Cost-effectiveness of the hepatitis B birth-dose vaccine in South Africa: a mathematical modelling analysis.

BACKGROUND: In 2022, South Africa had an estimated hepatitis B prevalence of 4·7-6·0%. In an effort to eliminate viral hepatitis as a public health threat, a targeted (ie, selective) hepatitis B birth-dose vaccination (HepB-BD) policy was introduced in 2023. Under this policy, only infants born to mothers with confirmed hepatitis B positivity are eligible for HepB-BD, which is given in addition to peripartum antiviral prophylaxis (PAP) for mothers from the second or third trimester of pregnancy. This policy differs from the WHO recommendation that all newborns should receive a HepB-BD vaccine

Precise immunomodulation by mesenchymal stem cells mitigates hepatitis B virus-specific CD8⁺ T cell-mediated acute liver injury without compromising viral clearance.

Hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF) is associated with a high mortality rate. Although mesenchymal stem cell (MSC) therapy markedly improves liver function and survival in patients with HBV-ACLF, it remains unclear whether the immunomodulatory effects of MSCs suppress antiviral CD8⁺ T cell function and trigger HBV reactivation. Here, we developed a novel in vitro HBV-specific CD8⁺ T cell-mediated acute liver injury organoid model, which was co-cultured with either menstrual blood-derived MSCs (MenSCs) or umbilical cord-derived MSCs (UC-MSCs). We found that both

Chronic Hepatitis E infection in a pediatric liver transplant recipient with EBV-associated post-transplant lymphoproliferative disorder.

Chronic Hepatitis E infection is increasingly recognized in immunocompromised individuals and is primarily associated with genotype (Gt) 3, and rarely with Gt 1. Here, we report a unique case of chronic Gt1 HEV infection in a pediatric living donor liver transplant (LDLTx) recipient from India with concurrent Epstein-Barr virus (EBV)-associated post-transplant lymphoproliferative disorder (PTLD).

Molecular characterization and genetic diversity of hepatitis B virus among health care workers in the Bamenda Health District, Northwest Region, Cameroon.

OBJECTIVES: Hepatitis B virus (HBV) infection is endemic in Africa, with an estimated 82 million people living with chronic HBV infection. The outcome of exposure to HBV is a complex interplay between host immunity and viral factors, including HBV genotypes. Despite the high burden of HBV infection, data on HBV genotypes circulating in many African settings, including Cameroon, remain limited. This study investigated the genetic diversity of HBV among health care workers in the Bamenda Health District, Cameroon. METHODS: In this cross-sectional hospital-based study, HBV DNA was extracted from

Hepatitis E virus genome plasticity: a marker of chronicity and therapy failure?

Hepatitis E virus (HEV) is a globally distributed zoonotic pathogen, spread by contaminated food and blood products, and a leading cause of acute viral hepatitis. Recent data indicate that the HEV genome contains a highly heterogeneous hypervariable region (HVR) that can tolerate extensive genomic rearrangements, especially among immunocompromised patients. These rearrangements can increase HEV replication in tissue-culture and have further been observed in HEV-infected patients. Insertions comprise either duplications of virus-derived sequences, or alternatively, host-derived sequence fragmen

SARS-CoV-2 spike protein enhances HBV replication via autophagy and aggravates inflammation in CHB patients with liver dysfunction.

This study investigated the effects of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) spike protein (S protein) on hepatitis B virus (HBV) replication and immune function in patients with chronic hepatitis B (CHB), aiming to elucidate the underlying mechanisms of S protein-HBV interaction. Through in vitro cell experiments and clinical sample analyses, S protein expression was induced in HepG2.2.15 cells, leading to enhanced HBV replication; this effect was reduced by autophagy inhibitors, confirming the critical mediating role of autophagy. Furthermore, peripheral blood mono

The aMAP Score Effectively Stratifies Hepatocellular Carcinoma Risk in the Steatotic Liver Disease Population.

BACKGROUND & AIMS: Steatotic liver disease (SLD) is an increasingly important cause of hepatocellular carcinoma (HCC), yet validated risk stratification tools remain limited. We aim to evaluate the aMAP score, initially developed for HCC risk prediction mainly in viral hepatitis across international cohorts, in the SLD population. METHODS: 156 406 SLD participants were included from the UK Biobank (UKB) and 3166 SLD patients with virally suppressed chronic hepatitis B from a Chinese cohort. The predictive and stratification performance of the aMAP score was assessed and compared with the PAGE

Exploratory associations of gut microbiota composition with HBeAg and HBsAg status in nucleos(t)ide analog-treated chronic hepatitis B.

