ACL injury reprograms quadriceps myonuclear epigenetic and transcriptional signatures in mice.

BACKGROUND: ACL injury is a debilitating sport-related injury that induces prolonged deficits in muscle size and strength. Using a time-course of in vivo fluorescent metabolic labeling of nascent RNA in an ACL injury mouse model, we identified upregulated myonuclear transcriptional output early following the injury. METHODS: To define altered myonuclear transcription, we bred HSArtTA/rtTA:TetO-H2B-GFP mice to allow for stable in vivo fluorescent labeling and sorting of resident (non-satellite cell-derived) myonuclei 3-days post ACL transection surgery. We performed myonucleus-specific RNA seq

Editorial Commentary: Sagittal Tibial Tubercle-Trochlear Groove Distance Refines Patellar Instability Phenotyping but Should Not Yet Dictate Surgical Correction.

Patellofemoral instability is a 3-dimensional disorder, yet surgical planning remains dominated by axial and coronal measurements. Recent literature reports that greater preoperative sagittal tibial tubercle-trochlear groove (sTT-TG) distance was associated with better quality of life, less pain, and better function after combined sulcus-deepening trochleoplasty and medial patellofemoral ligament reconstruction in patients with high-grade trochlear dysplasia. The finding is compelling because trochleoplasty directly modifies the femoral landmark used to calculate sTT-TG. A greater sTT-TG may,

Advances in Stem Cell Therapy for Hearing Loss.

Hearing loss is the most common sensory disorder, causing substantial impacts on language development, literacy, mood, and cognition. Sensorineural hearing loss, caused by injury to the cochlear hair cells, cochlear nerve, or central auditory pathways, accounts for most cases of hearing loss. Existing therapies for sensorineural hearing loss include hearing aids and cochlear implants; however, these modalities have limited efficacy in noisy environments, variable hearing outcomes, and limited frequency discrimination. There are currently no Food and Drug Administration-approved cellular, molec

Ginsenoside Rg1/Rb1 relieves periodontitis via inhibiting IKBKG -mediated NF-κB pathway to enhance COL1A1 expression.

BACKGROUND: Periodontitis is a chronic inflammatory disease driven by microbial dysbiosis, and effective pharmacological strategies remain limited. This study evaluated whether combined ginsenoside Rg1/Rb1 protects against periodontitis and whether this protection involves IKBKG-dependent NF-κB regulation with restoration of COL1A1. METHODS: Periodontitis was induced in rats by steel-wire ligation plus Porphyromonas gingivalis lipopolysaccharide (Pg-LPS) challenge, followed by local gingival administration of Rg1/Rb1. In parallel, Pg-LPS-stimulated human periodontal ligament stem cells (hPDLS

Ferroptosis in endodontic pathobiology: Emerging evidence, mechanistic rationale, and translational uncertainties.

INTRODUCTION: Ferroptosis is an iron-dependent form of regulated cell death characterized by lipid peroxidation and impaired antioxidant defense. Its defining biochemical features, including iron-redox imbalance and lipid peroxide accumulation, overlap with biological conditions present in infected, hypoxic, and cytokine-rich endodontic tissues. This narrative review examined the potential role of ferroptosis in endodontic pathobiology, with emphasis on separating direct endodontic evidence from indirect or extrapolated oral evidence. METHODS: Electronic searches were performed in PubMed, Sco

[Carpal Tunnel Syndrome and Lumbar Spinal Stenosis: When to Suspect Amyloidosis and How to Proceed with the Diagnostic Work-up].

Transthyretin (TTR) amyloidosis, particularly the wild-type form (ATTRwt), is an age-related systemic disease characterized by the deposition of misfolded TTR in tendons and ligaments. Patients therefore frequently present initially to orthopaedic or hand surgeons with carpal tunnel syndrome (CTS) or lumbar spinal stenosis (LSS) rather than to cardiologists or neurologists. These musculoskeletal manifestations may precede ATTR cardiomyopathy (ATTR-CM) by several years to more than a decade, serving as "red flags" that provide an opportunity for early diagnosis, particularly now that disease-mo

SATB2 promotes osteogenic differentiation of OPLL-derived posterior longitudinal ligament cells via Wnt/β-catenin signaling in association with RUNX2.

BACKGROUND: Ossification of the posterior longitudinal ligament (OPLL) is a prevalent pathological condition characterized by ectopic ossification of spinal ligaments, leading to myelopathy through spinal cord compression. SATB2 is a multifunctional regulator of osteogenic differentiation, and prior studies in other osteogenic systems have suggested links between SATB2, RUNX2, and Wnt/β-catenin signaling [1, 2]; however, whether these regulatory relationships operate in OPLL-derived posterior longitudinal ligament cells (OPLL cells) remains incompletely understood. In this study, we investigat

[A CASE OF PENILE CANCER WITH PELVIC LYMPH NODE METASTASIS SUCCESSFULLY TREATED WITH SIMULTANEOUS INGUINAL AND PELVIC LYMPH NODE DISSECTION AFTER TIP-BASED NEOADJUVANT CHEMOTHERAPY].

We report a case of penile cancer with bilateral inguinal and pelvic lymph node metastases in which neoadjuvant chemotherapy (NAC), followed by simultaneous bilateral inguinal and pelvic lymph node dissections achieved curative resection. A 66-year-old man presented with a penile tumor. Computed tomography (CT) revealed bilateral inguinal lymph node metastases and enlargement of the external iliac lymph nodes bilaterally. He underwent total penectomy with fine-needle aspiration cytology, which confirmed squamous cell carcinoma (pT2). Although cytology from both inguinal regions was negative, p

Phage-antibiotic synergy dismantles multidrug-resistant Enterococcus faecalis infection in SD rat apical periodontitis model.

