We report on 207 cases of elderly patients suffering from femoral neck fractures treated with cementless Moore-hip prosthesis in the years 1967 to 1979. Low complication rate and good functional results were also confirmed in 29 follow-up examinations 3 years or more after arthroplastic. Loosening of prosthesis, breaking of material or protrusio acetabuli could not be found. The method is easy to be learned and well tolerated by the aged patients. In regard of the contraindications it leads to good results.

Of 155 adults with sickle-cell anemia (SS, SC), radiographs of the pelvis or hip demonstrated protrusio acetabuli on at least one side in 14 (3 men and 11 women), as indicated by projection of the acetabular line medial to the ilio-ischial line by greater than or equal to 3 mm in men and greater than or equal to 6 mm in women. We reviewed our experience with total hip replacement for coxarthrosis due to Paget's disease. The clinical and radiographic results in twenty-one patients who had a total hip replacement between October 1972 and February 1982 were analyzed. The ages of the patients averaged 68.6 years (range, fifty-seven to eighty years) and there were twelve women and nine men. Marfan's syndrome is a hereditary disorder of connective tissue, involving the ophthalmic, cardiovascular and musculoskeletal systems. Seven hips with protrusio acetabuli which showed complete or partial resolution of the protrusion after replacement with Ring's prosthesis are reported. It is suggested that this prosthesis offers a simple and effective method of treating painful protrusion into the pelvis; it allows healing of the medial wall of the acetabulum while avoiding many of the hazards of other methods of treatment.

Eight children, aged 9 to 15, referred to a juvenile rheumatology service on account of undiagnosed hip pain, were ultimately found to have idiopathic protrusio acetabuli (IPA). Clinical and radiographic findings were generally asymmetrical on presentation. Thirty-five total hip arthroplasties done in twenty-five patients with protrusio acetabuli secondary to rheumatoid arthritis were reviewed. There was an average follow-up of 4.3 years, with a range of three to seven years. The results were rated as excellent or good in 66 per cent, fair in 26 per cent, and poor in 8 per cent. Since 1971 we have used homologous and autogenous bone grafts to reinforce the medial acetabular wall when doing a total hip replacement in patients with painful protrusio acetabuli. Thirty-two patients have been followed for a minimum of two years, the longest follow-up being eight years. In an unselected series of 14 patients with Marfan's syndrome, protrusio acetabuli was documented by both presence of a center edge (CE) angle of 40 degrees or more and crossing of the teardrop by the femoral head. The disorder was unilateral in 8 patients, bilateral in 6. The internal shell endoprothesis for the hip joint after Wagner has considerably limited the indications for the implantation of ceramic endoprotheses to patients under 60 years, because less bone substance has to be sacrificed.
