Relationship between Postoperative Neuropathic Pain and Clinical Scores in Patients with Cervical Ossification of the Posterior Longitudinal Ligament.

INTRODUCTION: Postoperative neuropathic pain (NeP) is common after surgery for cervical ossification of the posterior longitudinal ligament (cOPLL) and may affect clinical outcomes. The PainDETECT questionnaire (PDQ) is a widely used tool for evaluating NeP, while the Spine PDQ (SPDQ) was developed to improve specificity for spinal disorder-related NeP. This study aimed to evaluate postoperative NeP using PDQ and SPDQ and to examine their associations with clinical scores. METHODS: A total of 234 patients (170 men and 64 women; mean age, 61.9 years) who underwent cOPLL surgery were analyzed a

Local antibiotic delivery using biodegradable calcium sulphate beads in paediatric masticator space infection- a technical note.

Deep masticator space infections are difficult to manage because of complex anatomy, rapid spread, necrotic tissue and limited antibiotic penetration. We describe the use of biodegradable antibiotic-loaded calcium sulphate beads in a refractory paediatric infection. A 5-year old child developed left masticator and temporal space infection with severe trismus following temporomandibular joint ankylosis surgery at an elsewhere centre. Swab culture isolated Staphylococcus pseudintermedius susceptible to vancomycin and gentamicin. After failure of systemic antibiotics, surgical debridement was per

Falciform Ligament Inflammation Complicated by Umbilical Vein Thrombophlebitis, Left Portal Vein Thrombosis, and Multiple Hepatic Abscesses: A Case Report.

Falciform ligament pathology is a rare cause of acute upper abdominal pain and may resemble more common pancreatic and hepatobiliary disorders. A woman in her 50s presented with sudden severe epigastric pain radiating to her back and marked hyperamylasaemia of 1,747 U/L. This combination fulfilled clinical and biochemical diagnostic criteria for acute pancreatitis, although computed tomography (CT) demonstrated no pancreatic inflammation and instead showed severe inflammatory change centred on the falciform ligament, with associated focal fat infarction, left portal vein thrombosis, and gas lo

Systematic review and meta-analysis on comparing treatment approaches of cervical fractures in ankylosing spondylitis to determine improvement in outcomes.

BACKGROUND & OBJECTIVE: Anterior, posterior and combine surgical approach for treatment of cervical fractures in ankylosing spondylitis is debatable in current existing literature. There are recognized advantages and disadvantages associated with anterior, posterior, and combined surgical approaches to the cervical spine when treating fractures in patients with ankylosing spondylitis (AS). Despite growing surgical experience, the optimal approach with respect to neurological recovery, complication rates, and mortality remains unclear, and the choice is largely guided by surgeon expertise and p

Extracellular vesicles at the periodontal immune-regenerative interface: coupled mechanisms and translational challenges.

Periodontitis is a chronic immune-inflammatory disease in which dysbiotic microbial communities and sustained host responses disrupt the gingiva, periodontal ligament (PDL), cementum, and alveolar bone. Because these tissues form an integrated attachment apparatus, effective regeneration requires coordinated control of inflammation and reconstruction of soft and mineralized tissues. Extracellular vesicles (EVs) function as intercellular mediators at this immune-regenerative interface by transferring proteins, lipids, and nucleic acids among immune, stromal, vascular, and bone-remodeling cells.

Schwann cell-derived extracellular vesicles promote periodontal tissue regeneration.

Periodontitis is a common chronic inflammatory disease worldwide, and conventional therapies control inflammation but fail to achieve predictable regeneration. The inferior alveolar nerve (IAN) is a key peripheral nerve that governs mandibular bone homeostasis, and its disruption precipitates bone metabolic imbalance and impaired repair. However, the dynamic behavior of Schwann cells (SCs), the principal glial cells of this neural axis, and their mechanistic influence on periodontal ligament stem cells (PDLSCs) remain elusive. We established a rat IAN transection model and found that denervati

Postoperative Complications Following Shoulder Stabilization for Primary Versus Recurrent Anterior Shoulder Dislocation: A National Database Study.

BACKGROUND: Anterior shoulder dislocation (ASD) is the most common joint dislocation in young patients with recurrence rates of nearly 90%. Multiple dislocations can cause progressive structural damage and bone loss, increasing the risk of instability. This study compares recurrence and postoperative complications following ASD between patients with a primary versus recurrent ASD. We hypothesized that surgical stabilization following recurrent ASD would be associated with an increased risk of subsequent instability and a higher complication burden. METHODS: A retrospective query was conducted

An anterior-first two-stage surgical strategy for neglected traumatic facet dislocation of subaxial cervical spine: a technical note.

