Exploring the biomechanical mechanisms of neuromuscular joint facilitation intervention in patients with knee osteoarthritis: a finite element analysis.

INTRODUCTION: Knee osteoarthritis (KOA) causes joint pain and gait disorders in older adults. Exercise is an effective non-pharmaceutical therapy. Neuromuscular joint facilitation (NJF) improves knee joint function by combining neuromuscular facilitation with intra-articular joint motion; however, its biomechanical mechanism remains unclear. Finite element analysis, the mechanisms by which two NJF movement patterns-knee extension with lateral tibial rotation and knee flexion with medial tibial rotation-affect the stress distribution on the meniscus and major ligaments in patients with KOA. Protective Effects of the Phytocystatin CaneCPI‑5 on Periodontal Ligament Cell Osteogenic Activity and Alveolar Bone Loss under Inflammatory Challenge.

Periodontal disease is characterized by a chronic inflammatory environment that compromises the regenerative capacity of periodontal tissues, particularly by impairing the osteogenic potential of periodontal ligament cells. Phytocystatins have emerged as bioactive molecules with potential regenerative and immunomodulatory properties; however, their effects on periodontal tissues under inflammatory conditions remain unexplored. In this study, we investigated the effects of CaneCPI-5 on the osteogenic behavior of human periodontal ligament cells (hPDLCs) under basal and inflammatory conditions i

CT-based evaluation of bony Bankart lesions after arthroscopic Trillat stabilization for anterior shoulder instability.

PURPOSE: Bony Bankart lesions are known to contribute to anterior shoulder instability, and their evolution has mainly been studied after arthroscopic repair. However, the evolution of these lesions after shoulder stabilization without any specific procedure performed on the osseous fragment remains unknown. The aim of this study was to assess the CT-based evolution of bony Bankart lesions after arthroscopic Trillat stabilization. METHODS: This retrospective, single-center study included patients treated between 2013 and 2022 for chronic anterior shoulder instability with a bony Bankart lesio

GsMTx4 Alleviates Knee Osteoarthritis by Inhibiting PIEZO1/2-mediated Chondrocyte Apoptosis.

BACKGROUND: Knee osteoarthritis (KOA) is a prevalent, disabling degenerative joint disorder featuring progressive articular cartilage loss, persistent chronic pain, and compromised knee mobility. This work aimed to characterize the expression pattern of mechanosensitive ion channels PIEZO1/2 in KOA and explore their potential regulatory roles, as well as preliminarily evaluate whether non-selective PIEZO inhibitor GsMTx4 could slow KOA-related pathological changes. MATERIALS AND METHODS: Cartilage tissues and matched clinical data were collected from 54 KOA patients undergoing knee arthroplas

Management of Vertical Root Fracture Using Intentional Reimplantation and Radicectomy: A Case Report.

This case report describes the management of a clinically suspected vertical root fracture (VRF) using intentional reimplantation combined with radicectomy and documents the 36-month outcome with cone-beam computed tomography (CBCT). A 63-year-old man presented with spontaneous pain and intense pain on mastication associated with tooth 46. Clinical examination showed grade II mobility, positive percussion and buccal palpation, and an isolated 10-mm periodontal defect with bleeding and suppuration on the buccal aspect of the mesial root. Periapical radiography showed mesial-root underfilling an

ACL injury reprograms quadriceps myonuclear epigenetic and transcriptional signatures in mice.

BACKGROUND: ACL injury is a debilitating sport-related injury that induces prolonged deficits in muscle size and strength. Using a time-course of in vivo fluorescent metabolic labeling of nascent RNA in an ACL injury mouse model, we identified upregulated myonuclear transcriptional output early following the injury. METHODS: To define altered myonuclear transcription, we bred HSArtTA/rtTA:TetO-H2B-GFP mice to allow for stable in vivo fluorescent labeling and sorting of resident (non-satellite cell-derived) myonuclei 3-days post ACL transection surgery. We performed myonucleus-specific RNA seq

Editorial Commentary: Sagittal Tibial Tubercle-Trochlear Groove Distance Refines Patellar Instability Phenotyping but Should Not Yet Dictate Surgical Correction.

Patellofemoral instability is a 3-dimensional disorder, yet surgical planning remains dominated by axial and coronal measurements. Recent literature reports that greater preoperative sagittal tibial tubercle-trochlear groove (sTT-TG) distance was associated with better quality of life, less pain, and better function after combined sulcus-deepening trochleoplasty and medial patellofemoral ligament reconstruction in patients with high-grade trochlear dysplasia. The finding is compelling because trochleoplasty directly modifies the femoral landmark used to calculate sTT-TG. A greater sTT-TG may,

Advances in Stem Cell Therapy for Hearing Loss.

Hearing loss is the most common sensory disorder, causing substantial impacts on language development, literacy, mood, and cognition. Sensorineural hearing loss, caused by injury to the cochlear hair cells, cochlear nerve, or central auditory pathways, accounts for most cases of hearing loss. Existing therapies for sensorineural hearing loss include hearing aids and cochlear implants; however, these modalities have limited efficacy in noisy environments, variable hearing outcomes, and limited frequency discrimination. There are currently no Food and Drug Administration-approved cellular, molec

Ginsenoside Rg1/Rb1 relieves periodontitis via inhibiting IKBKG -mediated NF-κB pathway to enhance COL1A1 expression.

