The HBV basal core promoter mutation confers a replicative advantage and transcriptionally reprograms hepatocytes toward HCC subtypes.

BACKGROUND: Hepatitis B virus (HBV) basal core promoter (BCP) and precore (PC) mutations occur during chronic HBV infection with the BCP-mutant being associated with increased hepatocellular carcinoma (HCC) risk. OBJECTIVES: The effects of these mutants on viral replication, hepatocyte biology, and carcinogenesis remain poorly defined. To address this, we characterized BCP- and PC-mutants using human hepatocyte chimeric mice and asked whether the resulting infection-induced transcriptomes correspond to subsets of human HBV-related hepatocellular carcinomas (HBV-HCCs). DESIGN: Isogenic wild-t

Changing epidemiology and etiology of hepatocellular carcinoma in Europe: implications for clinical practice and health policy.

Primary liver cancer, of which hepatocellular carcinoma (HCC) represents the vast majority, is on the rise globally. Despite an expected decrease in viral hepatitis-related HCCs, the number of new cases and deaths from liver cancer is predicted to increase by over 30% in 2050 in Europe. This is largely driven by an increase in metabolic dysfunction-associated steatotic liver disease-related HCC and an aging population. Since HCC usually develops in patients with underlying chronic liver disease, over half of all cases appear to be preventable by targeting modifiable risk factors. This series p

THE ROLE OF CELL THERAPY IN THE COMPREHENSIVE TREATMENT OF CHRONIC HEPATITIS C-RELATED LIVER CIRRHOSIS.

BACKGROUND: Despite the high efficacy of direct-acting antiviral (DAA) therapy for chronic hepatitis C virus (HCV) infection, a considerable proportion of patients continue to exhibit significant structural liver damage, including advanced fibrosis and cirrhosis. Therefore, the search for additional therapeutic approaches aimed at enhancing liver regeneration and improving hepatic functional status remains highly relevant. OBJECTIVE: To evaluate the clinical efficacy of fetal stem hepatocyte transplantation as part of a comprehensive treatment strategy for liver cirrhosis associated with chro

Integrated computational approaches for identifying and evaluating phytochemical inhibitors against HCV NS3/4A protease.

Hepatitis C virus (HCV) remains a major global health concern, causing chronic hepatitis, cirrhosis, and hepatocellular carcinoma (HCC), with an estimated 185 million people affected worldwide. In the absence of an FDA-approved vaccine, the development of effective antiviral strategies remains essential. The HCV NS3-4A protease, which mediates cleavage of the viral polyprotein at specific downstream sites of NS3 and is essential for replication-complex formation, represents a key therapeutic target. In this study, approximately 1,480 phytocompounds were compiled from the IMPPAT database and pr

Hepatic Steatosis in People With Chronic Hepatitis B in Africa: Prevalence, Correlates, and Clinical Significance.

To address data gaps around metabolic liver disease in Africa, within the Hepatitis B in Africa Collaborative Network (HEPSANET), we evaluated participants with chronic hepatitis B virus infection (CHB) for steatotic liver disease (SLD) using transient elastography with controlled attenuation parameter (CAP). Using multivariable logistic regression, we identified sociodemographic, anthropometric, and clinical associations with SLD (CAP ≥ 248 dB/m), and evaluated whether SLD severity was independently associated with fibrosis. Short-term impacts of SLD on mortality were also assessed with Cox p

RESONANCE: Protocol for a Nationwide Cohort Study of Chronic Liver Diseases Using the French National Health Data System.

BACKGROUND: Chronic liver diseases (CLDs) are frequent in Europe, including in France, and are mainly driven by alcohol, metabolic dysfunction (diabetes and obesity), and viral hepatitis. Although risk factors are well established and easy to identify, CLDs are frequently diagnosed at an advanced stage, translating into poor prognosis. Descriptive data on CLD burden in France remain scarce, and health trajectories of these patients are uncharted. However, such data are crucial to guide clinical practice, to determine the target population for personalized public health policies, and to develop

Becoming Physically Active and Risks of Mortality and Liver-Related Events in Adults With Chronic Hepatitis B: A Nationwide Cohort Study.

Physical activity (PA) confers several health benefits; however, whether increasing PA among physically inactive adults with chronic hepatitis B (CHB) is associated with lower risks of adverse clinical outcomes remains unclear. We used the nationwide Korean National Health Insurance database. We included 318,131 adults with CHB who were physically inactive at the initial health screening (Exam 1) and completed a subsequent screening within 4 years (Exam 2). Among them, 58,703 initiated moderate-to-vigorous PA (MVPA), defined as vigorous activity ≥ 20 min/day on ≥ 3 days/week or moderate activi

Direct-Acting Antiviral Therapy in Patients With Hepatocellular Carcinoma Receiving Non-Curative Treatment: High Sustained Virological Response Rates and Preservation of Liver Functional Reserve.

Direct-acting antivirals (DAAs) cure most chronic hepatitis C virus (HCV) infections, but their impact on liver functional reserve in patients with a history of hepatocellular carcinoma (HCC), particularly after non-curative HCC treatment, remains unclear. We evaluated antiviral efficacy and liver functional reserve after DAA therapy according to HCC history and timing of DAA initiation. In this retrospective single-center cohort, 716 patients initiated DAA therapy, and 690 with evaluable sustained virological response at 12 weeks after treatment (SVR12) were analysed. Patients were classified

Finite Versus Indefinite Nucleos(t)ide Analogue Therapy in HBeAg‑Negative Chronic Hepatitis B: Meta‑Analysis of Randomized Trials.

