OBJECTIVE: A decreased range of motion (ROM) of the hip joint is known to predispose to athletic groin injury. Femoroacetabular impingement (FAI) of the hip leads to a reduced ROM. This study examined the prevalence of radiological signs of FAI in patients presenting with long-standing adductor-related groin pain (LSARGP).
DESIGN: Prospective case series.
SETTING: Outpatient Sports Medicine Department.
PATIENTS: 34 athletes with LSARGP defined as pain on palpation of the proximal insertion of adductor muscle and a painful, resisted adduction test.

Protrusio is a central acetabular defect resulting from migration of the femoral head medial to Kohler's line. Protrusio can develop in association with a number of conditions that affect the metabolism or mechanical behavior of the periacetabular bone. OBJECTIVE: To report on a 30-year prospective study of deep infection in 1993 consecutive total hip arthroplasties performed by a single surgeon.
METHODS: The relations of numerous variables to the incidence of deep infection were studied.

This retrospective study examined the results of 29 total hip arthroplasties on patients with protrusio acetabuli, implanted with a dual-geometry shell with a mean follow-up of 48 months (range, 24-110). All surgeries were performed using a conical reamer in addition to a spherical reamer with morsellized autogenous graft packed into the medial acetabular protrusio defect. Between 1990 and 2005, 25 acetabular reconstructions in 25 patients were performed using autogenous morcellized bone-graft and acetabular wiremesh due to acetabular bone stock loss. A 93-year-old man presented with a one-month history of persistent left hip pain of sudden onset. At first visit, radiographs revealed a fracture line at the medial portion of the acetabulum with no displacement. This case report presents the case of a patient with a history of rheumatoid arthritis and protrusio acetabuli progressing to a medical wall acetabular fracture. A detailed history and physical examination revealed previous radiation treatment of prostate cancer requiring extensive preoperative workup and consultation. Prompt recognition and elective surgical intervention of protrusio acetabuli, prior to fracture, may obviate the need for prolonged hospitalization and delayed surgical intervention. The preoperative evaluation and surgical technique used to optimize the outcome are described.

Complex primary total hip arthroplasty (THA) is defined as primary THA in patients with compromised bony or soft-tissue states, including but not limited to dysplastic hip, ankylosed hip, prior hip fracture, protrusio acetabuli, certain neuromuscular conditions, skeletal dysplasia, and previous bony procedures about the hip. Intraoperatively, provisions must be made for the possible use of modular implants and/or bone grafts. In this article, we review the principles of preoperative, intraoperative, and postoperative management of patients requiring a complex primary THA.

Thirty primary total hip arthroplasties were performed for protrusio acetabuli in 23 patients (mean age, 46 years) using impacted autologous bone grafting and a cementless porous-coated hemispheric cup without the use of metal cages or rings.
