Postoperative instability following total knee arthroplasty (TKA) is a serious complication that challenges both the surgeon and the patient. Management becomes particularly complex when instability is recurrent, as restoration of mechanical alignment and soft-tissue balance is critical for a successful outcome. We report a case of a man in his sixties who presented with knee dislocation after a primary TKA performed elsewhere. The patient subsequently underwent tibial insert exchange, following which he again experienced recurrent dislocation. Definitive management was achieved with revision

Reverse Hill-Sachs lesions pose a complex challenge in the management of posterior shoulder dislocations. These lesions complicate surgical management as a result of their association with recurrent instability and further joint destruction. We describe an arthroscopic reverse remplissage technique that utilizes both the long head of the biceps tendon and the subscapularis to address reverse Hill-Sachs defects. This combined approach provides comprehensive defect filling and enhanced stability while avoiding the morbidity associated with extensive subscapularis tenodesis.

Background/Objectives: Recurrent temporomandibular joint dislocation is associated with repeated dislocation episodes, pain, impaired jaw function, and psychosocial burden. Injectable intra- and periarticular therapies have been proposed as minimally invasive methods of improving joint stability, but the available evidence is heterogeneous and frequently involves adjunctive procedures. This mapping review aimed to characterize injectable treatments whose effects could be assessed independently from simultaneous non-injectable interventions. Methods: PubMed (MEDLINE), Europe PMC, and BASE were

Recurrent lateral patella instability is primarily a result of lateral patella entry to the femoral trochlea. Using 3-D imaging, one can accurately assess this lateral entry more accurately than with plain radiographs, making 3-D imaging important for optimal surgical planning, particularly when deciding whether to do an anteromedial tibial tubercle transfer osteotomy (AMTTO). To understand the laterality of patella entry to the femoral trochlea of a patient, one must understand the patella entry zone and its midpoint, defined as the entry point (EP) to the trochlea, and then measure its coron

INTRODUCTION: Medial patellofemoral ligament (MPFL) reconstruction is a well-established treatment for recurrent lateral patellar dislocations, yielding satisfactory clinical outcomes. Although synthetic materials are not widely used due to limited long-term data, they offer the potential to eliminate donor-site complications and may provide promising results. This study evaluated the five-year clinical and functional outcomes of MPFL reconstruction using suture tape, hypothesising that it is a safe alternative to traditional grafts.
METHODS: Thirty patients aged 20 - 45 years with recurrent

PURPOSE: Anterior shoulder instability (ASI) is a common shoulder condition that primarily affects adolescents and young adults. In older adults this represents a distinct condition with a distinct pathomechanism and complication profile. This review synthesizes the literature on ASI in patients 40 years or older, focusing on injury characteristics, treatment modalities, and clinical outcomes.
METHODS: Three databases (PubMed, MEDLINE, Embase) were searched on October 18, 2025. Primary studies investigating operative or non-operative management of ASI in patients with onset of instability at

BACKGROUND AND PURPOSE: Primary traumatic anterior shoulder dislocations are common and can have long-term personal implications for quality of life and the use of the arm for occupational activities and sport participation. A patient care pathway has been reported to contribute to improved quality of care and a reduction in re-dislocations. We developed clinical practice guidelines intended to provide healthcare professionals with an updated pathway for the optimal diagnosis and management of traumatic primary anterior shoulder dislocations.
METHODS: The clinical practice guidelines were dev

Foot and ankle injuries are among the most common musculoskeletal conditions encountered in both athletic and general populations, accounting for a substantial proportion of emergency and sports medicine visits. These injuries encompass a wide spectrum, from acute ligamentous sprains to more subtle and frequently misdiagnosed conditions such as Lisfranc ligament injuries, tarsal coalitions, and posterior tibial tendon dislocations. The complex anatomy and biomechanics of the foot and ankle, combined with the high functional demands placed upon them during gait and athletic activity, underscore

INTRODUCTION: Irreducible lesser-toe interphalangeal joint dislocation in children is rare, but may cause pain, recurrent dislocations, and soft-tissue injury. The optimal method of fixation remains unclear. We present the first-ever reported case of an irreducible fourth toe proximal interphalangeal joint dislocation in a 5-year-old boy, successfully fixed with JuggerKnot™ soft mini-anchor.
CASE REPORT: A 5-year-old boy with a crush injury to his left fourth toe, shown persistent dorsolateral dislocation of the proximal interphalangeal joint. Intraoperatively, close reduction failed, and we

