Ninety-three total hip arthroplasties were examined radiographically and clinically to investigate the best method of correction in protrusio acetabuli. Fifty-three percent were treated with cement alone, 36% with mesh or protrusio shell, and 11% with bone graft. Long-term follow-up study revealed a high percentage of immediate and late radiolucent lines. The highest rate of loosening, 50%, occurred in the cement alone group, which had not been corrected to within 10 mm of anatomic position. Two years after pelvic irradiation for prostatic cancer, bilateral protrusio acetabuli and collapse of the right femoral head requiring prosthetic arthroplasty developed in a 73-year-old man with chronic rheumatoid arthritis. There was no evidence of metastases. Histologic evaluation showed empty lacunae in the bone but no evidence of obliterative endarteritis. Osteonecrosis and pathologic fractures constitute a rare complication of high voltage irradiation.

Total hip replacement has been very successful in patients with painful, stiff hips associated with protrusio acetabuli, but the heat of polymerisation of methylmethacrylate cement may cause necrosis of the thin medial wall with consequent danger of migration of the cup. Two cases of acute chondrolysis of the upper femoral epiphysis associated with protrusio acetabuli are reported in two girls respectively 13 and 12 Y.O. The disease was unilateral in one patient and bilateral in other. Acute chondrolysis is characterized by onset of pain, restricted movements of the hip and evolution to an hip ankylosis within a few months. As usual no evidence of inflammatory disease could be shown at biology or at pathology of synovial membrane or of femoral head. CT and MR studies proved to be contributive in the first case. The results of using the Ring uncemented polyethylene-to-metal (UPM) prosthesis in patients with protrusio acetabuli or dysplasia are reported. Total hip arthroplasties using the Ring uncemented polyethylene to metal system were carried out in 84 hips with either protrusio or dysplasia of the acetabulum. The results of the operation, the stability and function of the acetabular implant, and radiologic changes in the medial wall of the acetabulum in the protrusio hips were assessed over a two- to five-year follow-up period.
