Arthroscopic transglenoid dynamic subscapular sling achieves short-term clinical outcomes in patients with recurrent shoulder dislocation.

PURPOSE: This study evaluated short-term outcomes of arthroscopic transglenoid dynamic subscapular sling (ATDSS) for recurrent anterior shoulder instability (RASI) with < 20% glenoid bone loss (GBL). METHODS: A retrospective series of 25 patients (mean age 36.4 ± 17.3 years) undergoing ATDSS using an adjustable-length loop cortical suspensory fixation device with a tendon graft (semitendinosus autograft, palmaris longus autograft, or tibialis anterior allograft) around the upper subscapularis tendon was analyzed. Outcomes included Oxford Shoulder Instability Score (OSIS), visual analogue scal

Medial patellotibial ligament reconstruction concomitant with medial patellofemoral ligament (MPFL) reconstruction in fixed and obligatory patellar dislocation: A consecutive case series.

INTRODUCTION: Patellofemoral instability (PFI) is a common knee injury in the pediatric population, and medial patellofemoral ligament (MPFL) reconstruction is common surgery to minimize risk of chronic PFI. Medial patellotibial ligament (MPTL) reconstruction may be subsequently performed, providing more lateral stability to reduce rotational instability and tilt. Recent literature has described MPTL reconstruction in adults and nonpathologic pediatric patients; however, little literature has discussed this procedure in pediatric patients with underlying abnormal patellofemoral morphology. Extension block pinning with percutaneous intramedullary reduction for dorsal PIP joint fracture-dislocation of the finger.

BACKGROUND: Dorsal fracture-dislocations of the proximal interphalangeal (PIP) joint can cause chronic pain, stiffness, and loss of motion. We present a minimally invasive method-extension block pinning combined with percutaneous intramedullary reduction and fixation-for unstable dorsal PIP fracture-dislocations and evaluate its radiologic and functional outcomes. METHODS: Twenty consecutive patients with acute unstable dorsal PIP joint fracture-dislocations and persistent post-reduction subluxation were treated with this technique. In the treated cohort, volar articular involvement ranged fr

Optimizing medial patellofemoral ligament reconstruction: a randomized trial of 3D-printed guide plates for enhanced femoral tunnel accuracy and early functional recovery.

This study aimed to determine whether patient-specific 3D-printed guide plates enhance femoral tunnel placement accuracy, reduce operative time, and promote faster functional and kinematic recovery in medial patellofemoral ligament (MPFL) reconstruction compared to conventional techniques. Prospective, randomized controlled trial. Single academic tertiary care hospital. Sixty patients diagnosed with recurrent patellar dislocation were enrolled and randomly assigned to either a 3D-printed guide plate group (n = 30) or a conventional fluoroscopy-guided group (n = 30). Patients in the interventio

Outcomes following Medializing Tibial Tubercle Osteotomy for Lateral Patellar Instability in Patients with Patella Alta.

INTRODUCTION: Lateral patellar instability is characterized by patellar subluxation or dislocation secondary to traumatic injury, ligamentous laxity, or other pathoanatomy of the knee. Management often requires surgical stabilization via a tibial-tubercle osteotomy. The purpose of this study is to determine if patella alta is a risk factor for poor postoperative outcomes and return to activity following a medializing tibial-tubercle osteotomy in a young, active-duty military population. MATERIALS AND METHODS: This is a retrospective review of 42 military patients who received a medializing ti

Surgical management of posterolateral rotatory instability of the elbow with associated posterior capitellar lesions: a retrospective case series.

PURPOSE: Elbow dislocations are associated with complex ligamentous and osseous injuries, leading to a spectrum of instability patterns, most notably posterolateral rotatory instability (PLRI). Among these, bony defects of the posterior capitellum are commonly observed in patients following elbow dislocation or presenting with persistent instability. The clinical relevance of these lesions and the optimal management strategy remain subject of ongoing debate. This study aimed to assess the outcomes of surgically treated PLRI in the presence of posterior capitellar lesions, with the hypothesis t

Editorial Commentary: Retaining Its Clinical Importance: Open Bankart Repair With Inferior Capsular Shift in Collision and Contact Athletes With Recurrent Anterior Shoulder Instability Retains Its Clinical Importance-But Will It Also Endure in the Future?

