Development of a Radiographic Scoring System to Estimate Acetabular Protrusion Risk in Patients with Osteolytic Periacetabular Metastases.

BACKGROUND: For patients with periacetabular metastases, protrusio acetabuli is a severely painful and mobility-impairing complication that requires subsequent open joint surgery. We aimed to identify specific structural changes that are associated with progression to protrusio acetabuli and to create a scoring system to guide risk stratification. METHODS: In this single-institution cohort study, we identified all patients who underwent primary surgical stabilization for periacetabular metastases with osteolytic or mixed osteolytic-osteoblastic characteristics from October 2017 through Januar

Protrusio Acetabuli in Osteogenesis Imperfecta: A Two-Center Study on Incidence and Associated Factors.

Osteogenesis imperfecta (OI) is a rare genetic connective tissue disorder characterized by increased fracture risk and bone deformity. Protrusio acetabuli (PA) is a well-recognized radiographic finding in patients with OI, yet data on its incidence, severity, and associated factors across different OI phenotypes remain limited. This retrospective study evaluated pelvic radiographs and clinical data of patients with OI treated in two centers. PA was assessed using established radiographic criteria and graded according to the Sotelo-Garza/Charnley classification. Demographic and clinical variabl

Characterization of hip morphology phenotypes in individuals with femoroacetabular impingement syndrome with cam morphology: Data from the PhysioFIRST trial.

OBJECTIVE: To characterize different hip morphology phenotypes in individuals with femoroacetabular impingement (FAI) syndrome with cam morphology. MATERIALS AND METHODS: Supine pelvic anterior-posterior radiographs from 119 hips from 119 individuals diagnosed with FAI syndrome with cam morphology from a RCT were analysed. The lateral center edge angle (LCEA), Wiberg center edge angle (W-CEA), global retroversion, protrusio acetabuli, and femoral neck shaft angle (FNSA) measures were assessed independently by two raters. Criteria from the Lisbon Agreement on FAI imaging were used to character

Isolated Posterior Malleolus Fracture with Early Symptom Resolution: A Case Report.

INTRODUCTION: Giant cell tumor of bone (GCTB) is a benign but locally aggressive neoplasm that predominantly affects the epiphyses of long bones. Pelvic involvement is uncommon, and acetabular GCTB presents particular diagnostic and therapeutic challenges because of the complex pelvic anatomy, proximity to neurovascular structures, and critical weight-bearing function of the hip. CASE REPORT: We report the case of a 50-year-old female who presented with progressive left hip pain, restricted movements, and inability to bear weight following a trivial fall. Radiographs and cross-sectional imagi

Case Report on Steroid-induced Bilateral Femoral Head Osteonecrosis with Concomitant Hip Septic Arthritis in Primary Membranous Nephropathy.

INTRODUCTION: Concomitant femoral head osteonecrosis and hip joint septic arthritis is rare. Membranous nephropathy is an immune-mediated glomerular disease producing nephrotic syndrome in adults. Patients take glucocorticoids or immunosuppressants during treatment of this condition. We report the first case of a patient with nephrotic syndrome during treatment developing concomitant hip septic arthritis and femoral head osteonecrosis with an opportunistic pathogen seen in gastrointestinal tract. CASE REPORT: A 37-year-old gentleman on treatment with high-dose prednisolone and mycophenolate f

Revision total hip arthroplasty using ETO for failed bipolar hemiarthroplasty with protrusio acetabuli.

A woman in her 40s presented with persistent left hip pain and restricted motion for 5 years. X-ray findings revealed an uncemented bipolar stem, tension band wiring of greater trochanter with K wires, a broken Shenton line and protrusion acetabuli. The patient previously underwent unsuccessful cancellous screws fixation for an intracapsular neck of femur fracture, leading to non-union and subsequent bipolar hemiarthroplasty outside our facility. The planned extended trochanteric osteotomy (ETO) aimed to remove the stem, assess the acetabular defect and fill it with allograft. The use of a dis

The natural history of protrusio acetabuli in Marfan syndrome and other hereditary connective tissue disorders: a 10-year follow-up CT study.

OBJECTIVES: To explore the natural history of protrusio acetabuli (PA) in adults with Marfan syndrome (MFS) via a prospective 10-year follow-up study. METHODS: 2014 through 2015, 62 of 87 survivors from a nationwide cross-sectional study of 105 adults with presumed MFS were re-examined. At follow-up, MFS was diagnosed in 46 participants, and other hereditary connective tissue disorders in 12 participants. As in the baseline study, matched hospital controls were collected for comparison. CT images were obtained of the hips. PA was evaluated quantitatively and qualitatively. Measurements were p

Total Femoral Head Collapse in a Corticosteroid-Induced Pathologic Fracture.

Glucocorticoid therapy is a well-established iatrogenic cause of secondary osteoporosis. This form of osteoporosis is known as glucocorticoid-induced osteoporosis (GIOP). We present the case of a 78-year-old woman who developed avascular necrosis and complete collapse of the femoral head after a high-dose steroid taper for asthma. Imaging showed subsequent femoral head collapse with the presence of protrusio acetabuli. The patient underwent a posterior total hip arthroplasty. This case is unique because of the extremely high dose of steroids and the timing of her fracture. The case reveals the

Robotic-Assisted Triple Reaming Technique for Total Hip Arthroplasty in Protrusio Acetabuli.

We describe a robotic-assisted triple reaming technique during total hip arthroplasty in protrusio acetabuli. This technique uses computerized tomography-based 3-dimensional preoperative planning and intraoperative dynamic referencing to prepare the acetabulum. Two separate surgical plans are created using the robotic workflow. Using the first plan, the protrusio defect is prepared utilizing a reamer that matches the size and location of the defect. Using the second plan, the true acetabulum is reamed, using a reamer that restores the hip center and provides an adequate rim fit. Thirdly, the i

Perioperative Considerations for Hip Arthroplasty in Patients with Rheumatoid Arthritis.

Due to its distinct features, rheumatoid arthritis (RA), an inflammatory autoimmune disorder, poses challenges in planning for surgical interventions. This review includes available evidence regarding perioperative considerations in management of RA patients, with a focus on hip surgery. RA can affect multiple joints, with development of extra-articular manifestations; therefore, preoperatively, comprehensive medical assessments, including cardiovascular or pulmonary evaluation must be considered in addition to surgical considerations. Modification of medications capable of controlling RA-rela