Continuous nucleos(t)ide analogue (NA) therapy for chronic hepatitis B (CHB) effectively maintains viral suppression but rarely results in functional cure. Finite NA therapy has been proposed to increase rates of HBsAg loss; however, international guidelines provide conflicting recommendations. This study compares the efficacy and safety of finite versus continuous NA therapy in HBeAg-negative adults using randomized controlled trials. We included parallel-group randomized controlled trials (RCTs) comparing nucleos(t)ide analogue (NA) cessation versus continuation in non-cirrhotic, virally sup

Correlation and discriminatory performance of non-invasive serum fibrosis indices according to liver disease etiology using two-dimensional shear wave elastography as reference.

BACKGROUND/OBJECTIVES: Non-invasive serum fibrosis indices are widely used for liver fibrosis assessment; however, their correlation with liver stiffness and diagnostic performance may vary according to liver disease etiology. This study aimed to evaluate the etiology-dependent correlation between multiple serum Non-Invasive Test (NITs) indices and liver stiffness measured by two-dimensional Shear Wave Elastography (2D-SWE), and to explore their ability to discriminate patients according to 2D-SWE-based fibrosis categories. METHODS: We conducted a retrospective cross-sectional study including

Family cluster screening may help identify hepatitis D virus infections among individuals from highly endemic regions.

Hepatitis D virus (HDV) infection is the most severe form of chronic viral hepatitis and may remain substantially underdiagnosed, particularly among individuals originating from highly endemic regions. We report a Romanian family cluster identified through cascade screening following the diagnosis of an index case living in Spain. Six household contacts and first-degree relatives underwent hepatitis B virus (HBV) and HDV screening, identifying three additional cases of chronic HDV infection. Notably, all affected individuals had advanced liver disease at diagnosis, and two subsequently require

Exploring the independent factors influencing prolonged QTc intervals in patients with acute-on-chronic liver failure caused by hepatitis B.

Prolonged QTc interval is a common electrophysiological abnormality in patients with chronic liver disease, but limited evidence exists regarding its occurrence in acute-on-chronic liver failure (ACLF), particularly in hepatitis B virus-related ACLF (HBV-ACLF). Given the rapid progression, high mortality, and severe complications associated with ACLF, understanding cardiac electrophysiological changes is clinically important. This study aimed to determine the incidence of QTc prolongation in patients with HBV-ACLF and to identify independent factors associated with QTc abnormalities. A retrosp

PYHIN proteins: guardians or contributors to hepatitis B virus pathogenesis?

Human pyrin and HIN domain (PYHIN) family proteins play central roles in the host defense against viral infection. These PYHIN proteins, including IFI16, AIM2, MNDA and PYHIN1, mainly function as intracellular DNA sensors. Among them, IFI16 is supported by the most robust mechanistic evidence, including direct binding to hepatitis B virus (HBV) covalently closed circular DNA (cccDNA) in the nucleus and subsequent suppression of viral transcription. They recognize HBV DNA in different cellular substructures, triggering innate immune responses and interfering with steps of the viral life cycle.

Combining Antiviral Therapy with Cancer Treatment in HBV-associated HCC: A Translational Approach.

Hepatitis B virus (HBV) infection remains a major global health concern and is a leading cause of hepatocellular carcinoma (HCC). Despite advances in antiviral therapy and cancer treatment, HBV-related HCC (HBV-HCC) continues to pose significant challenges due to its complex pathogenesis involving chronic inflammation, viral persistence, and liver fibrosis. Current antiviral therapies reduce viral load and liver inflammation but often fail to eliminate integrated viral DNA or prevent carcinogenesis. On the other hand, cancer therapies such as surgery, ablation, immunotherapy, and targeted ther

Transposable Element Dysregulation in Hepatocellular Carcinoma: Epigenetic Mechanisms, Immune Remodeling, and Translational Opportunities.

Hepatocellular carcinoma (HCC) commonly develops in the context of chronic liver injury, viral hepatitis, metabolic dysfunction, alcohol-related liver disease and cirrhosis. Although immune checkpoint inhibitor-based combination therapies have improved the treatment of unresectable HCC, primary and acquired resistance and the lack of reliable predictive biomarkers continue to limit precision treatment. Transposable elements (TEs) and TE-derived sequences represent a regulatory layer linking epigenetic disruption, aberrant transcription and immune remodelling. In HCC, DNA hypomethylation, alter

"Five Decades On: metALD Emerges as Liver Disease's New Frontier".

This year marks the 50th anniversary of the American Liver Foundation (ALF). Over the five ensuing decades since its inception, the ALF has advocated for and advanced research and public awareness in liver disease. The rise and subsequent improvement of viral hepatitis prevalence during this time contrasts with rising incidence of alcohol-related liver disease and metabolic dysfunction-associated steatotic liver disease (MASLD). Moreover, the concomitant actions of alcohol toxicity in context of metabolic liver disease are now recognized as metabolic and alcohol-related liver disease (metALD).