BACKGROUND: Limb salvage is the mainstay of surgical treatment for primary sarcomas as well as metastatic lesions of the proximal humerus. This involves wide excision of the involved humeral segment followed by a reconstruction of the defect. The purpose of this review was to determine which reconstruction modality offers a better overall functional outcome.
METHODS: After registering a review with the PROSPERO database (ID No. CRD 42025642215), PubMed and Cochrane Library databases were systematically screened for relevant studies meeting our inclusion criteria. The Methodological Index for

Operative management should be considered from the initial episode of anterior shoulder instability to prevent bone loss, minimize risk of recurrence, maximize the potential for return to sport at the preinjury level, and from a cost-effectiveness standpoint. The glenoid track concept will be examined as a crucial factor in determining whether bone loss is significant. The increasingly comprehensive understanding of the dynamic interactions between bone loss on the humeral head and glenoid rim will be further discussed as a key factor influencing both surgical management and outcomes. The indi

Introduction: Total hip arthroplasty is one of the most successful modern surgical procedures, yet periprosthetic joint infection (PJI) remains a serious complication associated with disability and increased postoperative mortality. Aim: This study highlights the importance of identifying the causative microorganism and its antibiotic susceptibility profile for the successful application of a one-stage revision protocol in treating PJI. Materials and methods: Between October 2017 and July 2023, 33 patients underwent one-stage revision total hip arthroplasty. Preoperative joint aspiration and i

INTRODUCTION: Shoulder dislocation following trauma is associated with high rates of recurrence. Numerous surgical techniques, although effective, often reduce the shoulder's range of motion. The Modified Boytchev procedure has emerged as a preferred method for treating recurrent anterior shoulder dislocation.
MATERIALS AND METHODS: This prospective observational study was conducted between August 2022 and February 2024 in the Department of Orthopaedics at a tertiary hospital. Thirty patients with recurrent anterior dislocations were enrolled and treated using the Modified Boytchev procedure.

INTRODUCTION: Patellar instability is an increasingly prevalent condition in the adolescent population, often requiring surgical intervention for traumatic or recurrent dislocations. Despite growing recognition of health disparities in orthopaedic surgery, little is known about sociodemographic factors associated with delays to care. We hypothesized that government insurance and higher Area Deprivation Index (ADI) would be associated with increased time to care.
METHODS: A retrospective review was conducted of pediatric patients (ages 7 to 19 years) undergoing surgery for traumatic patellar i

CASE: A 26-year-old man with recurrent posterior shoulder instability in the setting of glenoid dysplasia (41° of glenoid retroversion and 18% posterior glenoid bone loss) underwent combined opening-wedge osteotomy and posterior bone-block augmentation surgery. Postoperative imaging demonstrated retroversion correction to 9° of residual glenoid retroversion and increase in the glenoid arc by 22%. At 18 months, he reports no pain or instability and has regained full motion and strength.
CONCLUSION: Combined glenoid osteotomy and posterior bone block technique is a novel treatment option for po

INTRODUCTION: Recurrent anterior shoulder instability is a common complication after first-time traumatic dislocation, particularly in young patients treated conservatively. While established risk factors include age and osseous defects, the influence of glenoid morphology - specifically glenoid version - remains incompletely understood. The purpose of this study was to evaluate whether glenoid version is associated with recurrent instability following nonoperative treatment of first-time anterior shoulder dislocation.
METHODS: This retrospective study included 72 patients (aged 18-40 years)

BACKGROUND: Locked posterior fracture-dislocation of the proximal humerus is a rare and technically demanding injury in which the humeral head remains impacted posterior to the glenoid. Although several reduction techniques have been described, the practical details of anterior-only reduction remain incompletely standardized. We evaluated a structured three-step anterior reduction strategy performed through a standard deltopectoral approach.
METHODS: We retrospectively reviewed 16 consecutive patients with acute locked posterior fracture-dislocation of the proximal humerus treated between Jan

BACKGROUND: Recent research has highlighted the adverse effects of immobilization in shoulder rehabilitation. The present study aimed to compare the efficacy of immediate self-rehabilitation with and without sling immobilization following an open Latarjet procedure for recurrent anterior shoulder instability.
METHODS: This monocentric randomized controlled trial included 86 patients (median age, 26; 92% male) divided into 2 groups: sling immobilization for 3 weeks postoperatively and no sling immobilization. Patients were evaluated with use of patient-reported outcome measures (PROMs), motion
