movements associated with a growing deformity in the right knee joint. This case report aims to describe a rare synovial pathology that nec

Double Trouble: A Case Report of a Locked Knee Due to a Loose Body and a Degenerative Osteophyte.

An acutely locked knee is an incapacitating condition that requires urgent orthopedic intervention. Common causes of the locked knee include a tear of the meniscus, stump of a ruptured anterior cruciate ligament (ACL), loose body, and osteochondral injury. This report describes a rare case of an acutely locked knee due to the concurrent presence of a loose body and a degenerative osteophyte, highlighting the diagnostic challenges and treatment strategies employed for successful management. Notably, to our knowledge, there is no preceding report documenting the same etiology.

[Dealing with early complications in unicondylar knee arthroplasty-what works, what does not?].

Complications in the early postoperative period following the implantation of a unicondylar knee arthroplasty (UKA) are both rare and rarely described in the literature. Often, only small case series or individual case reports are available. In this article, the most common complications of periprosthetic infection, periprosthetic tibial fracture, inlay dislocation and intra-articular cement residue in (loose bone cement body) are described and recommendations for conservative and surgical treatment are presented, including case examples. Ideally, surgical errors or an infection should be reco

Excellent 10-Year survivorship of robotic-arm-assisted unicompartmental knee arthroplasty.

INTRODUCTION: Robotic-arm-assisted unicompartmental knee arthroplasty (UKA) is an excellent solution for patients suffering from single-compartment knee arthritis. While outcomes tend to be favorable for UKAs, revision operations, commonly due to component malpositioning and malalignment resulting in accelerated wear, are a major concern. Intraoperative technologies, such as robotic assistance, can help better ensure that implants are positioned based on a patient's specific anatomy and mechanical physiology. However, long-term survivorship and patient-reported satisfaction with robotic-assist

Effects of WeChat-Based Mindfulness-Based Stress Reduction on Psychological Resilience and Quality of Life Among Patients Undergoing Loose Body Removal Surgery.

BACKGROUND: The knee joint, a structurally intricate component, is highly susceptible to injuries, profoundly impacting the afflicted patients' life quality. Among these injuries, loose body removal stands out, historically managed through conventional open surgical interventions. However, such procedures entail complications, including infection and extended recovery periods, fostering postoperative psychological distress like anxiety and depression. OBJECTIVE: This study aims to assess the impact of WeChat-based mindfulness-based stress reduction (MBSR) on psychological resilience and quali

Morbidity and Mortality Following Total Hip and Knee Arthroplasty With Spinal Versus General Anesthesia: A Retrospective Analysis.

BACKGROUND: The optimal anesthetic mode in total joint arthroplasty (TJA) has yet to be clearly identified. Patients undergoing TJA may recieve spinal anesthesia (SA) or general anesthesia (GA). While arthroplasty literature indicates differences in postoperative morbidity, hip fracture literature does not show clear superiority of SA or GA. The purpose of this study was to further investigate this relationship and determine if there is a significant difference in morbidity and mortality between GA and SA in patients undergoing primary total joint arthroplasty. METHODS: Patients undergoing pr

Extended Medial Coronal Plane Plica Formed by a Rare Fusion of Medial Patellar and Biblike Plicae in the Knee: A Case Report.

The traditional plicae of the knee have been updated recently with reports of new variants in the coronal plane. We report another variant, an extended medial coronal plane plica (EMCPP), previously unreported, in a 70-year-old lady with osteoarthritis of the knee and a loose body. The preoperative symptoms after the excision of the plica and loose body were temporarily relieved for 11 months before she underwent total knee arthroplasty (TKA) for associated osteoarthritis. Excision of the EMCPP could resolve symptoms, but resultant internal scarring or the plica itself could cause difficulty i

Extended Interportal Capsulotomy for Hip Arthroscopy, a Single-Center Clinical Experience.

Background and Objectives: The number of hip arthroscopy procedures is on the rise worldwide, and awareness regarding proper management of the hip capsule has increased. No capsulotomy shape is agreed upon as a standard approach, with literature supporting both isolated interportal and T-shaped capsulotomies. The aim of this retrospective cohort study is to report the clinical results of a standardized extended interportal capsulotomy (EIPC) during hip arthroscopy. Materials and Methods: Patients operated on between 2017 and 2020 with a hip arthroscopy were eligible. The inclusion criteria wer

Multiple Epiphyseal Dysplasia With Knee Joint Locking Symptoms Caused by Intra-articular Loose Bodies.

