Increased tibiofemoral rotation is associated with anatomical risk factors for lateral patella instability.

PURPOSE: Chronic patellofemoral instability (PFI) and pain are influenced by axial malalignment of the patellofemoral joint. Tibiofemoral rotation, defined as the rotation between femur and tibia knee, has shown to be correlated to PFI. This study aimed to determine whether tibiofemoral rotation is associated with anatomical risk factors for patellofemoral maltracking in patients with PFI. It was hypothesised that greater tibiofemoral rotation correlates with other predisposing factors for PFI. METHODS: Eighty-five consecutive patients (mean age 22.6 ± 8.9 years; 58 female) with PFI underwent

Progress in the study of irrigation strategies for percutaneous nephrolithotomy: a narrative review of the current literature.

Percutaneous nephrolithotomy (PCNL) is the gold-standard treatment for large renal and upper ureteral stones. Continuous irrigation is essential for visualization, thermal control, and fragment clearance but carries risks such as hypothermia, electrolyte imbalance, elevated intrarenal pressure, and infection. This review summarizes current evidence on irrigation strategies during PCNL and provides recommendations to mitigate these risks. A systematic literature search was conducted for English-language full-text articles published up to January 2025, using terms related to PCNL and irrigation.

Enzymatic hydrolysis of conjugated bile acids during benchtop processing: a preanalytical pitfall in quantitative bioanalysis.

Bile acids (BAs) are endogenous signaling molecules with diverse biological functions. In cholestasis, conjugated BAs are markedly elevated, making their ratio to unconjugated BAs a critical diagnostic biomarker. Accurate quantification of BAs is pivotal for assessing cholestatic disease progression. Conjugated BAs contain a fragile amide bond that is susceptible to hydrolysis in biological samples, resulting in an artificially low ratio of conjugated-to-unconjugated BAs. Here, we systematically investigated the benchtop stability of BAs in murine biological samples using liquid chromatography

Prevalence of dysmorphic sacral features in the general population.

BACKGROUND: Posterior pelvic ring injuries, including sacral fractures and sacroiliac (SI) joint disruption, can have potentially serious sequelae and are becoming increasingly common. Management, especially in the setting of instability, consists of internal fixation to decrease the risk of complications. Placement of SI screws requires a thorough knowledge of normal pelvic anatomy and the available osseous fixation pathways. The aim of this study was to define the prevalence of individual features of sacral dysmorphism in the general population. METHODS: We conducted a retrospective review

Does Anterior Cruciate Ligament Reconstruction with Lateral Extra-Articular Tenodesis Improve Objective Stability and Functional Outcomes in Athletes with Isolated Anterior Cruciate Ligament Tear? A Randomized Controlled Trial.

Numerous studies have compared anterior cruciate ligament reconstruction (ACLR) with and without lateral extra-articular tenodesis (LEAT) in patients with anterior cruciate ligament (ACL) tears and other associated pathologies. These associated conditions significantly affect the outcomes in terms of function and stability. Athletes with isolated ACL tears and high pivot shifts represent a unique and uncommon subgroup. Despite the presence of a high-grade pivot shift, these individuals do not exhibit associated meniscal, chondral, or ligamentous laxity. We have carefully selected this group of

Comparative study between lateral extra-articular tenodesis versus anterolateral ligament reconstruction in combination with anterior cruciate ligament reconstruction.

BACKGROUND: Residual laxity after anterior cruciate ligament reconstruction (ACL-R) is found in some cases. Nowadays, anterolateral augmentation procedures are performed to prevent this laxity. MATERIALS AND METHODS: A prospective randomized comparative clinical study took place from January 2021 to November 2022. The study included 48 patients after exclusion of losses to follow-up, who acquired ACL-R combined with either lateral extraarticular tenodesis (LET) in 25 or anterolateral reconstruction (ALL) in 23 patients. The patients were evaluated with the Tegner-Lysholm score, and follow-up

Improved Perioperative Outcomes in Robotic-Assisted Revision Total Knee Arthroplasty.

BACKGROUND: This study compares short-term outcomes of robotic-assisted revision total knee arthroplasty (RA-rTKA) to conventional rTKA. We hypothesize that RA-rTKA will accelerate gains in range of motion and time to ambulation postoperatively. METHODS: This is a retrospective case-control study reviewing consecutive rTKA performed by a single surgeon between 2017 and 2024. rTKA cases performed prior to 2022 were compared to RA-rTKA performed from 2022 through present day. Revisions for periprosthetic joint infection or fracture were excluded. The primary outcome was hospital length of stay.

