The results of total knee replacement in five patients aged between 22 and 37 with severe haemophilia A or B are described. All patients had been managed conservatively without success. Frequent bleeds, severe pain and limitation of movement were the indications for operation. Several types of marginal fracture of the patella are described together with the methods of treatment. The relationship between bipartite patellas and marginal fractures is also discussed. An experimental model of haemophilic arthropathy was developed in 12 growing dogs by anastomosing the popliteal artery to the joint cavity of the knee to produce haemarthrosis under considerable pressure. Twenty joints in 19 haemophiliacs were treated by radioactive synoviorthesis with gold (Au 198) to prevent recurrent haemarthrosis. Twelve knees, six elbows and two ankles were treated in two separate groups (29. 9. 76 and 9. 5. 77). In eight cases (40%) no further haemarthrosis occurred.
