In vivo biomechanical and morphological alterations in the Achilles tendon, ankle joint, and ankle plantar flexor muscles in individuals with diabetes mellitus without neuropathy.

BACKGROUND: Chronic hyperglycemia accelerates the accumulation of advanced glycation end-products and collagen cross-linking, which may impair collagen remodeling, alter tissue morphology, and increase muscle-tendon stiffness. These changes may restrict ankle mobility and elevate forefoot plantar loading, increasing susceptibility to skin breakdown and ulcers in individuals with diabetes. Because many muscle-tendon impairments in those with diabetes are attributed to neuropathy, whether these changes are present before clinically detectable neuropathy remains unclear. This study investigated b

From pelvic geometry to modal compliance: sex-specific deformation pathways in the human pelvic ring.

Walking, posture and childbirth all depend on the pelvic ring, yet human pelvic dimorphism is usually interpreted from static birth-canal dimensions. We asked whether sex differences in pelvic function are explained by overall size and static pelvimetry or by available mechanical deformation pathways. We analysed 278 anatomically standardized lumbopelvic finite-element models built from clinical computed tomography scans. In each model, modal decomposition separated pelvic deformation into a shared, ranked set of pathways, allowing us to compare pathway stiffness, load recruitment and sensitiv

Effectiveness of early initiation of idursulfase in infants and very young children with mucopolysaccharidosis II: a retrospective chart review in siblings.

INTRODUCTION: Enzyme replacement therapy (ERT) with intravenous idursulfase is the standard of care for patients with mucopolysaccharidosis II (MPS II; Hunter syndrome). Data are limited for children aged ≤12 months. This retrospective study evaluated outcomes with early ERT. METHODS: Male siblings with MPS II treated with idursulfase for >2 years were categorized into five cohorts by age at treatment initiation. The primary objective compared Cohort 1 (ERT initiation aged ≤12 months) with Cohort 2 (ERT initiation at an age ≥ 12 months older). Clinical outcomes were compared using Kaplan-Meie

Autologous protein solution and 3D-Printed technologies as potential adjuncts to arthroscopic elbow release: a narrative review and conceptual framework.

Post-traumatic and degenerative elbow stiffness is a major cause of upper-limb disability, as relatively small losses of elbow and forearm motion can significantly impair activities of daily living and work capacity. Arthroscopic elbow release has evolved as a joint-preserving alternative to open arthrolysis in selected cases. Multiple series have demonstrated meaningful gains in flexion including extension arc and forearm rotation, high patient satisfaction, and low complication rates. However, residual pain, recurrent stiffness, and progressive osteoarthritic change remain problematic in a s

Restoring the lost heat of the joint: Thermomechanical stimulation as a candidate treatment for early-stage osteoarthritis.

Two of the oldest osteoarthritis treatments, local heat and therapeutic exercise, have evolved along separate paths: heat as a symptomatic adjunct and exercise as a functional intervention. Neither intervention alone modifies the disease. This separation reflects clinical tradition, not joint physiology. Articular cartilage dissipates some of the energy absorbed during deformation. At rest, knee cartilage is 32-33 °C and warms toward 37 °C under loading, which enhances matrix production. Physical activity therefore provides two coupled cues: mechanical deformation and the resulting heat. These

[Age-related stiffness: can stretching improve function and autonomy in older adults?].

INTRODUCTION: Aging is associated with progressive changes in the musculoskeletal system, contributing to reduced joint mobility, flexibility, and motor function. These alterations may compromise autonomy in older adults, favoring musculoskeletal pain, functional limitations, and increased risk of falls. In this context, stretching is commonly included in adapted physical activity and rehabilitation programs for older individuals. OBJECTIVE: To critically analyze the available evidence regarding the usefulness of stretching in older adults, with particular focus on joint mobility, physical fu

Successful use of a Janus-kinase (JAK) inhibitor in chronic chikungunya arthritis refractory to conventional synthetic DMARDs: a case report.

Chronic chikungunya arthritis is a debilitating post-viral inflammatory condition that clinically mimics rheumatoid arthritis and often requires long-term treatment with disease-modifying antirheumatic drugs. Treatment options are limited when conventional synthetic disease-modifying antirheumatic drugs are not tolerated or lose efficacy. We report the case of a 50-year-old Brazilian woman with serologically confirmed chikungunya virus infection who developed chronic chikungunya arthritis with a rheumatoid arthritis-like phenotype. Following an acute febrile illness accompanied by rash and neg

Engineered Extracellular Vesicles for Cartilage Regeneration: Bridging Biological Complexity to Clinical Precision.