BACKGROUND: Periodontitis is a chronic inflammatory disease driven by microbial dysbiosis, and effective pharmacological strategies remain limited. This study evaluated whether combined ginsenoside Rg1/Rb1 protects against periodontitis and whether this protection involves IKBKG-dependent NF-κB regulation with restoration of COL1A1. METHODS: Periodontitis was induced in rats by steel-wire ligation plus Porphyromonas gingivalis lipopolysaccharide (Pg-LPS) challenge, followed by local gingival administration of Rg1/Rb1. In parallel, Pg-LPS-stimulated human periodontal ligament stem cells (hPDLS

The crossing-sign angle and trochlear dysplasia length: New radiographic parameters to measure the dysplastic trochlea.

INTRODUCTION: Trochlear dysplasia is one of the most relevant anatomic factors predisposing patients to patellofemoral instability and its recurrence. This study introduces two novel radiographic measurements: the crossing-sign angle and the trochlear dysplasia length. METHODS: A concordance study was conducted in patients aged 10-40 years with at least one episode of patellofemoral dislocation and radiographic evidence of trochlear dysplasia on a true lateral knee radiograph. Three observers independently measured the crossing-sign angle, the trochlear dysplasia length, and bump height, as w

Shoulder anterior capsular (SHAC) block for analgesia in anterior shoulder dislocation: An emergency department case series.

BACKGROUND: Anterior shoulder dislocation is among the most common major joint dislocations presenting to the emergency department (ED) and is frequently managed with procedural sedation. Several analgesic and regional anesthesia strategies are available for shoulder reduction, each with distinct advantages and limitations. The Shoulder Anterior Capsular (SHAC) block is a superficial, ultrasound-guided interfascial plane technique that targets pericapsular nerve contributions to the glenohumeral joint via an anterior approach, offering a potentially motor-sparing and sedation-sparing alternati

Ferroptosis in endodontic pathobiology: Emerging evidence, mechanistic rationale, and translational uncertainties.

INTRODUCTION: Ferroptosis is an iron-dependent form of regulated cell death characterized by lipid peroxidation and impaired antioxidant defense. Its defining biochemical features, including iron-redox imbalance and lipid peroxide accumulation, overlap with biological conditions present in infected, hypoxic, and cytokine-rich endodontic tissues. This narrative review examined the potential role of ferroptosis in endodontic pathobiology, with emphasis on separating direct endodontic evidence from indirect or extrapolated oral evidence. METHODS: Electronic searches were performed in PubMed, Sco

Circulating angiogenic T cells as potential immune-vascular biomarkers associated with disease activity in axial spondyloarthritis.

OBJECTIVE: Axial spondyloarthritis (axSpA) is a chronic inflammatory autoimmune disease associated with persistent systemic inflammation and endothelial dysfunction. Angiogenic T cells (Tang cells), defined as CD3+CD31+CD184+ T cells, are specialized immune cells involved in vascular repair and endothelial homeostasis, but their clinical relevance in axSpA remains unclear. This study aimed to characterize circulating Tang cells in axSpA and evaluate their potential as immune-vascular biomarkers of disease activity. METHODS: Peripheral blood samples were obtained from patients with axSpA strat

Targeted Intracellular Hydrogen Sulfide Delivery to Inflammatory Macrophages Reprograms Polarization and Mitigates Oxidative Stress in Osteoarthritis.

Osteoarthritis (OA) progression is driven by a chronic inflammatory microenvironment involving aberrant macrophage polarization and oxidative stress. Although the endogenous gasotransmitter hydrogen sulfide (H2S) possesses potent anti-inflammatory and antioxidant properties, its therapeutic application is hindered by rapid clearance, uncontrolled release profiles, and a narrow therapeutic window. Here, we present a macrophage-targeted and pH-responsive H2S delivery nanoplatform constructed by encapsulating acid-labile zinc sulfide within a zeolitic imidazolate framework-8 (ZIF-8) and functiona

Neglected Busch-Hoffa fracture following the patellar dislocation: a rare case report.

INTRODUCTION AND IMPORTANCE: Busch-Hoffa fractures are rare intra-articular coronal plane fractures of the femoral condyle that often remain unnoticed, particularly when complications develop. Due to their instability and risk of long-term problems, these fractures require immediate anatomical reduction and stable fixation. The occurrence of a Busch-Hoffa fracture in conjunction with patellar dislocation is extremely rare and has been rarely reported in the current literature. CASE PRESENTATION: We report a case involving a 21-year-old male who experienced a Bagaria type 2 Hoffa fracture of t

[Carpal Tunnel Syndrome and Lumbar Spinal Stenosis: When to Suspect Amyloidosis and How to Proceed with the Diagnostic Work-up].

Transthyretin (TTR) amyloidosis, particularly the wild-type form (ATTRwt), is an age-related systemic disease characterized by the deposition of misfolded TTR in tendons and ligaments. Patients therefore frequently present initially to orthopaedic or hand surgeons with carpal tunnel syndrome (CTS) or lumbar spinal stenosis (LSS) rather than to cardiologists or neurologists. These musculoskeletal manifestations may precede ATTR cardiomyopathy (ATTR-CM) by several years to more than a decade, serving as "red flags" that provide an opportunity for early diagnosis, particularly now that disease-mo

Concomitant Abdominoplasty and Repair of Midline Abdominal Wall Defects.

BACKGROUND: Diastasis recti and midline abdominal wall hernias are frequently encountered in patients undergoing abdominoplasty, particularly postpartum women and individuals following significant weight loss. The associated musculoaponeurotic laxity results in functional abdominal wall weakness and contour deformities. METHODS: We describe a surgical technique combining abdominoplasty with midline fascial repair, performed without mesh reinforcement. The procedure utilizes a modified overlapping Mayo double-suturing method to restore midline strength and contour. This paper presents a 39-yea
