The arthroscope in the diagnosis of disorders of the patellofemoral joint.

Despite our current knowledge and accumulated information, orthopedic surgeons often experience difficulties in the diagnosis of internal derangement of the knee. Too often, the admission diagnosis I.D.K. means "I don't know." Disorders of the patellofemoral joint contribute to this difficulty and may simulate torn menisci and loose bodies, as well as the instability which results from rotary displacement. Patients often have difficulty describing and locating their symptoms, and 2 or more pathologic lesions may be present in the same joint. Early lesions involving the articular cartilage are

[Isolated rupture of the anterior cruciate ligament. Clinical and experimental study (author's transl)].

The isolated rupture of the anterior cruciate ligament is possible. 5 own cases are described and compared with those of other authors. The mechanism of the injury is a forced inward rotation- and varus-stress of the knee joint under full pressure as a deceleration-twisting motion. Our own experimental measuremens on the ligaments of hte knee joint with strain gauges are proving this fact. The suspeced diagnosis of an isolated rupture of the anterior cruciate ligament is strengthened by a painful snapping or popping within the knee joint in the moment of the injury, a fast increasing swelling

A new type of fracture-dislocation of the thoracolumbar spine.

Ten instances of fracture-dislocation of the thoracolumbar spine are described in which the lower vertebra was displaced forward under the upper one by forward slipping of the posterior articular processes. The mechanism involved is a posterior lumbosacral shearing force. All patients had neurological deficits of different degrees of severity and all but three patients recovered. Surgical treatment was indicated because of instability in four patients and was done in three of the four. Surgical reduction was always difficult. A stable and painless spine was achieved in all patients surveyed, r

The yale cervical orthosis: an evaluation of its effectiveness in restricting cervical motion in normal subjects and a comparison with other cervical orthoses.

The Yale cervical orthosis is a lightweight polyethylene foam Philadelphia collar with molded fiberglass extensions over the thorax. This orthosis was studied on 17 normal subjects in the extremes of the ranges of flexion, extension, rotation, and lateral bending using roentgenograms and axial photographs to assess how effectively it limited motion of the neck. Overall, it satisfactorily controlled cervical motion and was similar to the most effective rigid cervical orthoses. Flexion and extension ranges were compared at different segmental levels of the spine. The Yale orthosis was most succe

Proximal femoral focal deficiency.

Proximal femoral focal deficiency, a distinct clinical and radiographic entity that results in leg length discrepancy, has received little attention in the radiographic literature. Thirteen patients were studied and the degree of deficiency established using radiographic criteria. The full extent of hip instability often could not be ascertained on radiographs obtained during the first year of life. Hip arthrograms combined with cine radiography can establish the presence of an unossified femoral head and aid in evaluating the degree of hip instability. Unlike other causes of leg length discre

Anterolateral rotatory instability of the knee joint. An early analysis of the Ellison procedure.

Our experience with the Ellison procedure for the control of anterior subluxation of the lateral tibial plateau includes fifty-two knees in which we used this procedure either alone or in combination with other reconstructive procedures. The results directly attributable to the Ellison procedure itself after follow-ups of six months or more could be evaluated in fifteen patients who had only the Ellison procedure for anterolateral instability and in thirteen who had both this procedure and a pes anserinus transfer for combined anterolateral and anteromedial instability. Of the twenty-eight pat

Atlanto-axial arthrodesis by the wedge compression method.

Fifteen patients with atlanto-axial instability (secondary to os odontoideum in three, nonunion of an odontoid fracture in seven, acute odontoid fracture in three, and rheumatoid arthirtis in two) were treated by wedge compression arthrodesis of the atlanto-axial joint. One patient died at home eight weeks after fusion with the cause of death never established. Of the two patients with rheumatoid arthritis (ankylosing spondylitis), one had a non-union and in the other the posterior arch of the atlas fractured and the fusion had to be extended up to the occiput and down to the third cervical ve

Pes anserinus transposition for chronic anteromedial rotational instability of the knee.

Forty-eight men and three women were reviewed an average of thirty-one months after pes anserinus transposition for chronic anteromedial rotational instability of the knee. Their ages ranged from eighteen to forty-two years (average twenty-five years). All but four of the injuries occurred during sport. The interval between injury and operation averaged thirty months. During this time twenty-three patients had had other operations of which eighteen were for excision of menisci. A further sixteen patients required excision of one or both menisci at the time of pes anserinus transposition. After

The patellar compression syndrome: surgical treatment by lateral retinacular release.

Patellar compression syndrome may be a manifestation of contraction of the lateral retinacular structures which consists largely of the lateral patellofemoral ligament producing a tethering action on the patella. This may produce parapatellar pain, particularly in activities requiring repetitive knee flexion. Minor patellofemoral incongruencies with pain, grating, and functional limitation may be found on examination but the patient does not demonstrate the problems of instability of marked congenital abnormalities often seen with other patellar problems. Forty patients underwent a simple rele

Post-traumatic ligamentous instabilities of the wrist.

Dorsiflexion, palmar flexion, ulnar translocation, and dorsal carpal subluxation are four, not uncommon, post-traumatic carpal ligamentous instabilities. Palmar carpal subluxation is a theoretical possibility. Dorsiflexion and palmar flexion instabilities are named for the way the distal lunate articular surface dorsally and palmarly, respectively. Ulnar translocation, and dorsal and ventral carpal subluxations are named according to movement of the entire carpus at the radiocarpal joint. Early detection of traumatic carpal instabilities is necessary to encourage optimal treatment of these sig

Proceedings of the Annual Scientific Meeting of the International Medical Society of Paraplegia held at Stoke Mandeville from 28-30 July 1977 (Part II). Fracture dislocation of the cervical spine: a critique of current management in the United States.

