BACKGROUND: The optimal position and number of anchors that need to be implanted in Hill-Sachs lesions (HSLs) during remplissage with Bankart repair (RMBR) to correct anterior glenohumeral instability remain unclear. Previous studies have mainly focused on anchor placement in the central, medial, and peripheral regions, as well as the superior and inferior poles of the HSL, but anchor placement in the upper or lower regions of the HSL has not been investigated.
PURPOSE: To assess, using finite element analysis, the effect of placing 2 anchors in the superior or inferior regions of the HSL dur

INTRODUCTION: Recurrent patellofemoral instability is frequent in adolescents and young adults, often associated with pain, cartilage damage, and functional impairment. While medial patellofemoral ligament (MPFL) reconstruction is considered the gold standard, it has limitations in skeletally immature patients and revision settings. Increasing attention has therefore been given to the reconstruction of medial quadriceps tendon–femoral ligament (MQTFL), a complementary stabiliser of the patella as the procedure avoids patellar bone tunnels and may reduce complications. However, current evidence

INTRODUCTION: Fungal periprosthetic joint infections (PJI) are exceptionally rare, accounting for <1% of all PJIs. Among them, Candida albicans is the most frequently implicated pathogen, yet its management remains highly challenging.
CASE REPORT: We report the case of an 85-year-old female who presented with a draining sinus at the right hip, 4 weeks after revision total hip arthroplasty (THA). Her surgical history included a prior failed revision due to recurrent dislocation. Clinical and laboratory findings suggested PJI. Intraoperative cultures and sonication confirmed multimicrobial and

OBJECTIVE: To assess the clinical efficacy of medial patellofemoral ligament reconstruction (MPFL-R) combined with medial-approach derotational distal femoral osteotomy (DDFO) in the treatment of recurrent patellar dislocation (RPD).
METHODS: In this retrospective Level IV case series, we reviewed 45 patients with recurrent patellar dislocation (2 males, 43 females; age 13-44 years, mean 22.8 ± 6.0 years) and a femoral anteversion angle (FAA) > 30° as measured using the Jarrett method (Kaiser et al. in Arch Orthop Trauma Surg 136(9):1259, 2016), who underwent combined medial patellofemoral li

INTRODUCTION: Recurrent patellar dislocation presents a significant rehabilitation challenge, particularly in athletes engaged in high-demand, multi-directional sports such as badminton. This case report describes a complex presentation in a state-level athlete with multiple patellar dislocations, followed by medial patellofemoral ligament (MPFL) reconstruction and a subsequent comminuted patellar fracture. The case was further complicated by proprioceptive deficits, extension lag, sensory changes, and kinesiophobia, all of which limited the athlete's functional performance and return to sport

The management of shoulder injuries resulting from epileptic seizures presents a particular challenge. These injuries often present complex patterns, ranging from dislocations and fractures to damage to the rotator cuff. Recurrent seizures can significantly impact the outcome and must be considered a central risk factor when choosing the therapeutic approach. Accordingly, these injuries require close multidisciplinary cooperation between orthopaedics/trauma surgery, neurology/epileptology and physiotherapy, both in diagnosis and treatment. Despite their clinical relevance, standardised treatme

BACKGROUND: Chronic acromio-clavicular dislocation is a challenging injury to treat. Many surgical procedures are available for its management which may be anatomical or non-anatomical. The aim of this study was to assess the clinical and radiological outcome in the patients with chronic acromio-clavicular dislocation treated by hybrid biological semitendinosus autograft and synthetic non-absorbable sutures.
METHODS: From July 2021 to January 2025, 21 patients with chronic acromio-clavicular dislocation were treated by hybrid biological semitendinosus autograft and synthetic non-absorbable su

BACKGROUND: About 10% to 16% of patients with patellar instability have bilateral involvement. The risk factors for future contralateral patellar instability in patients who have ipsilateral patellar instability are not known. Knowledge related to the possibility of future contralateral patellar instability would be helpful, as previous studies have shown suboptimal outcomes and increased complication rates in patients with bilateral patellar instability.
PURPOSE: The purpose of the current study was to identify the risk factors for future contralateral patellar instability and develop a pred