Multiple epiphyseal dysplasia (MED) is a congenital disease causing epiphyseal dysplasia in long bones. Herein, we report a case of a middle-aged man with bilateral knee joint locking symptoms who was diagnosed with multiple epiphyseal dysplasia caused by Matrilin-3 (MATN3) pathogenic variants and was successfully treated with arthroscopic loose body removal. A 48-year-old man has had bilateral knee pain since his twenties and underwent loose body removal of both knees in his thirties. He visited our hospital for worsening locking symptoms in both knees. Twenty years ago, his son had been diag

Clinical Outcomes and Survivorship of Lateral Unicompartmental Knee Arthroplasty: A Large Single Surgeon Cohort.

BACKGROUND: Lateral unicompartmental knee arthroplasty (UKA) is an effective treatment for isolated lateral compartment osteoarthritis. However, due to the rarity of the procedure, long-term outcomes and survivorship are poorly understood. We report the clinical and radiographic outcomes after lateral UKA. METHODS: We retrospectively reviewed a consecutive series of patients who underwent lateral UKA by a single fellowship-trained arthroplasty surgeon from 2001 to 2021 with a minimum 2 year follow up. There were 161 knees in 153 patients (average age 69 years) that met inclusion criteria, wit

Fresh Osteochondral Allograft Transplantation of the Capitellum for the Treatment of Osteochondritis Dissecans.

BACKGROUND: Osteochondritis dissecans (OCD) of the humeral capitellum is a rare and challenging condition to treat. Several surgical options exist, but in the last few years, the pendulum has swung from debridement and microfracture to restoration of the articular surface. Osteochondral autografts from the rib and knee have been described, but donor-site morbidity is a concern. PURPOSE: To expand the results of fresh osteochondral allograft transplantation (FOCAT) in a previously published report with inclusion of additional patients and a longer follow-up period. STUDY DESIGN: Case series;

Arthroscopic Excision of a Solitary Intra-articular Osteochondroma Manifesting as a Loose Body: A Presentation of an Extremely Rare Medical Condition.

Osteochondroma typically has extra-articular growths at the metaphysis. Intra-articular osteochondroma is extremely uncommon. We report a case of a 55-year-old woman who had been experiencing right knee pain for the past 12 months. An arthroscopy revealed a medial meniscus tear with a loose body in the right knee. It was removed arthroscopically. Histopathology identified it as an osteochondroma. Therefore, intra-articular osteochondroma can be regarded as an uncommon cause of loose bodies in adult patients.

Atypical Presentation of Metastatic Carcinoma Causing Patellar Destruction and Synovial Carcinomatosis: A Case Report.

INTRODUCTION: Wilson's disease is an autosomal recessive condition where excessive amount of copper accumulates in the body, especially in the liver, brain, and eyes. It is caused by a mutation in the ATP7B gene on chromosome 13. In 25-30% of patients, joint involvement occurs at the later course of disease; osteoarthritis being the commonest manifestation. Synovial chondromatosis (SC) is a benign metaplasia which occurs in the synovial membrane and it results in the formation of intra-articular loose bodies (LB), but its occurrence in a patient of Wilson's disease has not yet been reported in

Two Healthy Elderly Females with Contemporaneous Subchondral Insufficiency Fractures of Bilateral Femoral Heads: A Case Report.

INTRODUCTION: Unilateral subchondral insufficiency fractures (SIF) of the femoral head have been reported in many cases; however, bilateral cases occurring simultaneously are very rare. CASE REPORT: We report two cases of relatively active elderly females (aged 73 and 67 years), in which bilateral hip pain appeared simultaneously without any specific triggers. There were no notable lifestyle or medication histories. Imaging and histopathology of the bone excised surgically supported the diagnosis. In Case 1, there was a decrease in bone mineral density and poor congruency between the femoral

Combined quadriceps medial patellofemoral ligament reconstruction and osteochondral fixation offers good patient-reported outcomes and low rates of recurrent instability for osteochondral defects secondary to acute patella dislocation.

PURPOSE: Chondral injuries secondary to traumatic patella dislocation are common, and a subgroup of these are significant defects with fragments amenable to fixation. There is a paucity of published evidence assessing patients managed with combined acute patellofemoral stabilisation and osteochondral fixation. The purpose of this study is to report the outcomes of patients with osteochondral injuries secondary to acute traumatic patella dislocation treated with combined early fragment fixation and MPFL reconstruction using a quadriceps tendon turndown technique which has distinct advantages fo