The author has been asked to review the hospital records and X-rays of 12 patients from various regions of the United States who have sustained fracture dislocations of the cervical spine. In no instance was the initial care considered to be appropriate. No patient was significantly improved by treatment although only 17% had apparent complete transverse cord syndromes on admission. Sixty-seven per cent of the patients became worse. In the latter group there were three patients who were admitted with either no neurological deficit or only minimal pyramidal signs. All of these three patients be

Analysis of 100 patients with anterolateral rotatory instability of the knee.

Of 100 consecutive patients with anterolateral rotatory instability analyzed, 84 were males and the average age was 27.4 years. Over 50 per cent were injured while participating in athletics and 40 per cent were injured during daily activities. The group averaged 1.2 operations prior to diagnosis. The most frequently performed procedures were medial meniscectomy, pes anserinus transfer and medial capsular reefings. Eighty-four of 100 patients underwent surgery during the study period. Seventy-one had combined rotatory instability and 13 had anterior lateral rotatory instability alone. Sixty-si

[Anatomic principles as a prerequisite for the diagnosis of knee-ligament injuries in skiing].

The anatomy of the capsulo-ligamentous structures is most important because of its immense significance for the physiology of the knee joint and its high danger to lesions. On the other hand there exists new knowledge of anatomy which leads to a revision of the importance of the clinical findings. It must be emphasized that the capsule is as essential for the passive stability of the joint as the 4 known main ligaments. Therefore, the instability of varus and valgus must be tested in full extension and in slight flexion of 20 to 30 degrees. To exclude rotatory instability the anterior drawer t

Replacement of the severely damaged arthritic knee by the ICLH (Freeman-Swanson) arthroplasty.

A multi-centre clinical trial of ICLH (Freeman-Swanson) arthroplasty has been in progress since 1971. In this paper the results up to two years after operation are reported in seventy-one knees displaying at least 30 degrees of fixed flexion, 25 degrees of valgus or 20 degrees of varus, before operation. It has been found that knees displaying 70 degrees of fixed flexion, 70 degrees of valgus, 30 degrees of varus or 50 degrees of valgus/varus instability can be satisfactorily aligned and stabilised with acceptable function. Three knees required revision. The other complications are listed and

Arthroplasty of the knee in osteoarthritis and rheumatoid arthritis.

147 cases of arthroplasty of the knee were reviewed. Three types of prostheses were used, viz., St. Georg, Geomedic and hinge (Shier and Guépar). Pain was the most common indication for the operation which produced complete or almost complete relief in 90% of the cases. In half of the joints with a mobility of less than 80 degrees preoperatively, the range of movement was increased to more than 80 degrees by the operation. Lack of extension, deformity and instability could be largely corrected. Two cases of deep infection were observed, both in patients treated with a hinge prosthesis. In one

[Chronic fibular instability of the ankle-joint -- x-ray diagnostic and ligamentous graft (author's transl)].

It was tried to find out how many radiographs are necessary to show instabilities of the ankle-joint with ruptures of the ligamentum talofibulare anterius alone (which is the most common ligamentous lesion) and in combination with other fibular ligaments. Therefore stress views of anatomical speciems of ankle-joints with previous artifically coused lesions were done. It was found that one lateral view with the talus stressed in subluxation and one a.p. view with stressed pes equinus and supination with slight internal rotation of the foot are enough in comparison with the other side. The management of thoracic and thoraco-lumbar injuries of the spine with neurological involvement.

The results of treatment of injuries of the thoracic and thoraco-lumbar spine with neurological involvement have been reviewed in a retrospective study of 115 patients, of whom eighty-nine received conservative and twenty-six surgical treatment. Operation was reserved, in general, for patients with irreducible dislocations and incomplete neurological lesions, open reduction and internal fixation being the commonest procedure. Only three patients required a delayed spinal fusion for suspected instability after a period of conservative treatment. On the other hand, ten patients, eight of whom ha

Fractures and dislocations of the cervical spine in children.

Correct diagnosis of fractures and dislocations of the cervical spine in children requires an awareness of the pseudosubluxation and other normal variants noted in x-ray films of patients in this age group. Fractures of the odontoid process occur frequently and almost always can be treated by closed reduction and external fixation until union occurs. Fractures of the lower cervical spine are difficult to detect roentgenographically, and x-ray changes can belie the severity of soft tissue injury and cord trauma. Instability tends to persist in adolescent patients after cervical spine injury bec

Reconstruction of the anterior cruciate ligament by semitendinosus tenodesis.

Rupture of the anterior cruciate ligament constitutes a major disability in young athletes and those patients with persistent disabling instability who have failed to respond to conservative treatment. A new method of reconstruction by means of semitendinsous tenodesis was used in five patients with satisfactory end results after an average follow-up of 21.4 months. Limited clinical experience indicates that with this method less anteroposterior instability persists than with other methods.

In a radiological study of the pubic symphysis in 37 athletes (26 footballers and 11 others) and 156 young men as controls changes similar to those of osteitis pubis were found in 19 (76%) of the footballers and nine of the other athletes (81%) and 70 of the controls (45%). In the controls there was a significant correlation between their athletic ability and these changes. The clinical features consist of pain in the region of the pubis which may radiate to the groin or lower abdomen. Clicking may be present and indicates instability. Local tenderness is the only significant sign. Radiographs