Variety is Not the Spice of Life: Inconsistent Definitions of Failure After Hip Reconstruction in Cerebral Palsy.

BACKGROUND: Hip displacement (subluxation or dislocation) is common in children with cerebral palsy (CP), especially those who are nonambulatory. If unaddressed, this can cause pain, stiffness, hygiene difficulties, and seating imbalance. Progressive hip subluxation or dislocation often requires surgery, and further displacement after reconstruction may be described as "failure." There is currently no standardized definition of failure after the index procedure. This review aimed to identify published failure definitions and compile reported failure rates after index bony hip reconstruction.

Isolated intermediate cuneiform dislocation: A case report on the surgical considerations for a rare injury of the foot.

In this report, a rare case of an isolated dorsal dislocation of the intermediate cuneiform in a 32-year-old woman following a low-energy injury is presented. The patient sustained a direct medial-to-lateral midfoot crush injury. Imaging revealed a dorsal dislocation of the intermediate cuneiform with associated plantar avulsion fractures, indicating ligamentous instability. Closed reduction under general anesthesia was unsuccessful due to soft tissue interpositioning. Open reduction and internal fixation was performed using two lag screws, achieving a stable anatomical alignment. The patient

Remodelling and the fate of bone grafts in shoulder instability surgery.

UNLABELLED: Anterior glenoid bone loss is a critical factor in the surgical management of recurrent shoulder instability. In cases where bone loss exceeds the threshold for soft-tissue repair, bony augmentation is necessary to restore joint stability and prevent recurrence. However, there remains considerable debate regarding the optimal surgical technique and graft selection to maximise clinical outcomes and ensure long-term graft survival. The Latarjet procedure is widely performed and associated with high success rates. Nevertheless, it is not without limitations, particularly the potential

Comparison of kinetic adaptations in gait initiation following exergaming and balance training in athletes with chronic ankle instability.

Gait initiation (GI) requires coordinated postural control and is often impaired in athletes with chronic ankle instability (CAI). Exergaming has been proposed as an innovative rehabilitation strategy, but its effects on GI kinetics compared with conventional balance training remain unclear. Thirty-four athletes with CAI were assigned to either an exergaming intervention or a balance training control group. Both groups underwent a 12-session intervention, with sessions held three times per week and each session lasting 60 minutes. Kinetic measures, including center of pressure (COP) displaceme

Predicted effects of hip distraction on knee ligament mechanics following total knee arthroplasty: A finite element analysis.

BACKGROUND: Hip distraction is essential for hip arthroscopy and other orthopaedic surgeries, but high traction forces often lead to complications like nerve injury and soft tissue impairment. Patients with total knee arthroplasties (TKAs) may face increased risk due to altered joint mechanics; however, the impact of specific TKA configurations on knee behavior during distraction is poorly understood. METHODS: A validated finite element model of the pelvis and lower extremity was developed from patient CT and MRI data. Three configurations were modeled: bi-cruciate retaining (BCR), posterior-

Evaluation of femoro-epiphyseal acetabular roof index for assessing hip instability in hip disorders in female patients.

INTRODUCTION: The Femoro-Epiphyseal Acetabular Roof (FEAR) index is a proposed radiographic marker of hip instability, particularly in borderline developmental dysplasia of the hip (BDDH). Its diagnostic value in other hip morphologies and its association with patient-reported outcomes (PROMs) remain unclear. This study evaluated the FEAR index for assessing hip instability across different acetabular morphologies by analyzing its associations with radiographic measures obtained from standard anteroposterior pelvic and false-profile radiographs, dynamic ultrasonography, and PROMs. MATERIALS AN

Tibiofemoral rotation is significantly higher in patients with patellofemoral maltracking and torsional deformity.

INTRODUCTION: Tibiofemoral (TF) rotation, the relative rotational alignment between femur and tibia in the axial plane at the level of the knee has been investigated as a parameter of joint alignment. Its influence on patellofemoral pathologies is largely unknown. Within this cross sectional study, it was hypothesized that TF rotation is increased in patients with symptomatic torsional femoral or tibial deformity and associated patellofemoral maltracking compared to healthy individuals. METHODS: This single-center-study included patients with patellofemoral maltracking, including patellofemora
