Gut-Liver Axis Derangement in Non-Alcoholic Fatty Liver Disease.

Non-alcoholic fatty liver disease (NAFLD) is the most frequent type of chronic liver disease in the pediatric age group, paralleling an obesity pandemic. A "multiple-hit" hypothesis has been invoked to explain its pathogenesis. The "first hit" is liver lipid accumulation in obese children with insulin resistance. In the absence of significant lifestyle modifications leading to weight loss and increased physical activity, other factors may act as "second hits" implicated in liver damage progression leading to more severe forms of inflammation and hepatic fibrosis. In this regard, the gut-liver

Retrograde intubation through nasal route in patients with limited mouth opening undergoing oral and maxillofacial surgery.

BACKGROUND: Patients with limited mouth opening (LMO) often associated with difficult intubation. Retrograde intubation is an alternative technique of establishing definitive airway in these patients when blind nasal intubation fails and fiberoptic bronchoscope is not available. We tested the retrograde intubation through nasal route in patients with LMO less than 2 cm. MATERIALS AND METHODS: The procedure was performed with some modification with regard to retrograde guide on 18 patients requiring maxillofacial surgical procedures to increase mouth opening. Indications for this technique wer

ADAM10 localization in temporomandibular joint disk with internal derangement: an ex vivo immunohistochemical study.

The purpose of this study was to determine the presence of ADAM10 in temporomandibular joint disk with internal derangement. Twenty-five paraffin blocks of displaced temporomandibular joint (TMJ) disk specimens from earlier investigations were retrieved from the archives of the University of Catania. Of these 16 had been removed from females and 9 from males; 11 with anterior disk displacement with reduction (ADDwR) and 14 with anterior disk displacement without reduction (ADDwoR). The sections were dehydrated, embedded in paraffin and cut. Then they were incubated in 0.3% H2O2/methanol and ha

Assessment of Knee Joint Injuries with Low Field Strength Magnetic Resonance Imaging.

BACKGROUND: Magnetic Resonance Imaging is an appropriate screening tool before therapeutic arthroscopy, making diagnostic arthroscopy unnecessary in most patients. This study aims to evaluate the MRI findings in knee injuries and diagnostic value of low Strength MRI for assessing Meniscal and cruciate ligament tear. METHODS: A cross sectional study was conducted on patients undergoing "Magnetic Resonance Imaging of the Knee" for injuries of the knee and excluded patients undergoing MRI for other causes, poor diagnostic quality MRI and post operative MRI. All patients were interviewed for mech

An unusual cause of chest discomfort: case report of oesophageal hirudiniasis.

Osteoarthritis (OA) is a chronic, age‑related osteoarthropathy that causes a considerable decline in quality of life, as well as economic losses due to its high incidence and poor prognosis. Mitogen‑activated protein kinases (MAPKs) regulate multiple cellular processes, including proliferation, differentiation and apoptosis, in certain diseases, such as cancer, diabetes and Alzheimer's disease. The present study aimed to investigate the regulatory role of the MAPK signaling pathway in early‑stage OA. A rabbit model of early‑stage OA was induced by treatment with the enzyme papain. New agents for the treatment of lymphoid leukemia and lymphoma: focus on recent FDA approvals.

Leukemia and lymphoma are systemic malignancies that represent half of all childhood cancers, though 90% occur in adults. Various treatment options are available, but therapy is mainly systemic chemotherapy plus appropriate monoclonal antibodies. In certain situations radiotherapy and bone marrow transplantation play a role. Some types/subtypes of these diseases are potentially curable, yet many leukemias and lymphomas do not properly respond to current therapies. Although the FDA (US Food and Drugs Administration) approvals of new drugs have shown a small increasing trend between 2007-2012, o

Clinical orthopedic examination findings in the upper extremity: correlation with imaging studies and diagnostic efficacy.