BACKGROUND: Neglected subaxial cervical traumatic facet dislocation (TFD) presents unique surgical challenges due to fibro-osseous ankylosis and severe segmental instability. Traditional three-stage procedures are effective in complex cases but may increase operative complexity because of repeated patient repositioning and multiple surgical corridors. This study aimed to describe an anterior-first two-stage algorithm for neglected TFD and to report preliminary feasibility observations, including the selective use of a single-screw zero-profile cage as an anterior spacer. METHODS: Five patient

Occult Incarcerated Osteochondral Fracture of the Medial Patellar Facet Following Acute Lateral Patellar Dislocation in an Adolescent: A Case Report.

Acute lateral patellar dislocation is a common injury in adolescents and is most frequently associated with rupture of the medial patellofemoral ligament (MPFL). Concomitant osteochondral fractures may remain occult or be misinterpreted on initial imaging, resulting in delayed diagnosis and potentially compromising long-term joint function. We present a rare case of an incarcerated osteochondral fracture of the medial patellar facet that was initially misinterpreted as arising from the medial femoral condyle following acute lateral patellar dislocation. A previously healthy 15-year-old boy pre

Nutritional strategies to manage postural orthostatic tachycardia syndrome (POTS) and its comorbidities: A narrative review.

BackgroundPostural orthostatic tachycardia syndrome (POTS) is a heterogeneous disorder of the autonomic nervous system characterized by orthostatic intolerance and involvement of multiple organ systems. It commonly coexists with conditions such as hypermobile Ehlers-Danlos syndrome (hEDS), mast cell activation syndrome (MCAS), and a spectrum of gastrointestinal (GI) disorders. These overlapping conditions frequently manifest with shared GI symptoms such as nausea, bloating, abdominal pain, early satiety, and altered bowel habits that can substantially compromise nutritional status and quality

Medial patellofemoral ligament reconstruction in osteogenesis imperfecta using quadriceps tendon graft: a rare case report.

Osteogenesis imperfecta (OI) is a rare connective tissue disorder associated with bone fragility and ligamentous laxity, predisposing patients to recurrent patellar instability. Surgical treatment of recurrent patellar instability is challenging because standard medial patellofemoral ligament (MPFL) reconstruction techniques involving patellar tunneling may increase the risk of patellar fracture. We present a 15-year-old male patient with OI type V and recurrent patellar dislocation treated with a minimally invasive MPFL reconstruction using a pedicled quadriceps tendon graft. This patella-spa

Development of an impingement induced, reversible degenerative meniscus tear animal model to analyze pathogenesis and potential recovery from meniscus degeneration.

OBJECTIVE: We aimed to develop a novel, impingement-induced, reversible degenerative meniscal tear (DMT) animal model to replicate the natural progression of meniscal degeneration and provide insights into the underlying mechanisms of DMTs in relation to osteoarthritis. METHODS: A rabbit model was used to induce medial meniscal posterior horn (MMPH) impingement through anterior displacement. The animals were divided into degeneration and reversal groups, with reversal achieved through MMPH reduction. Histological analysis, MRI imaging, biomechanics, and gene expression assays were performed t

A mobile intra-abdominal low-grade fibromyxoid sarcoma arising from the jejunal mesentery in an adolescent: a case report and literature review.

Low-grade fibromyxoid sarcoma (LGFMS) is a rare fibroblastic neoplasm that only exceptionally involves the intra-abdominal cavity. We report a 17-year-old girl with a one-year history of a progressively enlarging, painless, freely mobile abdominal mass. Computed tomography and magnetic resonance imaging showed a localized, well-circumscribed, pedunculated lesion with positional variability, initially suggesting a completely resectable gastrointestinal stromal tumor. Preoperative biopsy was not performed because systemic treatment was not anticipated and intact resection was planned while minim

Patients with cyclops syndrome exhibit elevated synovial fluid interleukin-6 (IL-6) and tumor necrosis factor alpha (TNF-α) levels and disorganized collagen remodelling within the lesion: A histological and molecular study.

PURPOSE: Cyclops syndrome is a clinically relevant complication of anterior cruciate ligament (ACL) reconstruction characterized by the formation of a fibrotic nodule (cyclops lesion) that causes extension loss and often requires a revision surgery. Biological mechanisms underlying lesion formation remain poorly understood. The purpose of this study is to characterize the tissue architecture, cellular composition, inflammatory profile and single-cell cytokine secretion patterns of patient-derived biopsies to gain mechanistic insight into disease pathogenesis. METHODS: This prospective observa