HBeAg and HBsAg reflect viral replication and host immune status and are important determinants of treatment responsiveness in chronic hepatitis B (CHB). The gut microbiota, a key component of the gut-liver axis, may influence immune regulation and antiviral outcomes. This study characterized gut microbiota profiles across different HBeAg and HBsAg statuses in CHB patients receiving nucleos(t)ide analog (NA) therapy. Fecal shotgun metagenomic sequencing was performed in 29 healthy controls (HC) and 101 NA-treated CHB patients without cirrhosis or hepatocellular carcinoma, including 30 HBeAg-po

Prevalence of Y93H resistance-associated substitution in treatment-naïve hepatitis C virus genotype 1b patients: a systematic review and meta-analysis.

BACKGROUND: Hepatitis C virus (HCV) genotype 1b infection remains a leading cause of chronic liver disease despite advances in direct-acting antivirals (DAAs). Resistance-associated substitutions (RASs) in the non-structural 5 A (NS5A) region, particularly the Y93H mutation, have been associated with reduced antiviral susceptibility and variable treatment outcomes. The global baseline prevalence of this substitution in treatment-naïve populations has not been systematically characterized. METHODS: A systematic review and meta-analysis were conducted in accordance with PRISMA 2020 guidelines a

The new landscape of liver transplantation: Evolving candidates, new technologies, and personalized care.

Liver transplantation (LT) is entering a new era shaped by changes in disease epidemiology, candidate complexity, donor utilization, and post-transplant management. The decline of viral hepatitis-related cirrhosis, together with the rise of metabolic dysfunction-associated steatotic liver disease, alcohol-related liver disease, and hepatocellular carcinoma, has transformed the LT waitlist into an older, frailer, and more cardiometabolically complex population. At the same time, donation after circulatory death and machine perfusion are expanding the donor pool and redefining the boundaries of

Similar Long-Term Incidence of Hepatocellular Carcinoma in Chronic Hepatitis B Initiating Entecavir Versus Tenofovir Alafenamide: A REAL-B Study.

Data on potential differences in hepatocellular carcinoma (HCC) incidence between entecavir (ETV) and tenofovir alafenamide (TAF) in chronic hepatitis B (CHB) patients are limited. This retrospective multinational study included treatment-naïve adults with CHB without a history of HCC who initiated ETV or TAF therapy since 2016 at 23 centres in 4 countries. Baseline characteristics were balanced using inverse probability of treatment weighting (IPTW) and propensity score matching (PSM). HCC incidence and the association between antiviral regimen and HCC were analysed using competing-risk analy

Risk of Hepatocellular Carcinoma or Hepatic Decompensation Associated With Steatotic Liver Disease in People With and Without HIV.

BACKGROUND: Steatotic liver disease (SLD) is highly prevalent in people with HIV (PWH), but factors influencing progression to hepatocellular carcinoma (HCC) and hepatic decompensation (HD) remain unclear. It is unknown if the risk of these outcomes associated with SLD differs by HIV status. METHODS: Patients from the Veterans Aging Cohort Study-HIV (includes matched people without HIV [PWoH]) who underwent an ultrasound, computed tomography, or magnetic resonance imaging between 2001-2017 were included. Patients with diagnosed cirrhosis or chronic viral hepatitis were excluded. SLD was defin

Beyond fibrosis: the molecular roadmap from MASLD to hepatocellular carcinoma.

Metabolic dysfunction-associated steatotic liver disease (MASLD) has overtaken viral hepatitis as the fastest-growing cause of hepatocellular carcinoma (HCC) in most high-income countries. What makes this epidemic clinically treacherous is a feature that distinguishes MASLD-related HCC from other aetiologies: roughly half of all cases arise in livers that have never progressed to cirrhosis, slipping through the net of conventional surveillance strategies. The molecular journey from fatty liver to malignancy unfolds through at least six interlocking programmes: lipotoxic rewiring of hepatocyte

Distinct immune signatures define hepatitis D- versus hepatitis B-associated hepatocellular carcinoma.

BACKGROUND: Hepatocellular carcinoma (HCC) is a leading cause of cancer-related mortality worldwide. Hepatitis D is the most severe form of viral hepatitis. However, the clinical, virological, and systemic inflammatory characteristics of HDV-associated HCC remain insufficiently characterized. METHODS: In this single-center retrospective study, we identified 162 patients with HCC due to chronic hepatitis D (HDV-HCC; n=54) or chronic hepatitis B (HBV-HCC; n=108). Plasma levels of 92 soluble inflammatory mediators (SIMs) were quantified using proximity extension assay in HCC patients [HBV-HCC (n

Host restriction factors in HBV-related liver disease and hepatocellular carcinoma: mechanisms of viral-host interplay and implications for therapy.

Chronic hepatitis B virus (HBV) infection is a leading cause of liver cirrhosis and hepatocellular carcinoma (HCC), imposing a substantial global health burden. The natural history of persistent HBV infection progresses from chronic inflammation to fibrosis, cirrhosis, and malignant transformation, driven in part by the stability of covalently closed circular DNA (cccDNA) within hepatocyte nuclei. Nucleos(t)ide analogues (NUCs) effectively suppress viral replication but rarely lead to a functional cure, necessitating long-term therapy and carrying a residual risk of resistance.
