Proteolytic enzymes in human cartilage: the pathogenesis of osteoarthritis.

It is evident that human articular cartilage possesses, in addition to multiple forms of cathepsin D, multiple forms of other acid cathepsins, and, most important, a family of at least four closely related neutral protease enzyme forms, all of which degrade proteoglycan. In addition, caseinase and histonase activities are present. The search for these enzymes in human cartilage ahs been presented in some detail in order to give an idea of some of the problems faced in such research, as well as the hypotheses and hopes that flow from it and prepare the ground for further research. The actual ro

Chondrodysplasia punctata--rhizomelic form. Pathologic and radiologic studies of three infants.

Pathologic, ultrastructural and radiologic studies are described on 3 infants with the rhizomelic form of chondrodysplasia punctata. Radiologic criteria in the young infant include radiolucent coronal clefts dividing all or most of the thoracic and lumbar vertebral bodies, short humeri with flared metaphyses and punctate calcifications commonly present adjacent to the ossified ischial and pubic bones and less commonly in other locations. In late infancy and childhood the radiologic criteria include demineralization in all bones with slow maturation, flat vertebral bodies, short humeri and femo

Relapsing polychondritis: prospective study of 23 patients and a review of the literature.

Relapsing polychondritis (RP) is not a totally rare rheumatic disease. We have seen 23 patients from 1960-1975, and there are now a total of 159 reported cases, which form the basis of this study. RP occurs equally in both sexes, and has a maximum frequency in the fourth decade. 2) Empirically defined diagnostic criteria are proposed, to include the most common clinical features: a) Bilateral auricular chondritis b) Nonerosive sero-negative inflammatory polyarthritis c) nasal chondritis d) Ocular inflammation e) Respiratory tract chondritis f) Audiovestibular damage The diagnosis is based prim

Influence of the pericellular environment on the cells. The role of mucopolysaccharides in the protection of cartilage cells against immune reactions.

Problems related to rheumatoid arthritis have been investigated by a group at Cambridge using the organ culture technique. Since auto-allergic reactions may be concerned in the chronicity of the disease, the effects of reactive complement-sufficient antisera (AS+C') on embryonic and post-foetal cartilage were examined. The cartilaginous limb bone rudiments enlarged to several times their original volume in control medium, but in the presence of AS+C' they gradually disintegrated, owing to the breakdown of the cartilage matrix; only the superficial cells of the enveloping soft connective tissue

Acrylic cement stabilized joint replacements.

Surgical management of osteoarthritis, aseptic necrosis and rheumatoid arthritis has been revolutionized by the introduction of acrylic cement-stabilized joint surface replacement. Although single joint surface replacements have been employed extensively for more than half a century, total surface replacement operations with a wear-resistant high-density polyethylene and noncorrosive stainless steel stabilized by acrylic cement were introduced only a little more than 12 years ago. This evolved with Charnley's discovery of the high level of bone tolerance for acrylic cement. Acrylic cement made

Identification of immunoglobulins and complement in rheumatoid articular collagenous tissues.

Ninety-three patients with a variety of joint diseases were studied for evidence of immune complexes in articular collagenous tissues. Frozen sections of freshly obtained biopsies of hyaline articular cartilage and menisci were stained with fluoresceinated monospecific antisera for evidence of human immunoglobulins (IgG, IgM,IgA) and the beta1c component of complement. The criterion for the presence of complexes was the staining of two or more immunoglobulins and beta1c in an identical location of sequentially cut sections. Of the 42 patients with rheumatoid arthritis (RA) 83% were positive by

UCLA Conference. Diagnosis and treatment of erosive rheumatoid arthritis and other forms of joint destruction.

Single or multiple destruction of joints arises from many causes, ranging from the acute destructive process (usually monarticular) in septic joint disease to the chronic, slowly evolving destruction of cartilage and adjacent bone that occurs in osteoarthritis. Medical treatment of a few of these conditions (including rheumatoid arthritis) is reasonably satisfactory if it is initiated early. On the other hand, surgical procedures are indicated if physical and X-ray examination of the involved joints discloses the presence of a significant destructive process involving both articular cartilage

Calcium pyrophosphate dihydrate deposition disease without chondrocalcinosis.

The pseudogout syndrome is usually associated with radiographic evidence of articular cartilage calcification. Eight patients who had joints containing calcium pyrophosphate dihydrate crystals were studied. Extensive radiographic evaluation was obtained in seven patients and a limited evaluation in the other. None had evidence of chondrocalcinosis. Six had distinctive radiographic abnormalities of the wrists consisting of radiocarpal joint space narrowing and sclerosis, and subchondral cystic degeneration of the carpal bones. We conclude that calcium pyrophosphate dihydrate deposition disease

Chondromalacia of the patella. Physical signs in relation to operative findings.

Cartilaginous changes on the patella are common and were demonstrated in 50 out of 100 patients who underwent operation for intraarticular disease of the knee joint. In order to clarify further the signs of this condition, the physical signs were related to the operative findings. Retropatellar crepitation was present in most cases, but cannot be called pathognomonic. Pain on grating and tenderness on palpation of the patella in fact occurred together only in cases with cartilaginous changes on the patella. On the other hand, cartilaginous changes were found in several cases without such signs

Scattered mitoses in mature joint cartilage in rabbits after local trauma. A chalone effect?

This is a demonstration of local traumatization of articular cartilage inducing mitoses in parts of a joint other than the site of inquiry. Parts of one of the patellar cartilages of the full-grown rabbit were scored while the other knee was subjected only to arthrotomy. Radioactive thymidine and autoradiographic technique demonstrated that labeled chondrocytes appeared not only around the score in the patella but also scattered in the tibia and femoral condyles. Whether the mechanism of regional mitotic stimulation is a reduction of the concentration of the normal cell specific inhibitors of

[The coxopathies of Paget's disease. Radio-clinical and anatomopathological study].

A systematic study was carried out on 105 patients with Paget's disease (60 men and 45 women, average age 62 years) involving the acetabulum and/or the femoral head in the juxta-articular zone. In two-thirds of the patients only the pelvis (in particular the acetabulum) was affected, in a quarter both the pelvis and the femoral head were affected, and in one case in ten only the femoral head. 1. 18 coincidental coxopathies should first be excluded (12 coxarthrosis, 2 coxitis, etx.). The occurrence of 17 percent of coxopathies not caused by Paget's disease is normal in older patients (Vignon an

Studies in osteoarthritis of the hip. 3. Congenital subluxation and osteoarthritis of the hip.

Although congenital subluxation of the hip is an important primary abnormality, upon which OA hip may later supervene, other factors may also be important in individual patients. When osteoarthritis is seen with subluxation, it probably represents congenital subluxation and not displacement of the femoral head secondary to remodelling. Osteoarthritis in these cases is usually superolateral or supermedial in type, with the major impact of the disease in the superior articular cartilage. A similar form of osteoarthritis is seen in cases where leg-length disparity exists, with or without the pres