BACKGROUND: The high rates of failures and reoperations (up to 50%) after arthroscopic posterior Bankart repair (APBR) remain a concern.
PURPOSE: To evaluate mid- to long-term follow-up outcomes of APBR in patients with unidirectional, recurrent posterior shoulder instability and to identify risk factors for failure.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: A single-center, retrospective study was conducted that included all consecutive athletes with unidirectional recurrent posterior shoulder instability operated on between 2006 and 2022. Patients with voluntary instabilit

INTRODUCTION: The Latarjet procedure is the mainstay treatment in high-demand patients with substantial glenoid bone loss or after failed capsulolabral repairs. Patients typically return to sport (RTS) within 6 months postoperatively, requiring intensive rehabilitation. Current rehabilitation protocols focus on mobility, strength and stability. Yet, psychological factors, such as fear of reinjury, are the main reason not to RTS. Therefore, this study aims to determine whether integrating psychological interventions into postoperative rehabilitation improves patient-reported shoulder function c

BACKGROUND: Subscapularis (SSC) insufficiency after reverse total shoulder arthroplasty (rTSA) may result in recurrent anterior dislocation or persistent internal rotation (IR) weakness, both of which impair function. Secondary pectoralis major (PM) transfer has been proposed as a salvage option, but supporting clinical evidence is limited. METHODS: We retrospectively analyzed six consecutive patients who developed SSC retear following rTSA with concomitant SSC repair between 2020 and 2023. Indications for revision were recurrent anterior dislocation (n = 3) or disabling IR weakness affecting

BACKGROUND: Posterior sternoclavicular joint (SCJ) dislocations are uncommon because of the strength of the posterior joint capsule but pose a significant risk of neurovascular and mediastinal compression. Various surgical techniques have been described, with the sternal docking reconstruction using tendon allografts demonstrating improved biomechanical stability, elimination of anterior bulk, and ease of graft passage. This study outlines the intramedullary posterior SCJ reconstruction with a semitendinosus allograft for recurrent symptomatic posterior SCJ dislocations.

BACKGROUND: The Dejour classification is widely accepted for grading the severity of trochlear dysplasia. However, whether it serves as a reliable prognostic marker for patellar instability surgery or a guide for clinical decision-making has yet to be established. Medial patellofemoral ligament (MPFL) reconstruction, with or without tibial tubercle osteotomy (TTO), yields comparable outcomes across Dejour grades. Nonetheless, we hypothesized that specific trochlear parameters would be more significant predictors of surgical results.
METHODS: This study included patients who underwent MPFL rec

BACKGROUND: Medial patellofemoral ligament reconstruction (MPFL-R) is the primary surgical intervention for recurrent lateral patellar dislocation (LPD). Isolated MPFL-R is recommended for patients without anatomic high-risk factors that predispose to further dislocations.
PURPOSE: To compare instability recurrence in patients with recurrent LPD without underlying anatomic risk factors treated with MPFL-R versus active rehabilitation.
STUDY DESIGN: Randomized controlled clinical trial; Level of evidence, 1.
METHODS: Patients aged 12 to 30 years with recurrent LPD and no underlying anatomic

BACKGROUND: Kawasaki disease shock syndrome (KDSS) is a severe and uncommon complication of Kawasaki disease (KD). Grisel's syndrome (GS), a rare non-traumatic atlantoaxial subluxation, has been seldom described in KD. To our knowledge, this is the first documented case in which GS was the initial manifestation of KD and preceded the development of KDSS.
CASE PRESENTATION: A 4-year-old boy presented with fever, cervical lymphadenopathy, and restricted neck mobility. Cervical computed tomography revealed atlantoaxial subluxation. He was initially diagnosed with cervical lymphadenitis and treat