Apical periodontitis poses a significant challenge, as 80% of cases are linked to Enterococcus faecalis, which is often protected within a biofilm. Phage-antibiotic synergy (PAS) presents a promising approach to overcoming resistance, though evidence from disease-relevant models, particularly in vivo, remains limited. In this study, we demonstrate that the lytic phage PEf771, when combined with either amoxicillin or ampicillin trihydrate, effectively eliminates multidrug-resistant E. faecalis YN771 across various contexts, including planktonic cultures, mature biofilms, ex vivo infected teeth,

Median Arcuate Ligament Syndrome in an Elderly Patient: A Symptomatic Post-stenotic Aneurysm and Systemic Atherothrombotic Disease.

Median arcuate ligament syndrome (MALS) is atypical in elderly patients, in whom it frequently coexists with severe atherosclerotic disease. We report the case of a 77-year-old male with marked mitral regurgitation who presented with intestinal claudication and malnutrition. CT angiography demonstrated MALS associated with a fusiform aneurysm of the celiac trunk with dissection, along with 60% atherosclerotic stenosis of the superior mesenteric artery. Given the limiting ischemic presentation, the patient underwent laparoscopic release of the ligament. The procedure was complicated by avulsion

Case Report: Recurrent peptic ulcer perforations-a possible role of median arcuate ligament syndrome.

INTRODUCTION: Median arcuate ligament syndrome (MALS), also known as Dunbar syndrome, is a rare vascular compression disorder caused by extrinsic compression of the celiac artery by the median arcuate ligament, affecting 2/100,000 people. Clinical presentation is often non-specific and may include postprandial abdominal pain, nausea, vomiting, diarrhea, and weight loss. Impaired splanchnic perfusion may contribute to gastrointestinal ischemia and, in rare cases, recurrent peptic ulcer disease. We report a patient with multiple episodes of perforated peptic ulcer disease associated with previou

What's New in Hand and Wrist Arthroscopy: Indications, Techniques, and Emerging Applications.

Wrist arthroscopy has undergone substantial evolution over the past 2 decades, transitioning from a primarily diagnostic procedure to a versatile therapeutic platform with applications across trauma, instability, degenerative disease, and reconstructive surgery. Advances in instrumentation, visualization, and surgical technique have expanded indications while facilitating less invasive management of complex wrist pathology. In acute fracture care, arthroscopy provides direct assessment of articular reduction and associated soft tissue injuries in scaphoid and distal radius fractures, with emer

Preoperative clinical and imaging factors associated with arthroscopically confirmed occult syndesmotic instability in patients undergoing surgery for osteochondral lesions of the talus.

AIMS: Osteochondral lesions of the talus (OLTs) are common ankle disorders. Recent clinical observations suggest that a subset of patients with OLTs also exhibit occult syndesmotic instability (OSI) intraoperatively, implying a potential association between the two conditions. This study aimed to investigate the potential clinical association between OLTs and OSI, and to identify preoperative factors associated with OSI in patients with OLTs. METHODS: A retrospective review was performed on patients diagnosed with OLTs who underwent ankle arthroscopy between January 2021 and June 2023. Based

A quantitative X-ray microcomputed tomography workflow for dentoalveolar tissue analysis of soft and hard tissues in the mouse hemimandible.

Dentoalveolar tissues in the maxilla and mandible combine mineralized and nonmineralized components optimized for mastication. Such a combination of hard and soft tissues is difficult to simultaneously visualize and quantify by X-ray microcomputed tomography (μCT). While μCT provides excellent high-resolution, nondestructive imaging of mineralized structures, soft tissues such as predentin, pulp, and periodontal ligament provide only low X-ray attenuation (low contrast) and are often regretfully excluded from analyses. In addition, the relatively high water content of soft tissues further redu

MicroRNA-driven periodontal immune homeostasis and tissue regeneration: from regulatory networks to engineering translation.

Periodontitis is a chronic immune-inflammatory disease induced by dental plaque biofilms. Persistent host immune dysregulation disrupts the dynamic balance between local tissue repair and pathological destruction, ultimately leading to irreversible damage to the periodontal supporting tissues, including the alveolar bone, periodontal ligament, and cementum. As key post-transcriptional regulators, microRNAs (miRNAs) coordinate the osteogenesis-bone resorption balance, immune homeostasis, angiogenesis, and intercellular communication through multi-target synergistic mechanisms, and have thus eme

Baseball has an elbow injury problem: biomechanics may have the solution.

Elbow injuries-particularly to the ulnar collateral ligament (UCL)-have been a problem in sports medicine for at least half a century. As a pitcher rotates his trunk to face the target and externally rotates his throwing arm, varus torque is produced at the elbow to decelerate arm cocking and initiate arm acceleration. With today's pitchers being larger, throwing harder, and playing more often during their development years, repetition of large varus torque leads to more UCL injuries. Pitch count limits for games, seasons, and per year may provide some prevention of overuse injuries. Varying e

Diffuse Idiopathic Skeletal Hyperostosis (DISH) With Cervical Myelopathy: A Case Report From Sudan.

Diffuse idiopathic skeletal hyperostosis (DISH) is a non-inflammatory condition characterized by ossification of spinal ligaments, commonly affecting elderly individuals. While often asymptomatic, it may present severe neurological deficits due to spinal cord compression. We report what is believed to be the first documented case of DISH presenting with cervical myelopathy in Sudan. A 73-year-old diabetic man presented with progressive weakness in all limbs, accompanied by neck and back pain. Neurological examination revealed spastic quadriparesis, sensory loss to proprioception and vibration,