Osteoarthritis (OA) is a common degenerative joint disease characterized by pain, stiffness, progressive cartilage loss, and reduced mobility. Current treatments primarily aim to relieve symptoms rather than restore damaged cartilage, and durable regeneration of native hyaline cartilage remains a major clinical challenge. Extracellular Vesicles (EVs), particularly those derived from Mesenchymal Stem Cells (MSCs), have emerged as promising cell-free therapeutic platforms because of their ability to modulate inflammation, regulate chondrocyte activity, and influence extracellular matrix metaboli

Quadriceps Tendon Autograft Harvest Technique Affects Both Graft Strength and Cross-sectional Area.

BACKGROUND: Bone-patellar tendon-bone (BTB) autografts have been considered the gold standard for anterior cruciate ligament reconstruction, although the popularity of quadriceps tendon (QT) autografts is increasing. HYPOTHESIS/PURPOSE: The purpose of this study was to compare the ultrasound-derived cross-sectional area (CSA) and biomechanical strength of BTB and QT grafts harvested with different techniques. The authors hypothesized that ultrasound CSA measurements would be reliable and that QT grafts would be larger and stronger than BTB grafts, without differences between QT harvest method

Emulating the influence of exoskeleton stiffness on primary afferent feedback in rat isolated muscle-tendon unit.

Exoskeletons assist and augment movement, but their effects on proprioceptive feedback remain poorly understood due to challenges in making direct measures of sensory signals in humans. Here, we leveraged a benchtop animal model to explore how mechanical context akin to an elastic exoskeleton operating on a human lower limb joint influences primary muscle spindle firing. In an anesthetized rat preparation, we applied controlled stretches to the medial gastrocnemius with engineered springs (0-0.5 N/mm) in parallel to the muscle-tendon unit (MTU) while modulating muscle activation to maintain ov

Phase Relationship between Spinal Motion and Limb Support Determines High-speed Running Performance in a Cheetah Model with Asymmetric Spinal Stiffness.

Cheetahs are characterized by large spinal flexion and extension during high-speed running, yet the dynamical role of the phase relationship between spinal motion and limb support remains unclear. We aimed to clarify how this phase relationship affects running performance, focusing on the effect of asymmetric spinal stiffness. Using a simple sagittal planar cheetah model with asymmetric stiffness of the spine undergoing extension or flexion, we numerically searched for periodic bounding solutions over a range of stiffness parameters and compared their ground reaction forces, horizontal velocit

An analysis of different reconstruction techniques for the radius: a finite element analysis.

Pathological fractures due to metastatic bone disease frequently require palliative stabilization. While polymethylmethacrylate (PMMA) provides immediate mechanical stabilization, its limited bending and multiaxial properties raise concerns. Reinforcement strategies such as plate fixation, intramedullary nails, and metallic augmentation have been proposed, yet their mechanical behavior under physiological loading remains insufficiently characterized. This study aims to evaluate the mechanical performance of three reconstruction strategies for a proximal radial defect: PMMA alone, PMMA combined

Does the level of the supracondylar humerus fractures affect the stability of pinning configurations? A biomechanical study.

OBJECTIVE: Supracondylar humerus fractures (SHFs) are prevalent injuries in pediatric orthopedics and often necessitate surgical management to restore elbow function and prevent long-term complications. The study aimed to evaluate the biomechanical effects of different fracture levels relative to the olecranon fossa and the efficacy of different pin configurations. METHODS: Synthetic humerus bone models were used to standardize the fracture patterns and simulate surgical interventions. Pin configurations, including lateral divergent and cross-pin configurations, were applied using 3D-printed

Fascial Manipulation Compared to Stretching for Rehabilitation of Children With Cerebral Palsy: Protocol for a Pilot Randomized Controlled Trial.

BACKGROUND: Maintaining and improving motor function is essential in the rehabilitation of children with cerebral palsy (CP). Stretching is traditionally used to prevent muscle tightness and contractures to improve movement patterns in children with CP. Fascial manipulation (FM) is an increasingly used manual therapy method for movement difficulties associated with CP. However, evidence for FM remains limited, and its benefits over conventional stretching are unclear. A pilot randomized controlled trial (RCT) is warranted to assess the feasibility and acceptability of a definitive trial compar

Shear wave elastography for assessing lower extremity muscle stiffness after anterior cruciate ligament reconstruction: A systematic review.

BACKGROUND: Anterior cruciate ligament reconstruction (ACLR) restores passive joint stability, yet athletes who meet strength benchmarks may still land stiffly, hesitate to push off, and re-injure. Muscle stiffness governs how rapidly force is transmitted to the joint, and shear wave elastography (SWE) quantifies it muscle by muscle, but SWE findings after ACLR have not been synthesized. METHODS: Scopus, PubMed, SPORTDiscus, and CINAHL were searched through November 2025 following PRISMA 2020. Eligible studies compared SWE-derived lower-extremity muscle stiffness after ACLR with contralateral
