Shifting paradigms: how the glenoid track changed the concept of shoulder instability.

UNLABELLED: BACKGROUND : Historically, glenoid bone loss and humeral head defects (Hill-Sachs lesions) were evaluated independently. However, the clinical significance of a Hill-Sachs lesion depends on the morphology of the glenoid: an identical lesion may remain stable with an intact glenoid but become unstable in the presence of glenoid bone loss. Therefore, both lesions must be assessed simultaneously. LIMITATION OF TRADITIONAL ASSESSMENT: Traditional dynamic intraoperative examination has an inherent limitation. Engagement can be accurately assessed only after Bankart repair (post-repair

Bone Shape Feature Differences in Single and Recurrent Patellar Dislocations via Statistical Shape Modeling.

This study aims to develop MRI-based 3D statistical shape models for patellar dislocation patients and investigate bone shape features that distinguish between single and recurrent dislocation populations, as well as control and patellar dislocation populations. MRIs from 16 single dislocation patients, 17 recurrent dislocation patients, and 20 control subjects were used to build statistical shape models for femur and patella bones. Bone shape features were extracted and tested for statistical significance (p < 0.05) for distinguishing the single dislocation group from the recurrent dislocatio

Editorial Commentary: Isolated Medial Patellofemoral Ligament Reconstruction Affects Patellar Height, but Quadriceps Dysfunction Is a Potential Modifiable Factor That Influences Patellar Biomechanics.

Recurrent lateral patellar instability is a complex pathology that requires orthopaedic surgeons to consider a variety of factors with an à la carte approach to surgical solutions. In cases of isolated medial patellofemoral ligament reconstruction, patellar height is known to reduce significantly, and in some cases even normalize from a preoperative patella alta status. This becomes particularly important to consider through the lens of direct attachments to the patella, such as the vastus medialis oblique and retinacular tissues, and the influence of patellar height on quadriceps activation a

Residual acetabular roof obliquity is associated with long-term joint preservation after transposition osteotomy of the acetabulum.

PURPOSE: To evaluate whether residual post-operative acetabular roof obliquity is associated with long-term joint preservation after transposition osteotomy of the acetabulum for hip dysplasia. METHODS: This retrospective study included 52 hips with pre-operative Tönnis grade 0 or 1 osteoarthritis and complete electronic radiographic datasets after transposition osteotomy of the acetabulum performed between 1996 and 2005. Conversion to total hip arthroplasty was the primary endpoint. Because only 11 conversions occurred, associations were evaluated using Kaplan-Meier analysis and univariable

Pediatric Patellofemoral Instability Surgery-Current Concepts.

Patellofemoral instability is a common knee pathology affecting children and adolescents, with recurrence rates exceeding 50% in skeletally immature patients. Identification and stratification of anatomic risk factors for recurrence are essential. Trochlear dysplasia, patella alta, skeletal immaturity, and elevated tibial tubercle groove distance have been identified as anatomic predictors of recurrence. Emerging evidence is supporting more aggressive treatment and earlier surgical intervention in high-risk patients, even after a first-time dislocation. Medial patellofemoral ligament reconstru

Evaluation of Changes in Hip Morphology in a Rabbit Model of Patellar Dislocation via Computed Tomography Imaging.

BACKGROUND Although early patellar dislocation induces morphological changes in the patellofemoral joint, its potential influence on hip morphology during growth remains unclear. This study evaluated changes in hip morphology via computed tomography (CT) imaging in a rabbit model of patellar dislocation. MATERIAL AND METHODS Thirty female New Zealand White rabbits (2 months old) were included. The left hindlimbs of rabbits (n = 28 after 2 postoperative deaths) underwent patellar dislocation surgery and comprised the study group. Contralateral right hindlimbs (n = 28), which underwent no surgic

Editorial Commentary: Dynamic Anterior Stabilization Is Great! Dynamic Anterior Stabilization Augments Bankart Repair to Help Recreate Native Shoulder Kinematics.

Recurrent instability after arthroscopic Bankart repair remains a significant challenge, highlighting the importance of addressing underlying biomechanical factors beyond soft-tissue pathology alone. Dynamic anterior stabilization has emerged as an alternative stabilization strategy, using the long head of the biceps tendon to create a dynamic anterior sling effect. Recent literature provides important biomechanical evidence supporting the ability of dynamic anterior stabilization to restore glenohumeral stability more effectively than isolated Bankart. These findings suggest that dynamic ante

The Articular Dislocation Angle of the First Metatarsophalangeal Joint: Its Conception, Measurement Method, and Role in Hallux Valgus.

BACKGROUND: First metatarsophalangeal joint (MTPJ) subluxation or dislocation represents a common yet unquantified pathological feature in hallux valgus (HV). In this study, we introduce the concept of the articular dislocation angle (ADA), defined as the angle between a line perpendicular to the proximal articular surface of the proximal phalanx and a line perpendicular to the distal articular surface of the first metatarsal, to quantitatively evaluate the degree of joint subluxation or dislocation. Using the ADA concept, this study quantitatively investigated the relationships between HV-rel

Successful outcome of open distal interphalangeal joint luxation in equids treated by surgical debridement without internal implants.

INTRODUCTION: The distal interphalangeal joint (DIPJ) plays a crucial role in the function of the hoof and digit. Dislocations and subluxations in this region are rare and primarily of traumatic origin. CASE REPORT: This case report aims to describe two unusual cases of open DIPJ dislocation, including clinical presentation, diagnostic workup, treatment, and long-term follow-up. Both animals presented with open traumatic distal interphalangeal joint (DIPJ) luxation. In Case 1, the animal received initial care on the property 5 h post-trauma and presented with joint exposure and articular surf

Teenage Total Hip Arthroplasty Yields High Satisfaction and Excellent Survival up to 20-Year Follow-Up.

BACKGROUND: Total hip arthroplasty (THA) is increasingly performed in patients younger than 20 years who have destructive hip pathology. The unique anatomical and developmental characteristics of this population, along with the high likelihood of future revision surgery, have led to cautious clinical decision-making. Data on safety, effectiveness, and long-term (20 years) outcomes in teenagers remain limited. This study aimed to evaluate implant survival, patient satisfaction, functional outcomes, and complications following THA in teenagers. METHODS: This retrospective cohort study with cros

From reduction to replacement: a reconstructibility-based framework for chronic locked shoulder dislocations.

PURPOSE: Chronic locked glenohumeral dislocation is uncommon, and treatment algorithms have traditionally relied heavily on defect-size thresholds. This review proposes a unified, surgery-oriented framework for chronic locked anterior and posterior dislocations based on joint reconstructibility rather than defect size alone. METHODS: A narrative review considered PubMed-indexed literature on chronic, locked or neglected glenohumeral dislocation, including reduction techniques, joint-preserving reconstruction and arthroplasty. Shoulder-instability literature was used selectively to support bio

Hybrid approach for ventral hernia associated with rectus diastasis: endoscopic single-port diastasis plication with open preperitoneal hernia repair. ENDOP technique in 10 steps.

INTRODUCTION: Rectus Diastasis (RD) associated with ventral hernia disrupts the fascia, complicating repair. The European Hernia Society (EHS) recommends linea alba plication with mesh repair. At our center, we usually repair only the hernia in asymptomatic low-to-moderate RD to minimize complications, opting for plication when quality of life or aesthetic concerns arise. We employ the ENDOP technique, a hybrid single-incision approach combining onlay ENDoscopic plication with sublay OPen mesh repair. To our knowledge, no prior technique has addressed both conditions using different abdominal