Recurrent anterior shoulder instability (RASI) is frequently seen especially in young collision and contact athletes. The glenohumeral joint stability is often compromised by associated Hill-Sachs and Bankart lesions. In young and active patients with high risk of recurrence or failed nonoperative therapy a surgical treatment is indicated to stabilize the shoulder joint and to restore a painless shoulder function. Different open surgical and arthroscopic procedures have been described to achieve these aims in consideration of present soft-tissue lesions and potential glenohumeral bony defects

Age, glenoid bone fragment, and glenoid bone loss increase recurrence risk after arthroscopic bony Bankart repair.

AIMS: Numerous studies have identified risk factors for recurrent anterior shoulder dislocation, but the exact impact of each remains unclear. This study analyzes outcomes after arthroscopic bony Bankart repair (ABBR) and evaluates how time to surgery, number of preoperative instability events, age, glenoid bone loss (GBL), glenoid bone fragment (GBF), age at surgery, and preoperative Western Ontario Shoulder Instability (WOSI) scores influence recurrence. METHODS: A retrospective review was conducted of patients who underwent ABBR between January 2008 and December 2018. Data included demogra

[Controlled hip dislocation in synovial chondromatosis. Case report].

INTRODUCTION: hip pain is a common symptom linked to trauma, joint wear and tear or genetic predisposition. In the presence of progressive pain and functional disability without the above causes, it is necessary to explore and rule out other less frequent causes of joint damage, such as synovial chondromatosis. Currently, surgical dislocation of the hip represents an effective and safe alternative to treat this condition. CASE REPORT: male patient with synovial chondromatosis of the hip, characterized by progressive functional limitation, without relevant history, and with evolution of chroni

Cost-effectiveness of Adding Remplissage to Bankart Repair for On-track Hill-Sachs Lesions: A Markov Decision Analysis.

BACKGROUND: Remplissage decreases recurrence after arthroscopic Bankart repair for anterior shoulder instability but has historically been reserved for off-track Hill-Sachs lesions. The addition of remplissage for patients with on-track lesions remains controversial because of concerns about added operative costs and potential loss of range of motion. The cost-effectiveness of remplissage in patients with on-track Hill-Sachs lesions has not been established. PURPOSE: To evaluate the cost-effectiveness of arthroscopic Bankart repair with remplissage versus isolated Bankart repair in patients w

Patellofemoral instability: Anatomy, imaging techniques, and clinically relevant measurements.

The patellofemoral (PF) joint plays an integral role in knee biomechanics. Disorders of the PF joint are prevalent and a common source of anterior knee pain, particularly in younger and active patients. Abnormalities related to patellar tracking can lead to recurrent lateral patellar dislocation, early degenerative changes, and poor clinical outcomes if not detected and treated appropriately. Therefore, early recognition of PF instability is imperative to preserving joint function and improving long-term patient outcomes. Several alignment and morphological abnormalities are known risk factors

[Impact of associated injuries on anterior glenohumeral instability treated with Latarjet surgery: functional outcomes at 2-year follow-up].

INTRODUCTION: in anterior glenohumeral dislocation, in addition to classic lesions such as Hill-Sachs (HS), Bankart, or glenohumeral anteroinferior ligament injury, other associated lesion like superior labral (SLAP), osteocondral (LOC), or glenolabral articular disruption (GLAD) have been described. Although open Latarjet surgery yields excellent outcomes, it does not address these associated lesions. OBJECTIVE: to compare functional outcomes, recurrence rate, persistent symptoms and return to sport in patients undergoing open Latarjet surgery with and without associated lesions diagnosed by

Comparison of Bankart repair with remplissage and Latarjet procedure for anterior shoulder instability: a systematic review and meta-analysis.

BACKGROUND: Anterior shoulder instability is the most common type of shoulder instability. For cases with subcritical glenoid bone loss, both Bankart repair with remplissage (BR) and the Latarjet procedure are viable surgical options. The aim of this study was to compare the postoperative outcomes in patients undergoing BR and Latarjet procedure. METHODS: This review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guideline. The methodological index for nonrandomized studies (MINORS) was used to characterize the methodological q