Different orthopedic tests are used to evaluate internal derangements of joints. Radiologic examinations like magnetic resonance (MR) imaging are ordered on the basis of results of these tests to narrow the clinical diagnosis and formulate a treatment plan. Although these tests are clinically useful, the test terminology can be confusing and the significance of the tests not clearly understood. This article helps explain the clinical jargon of tests performed for the major joints of the upper extremity and their proper use and diagnostic value in conjunction with MR imaging. The article presen

Patellotibial contusions: a rare cruciate ligament injury pattern.

OBJECTIVE: Patellotibial contusions are a recently described contusion pattern in anterior cruciate ligament (ACL) tears. The purposes or our study were to determine if patellotibial contusions are specific to ACL injuries, determine the internal derangements in patients demonstrating this contusion, and suggest a mechanism of injury. MATERIALS AND METHODS: We conducted a retrospective search of radiology reports over a 6-year period to identify patients with patellar edema described on magnetic resonance (MR) imaging. Only individuals with patellar and tibial edema were included in the study

Patellotibial contusions in anterior cruciate ligament tears.

Bone contusions are an important ancillary finding of many knee injuries. Not only are they a source of pain, they may suggest a mechanism of injury or a specific derangement of the knee joint. We have encountered a small number of patients being evaluated for anterior cruciate ligament (ACL) tears with unexplained patellar and tibial edema at magnetic resonance (MR) imaging. We present three individuals with contusions of the inferior patella with a corresponding contusion of the anteromedial tibial plateau. Internal derangements in these patients were similar to other individuals with acute

Current innovations in wrist arthroscopy.

It has become clear that the stability of the scapholunate joint does not depend wholly on the scapholunate interosseous ligament, but rather on both primary and secondary stabilizers, which form a scapholunate ligament complex. Each case of scapholunate instability is unique and should be treated with tissue-specific repairs, which may partly explain why one procedure cannot successfully restore joint stability in every case. Wrist arthroscopy has a pivotal role in both the assessment and treatment of the scapholunate ligament complex derangements. Tears of the foveal attachment of the triang

Use of artificial neural network in differentiation of subgroups of temporomandibular internal derangements: a preliminary study.

PURPOSE: Artificial neural networks (ANNs) have been developed in the past few decades for many different applications in medical science and in biomedical research. The use of neural networks in oral and maxillofacial surgery is limited. The aim of this study was to determine the use of ANNs for the prediction of 2 subgroups of temporomandibular joint (TMJ) internal derangements (IDs) and normal joints using characteristic clinical signs and symptoms of the diseases. MATERIALS AND METHODS: Clinical symptoms and diagnoses of 161 patients with TMJ ID were considered the gold standard and were

Diabetic polyneuropathies: update on research definition, diagnostic criteria and estimation of severity.

Prior to a joint meeting of the Neurodiab Association and International Symposium on Diabetic Neuropathy held in Toronto, Ontario, Canada, 13-18 October 2009, Solomon Tesfaye, Sheffield, UK, convened a panel of neuromuscular experts to provide an update on polyneuropathies associated with diabetes (Toronto Consensus Panels on DPNs, 2009). Herein, we provide definitions of typical and atypical diabetic polyneuropathies (DPNs), diagnostic criteria, and approaches to diagnose sensorimotor polyneuropathy as well as to estimate severity. Diabetic sensorimotor polyneuropathy (DSPN), or typical DPN,

Evaluation of the Research Diagnostic Criteria for Temporomandibular Disorders for the recognition of an anterior disc displacement with reduction.

The aim of this Focus Article is to review critically the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) for the recognition of an anterior disc displacement with reduction (ADDR) in the temporomandibular joint (TMJ). This evaluation is based upon the experience gained through the careful analysis of mandibular movement recordings of hundreds of patients and controls with or without an ADDR. Clinically, it is a challenge to discriminate between the two most prevalent internal derangements of the TMJ: ADDR and symptomatic hypermobility. It is due to the very nature of th
