The Vertebral Artery Window in Endoscopic Ventral Craniovertebral Junction Surgery: Anatomy and Clinical Application.

OBJECTIVE: To define the "vertebral artery window" (VA window) and assess its anatomical basis, CTA depiction, laterality, and preliminary operative feasibility in endoscopic ventral CVJ surgery. METHODS: Fourteen fresh-frozen silicone-perfused cadaveric heads (28 sides) were examined through ventral and posterior approaches. Window dimensions, VA depth from the prevertebral fascia, and VA-to-landmark distances were measured. Cervical CTA datasets from 10 adults were assessed using the same convention. Paired left-right analyses were exploratory. Four endoscopic ventral CVJ procedures were re

Intraosseous Pneumatocyst of the Scapula: Report of Two Cases.

An intraosseous pneumatocyst is a gas-containing cystic bone lesion that is not associated with infection or bone necrosis due to vertebral compression fracture. The most common sites are the pelvis and spine, but it is reported rarely in other sites. To our knowledge, only two cases of intraosseous pneumatocyst in the scapula have been reported. Herein, we report two cases of intraosseous pneumatocyst of the scapula, in a 41-year-old woman and a 51-year-old man. Neither patient had shoulder symptoms, and the lesions were found incidentally during imaging studies. Plain X-rays revealed cystic

Hybrid Posterior Fixation Incorporating Cervical Facet Fixation for the Treatment of Proximal Junctional Failure.

INTRODUCTION: Proximal junctional kyphosis (PJK) and proximal junctional failure (PJF) are well-recognized complications following long thoracolumbar fusion constructs and frequently require complex revision surgery. Achieving stable fixation and solid arthrodesis at proximal levels is particularly challenging in the setting of advanced degeneration and altered anatomy. This case review describes a hybrid fixation strategy incorporating cervical facet fixation to address these challenges. CASE REPORT: A 76-year-old female presented 21 months after T4-pelvis fusion with progressive PJF, result

Cervical Osteophytosis Causing Tracheomalacia and Dyspnea.

Cervical osteophytosis, a common manifestation of degenerative spondylosis, can rarely exert mass effect to compromise adjacent airway structures. Acquired tracheomalacia secondary to anterior cervical osteophytes is exceptionally uncommon and results from dynamic airway collapse due to weakened tracheal cartilaginous support. Tracheomalacia may present with nonspecific respiratory symptoms such as dyspnea, cough, and orthopnea, and diagnosis is best established with direct visualization on bronchoscopy. We report a rare case of localized upper tracheal tracheomalacia caused by cervical diffus

Heiguteng Zhuifeng Huoluo capsule alleviates osteoarthritis associated with glycerophospholipid metabolism remodeling and PI3K/AKT/mTOR-related inflammatory signaling.

Osteoarthritis (OA) is characterized by cartilage destruction, osteophyte formation, and chronic inflammation. Heiguteng Zhuifeng Huoluo Capsule (HZHC) is a traditional preparation used to relieve knee joint pain and swelling. However, its anti-osteoarthritic effects and pharmacological mechanisms remain unclear. Here, an OA rat model was established by anterior cruciate ligament transection combined with excessive running. Following HZHC intervention, OA-related pathological changes were systematically evaluated. UHPLC-Q-Orbitrap HRMS technology was used to identify the chemical constituents

Therapeutic Potential of Intra-Articular Delivery of Allogeneic Dedifferentiated Fat Cells in a Canine Monoiodoacetate-Induced Knee Osteoarthritis Model.

Osteoarthritis (OA) is the leading cause of disability worldwide. Although rodent models are widely used in OA research, the disease is often artificially accelerated in these systems, and joint biomechanics differ substantially from those of humans. In contrast, canine models offer greater translational relevance due to similarities in joint size, cartilage thickness, and physiological mechanical loading of the knee, making them suitable surrogates for evaluating regenerative therapies. In this study, we assessed the therapeutic efficacy of allogeneic dedifferentiated fat (DFAT) cells, which

Research progress on cellular senescence in the pathogenesis and treatment of osteoarthritis and temporomandibular joint osteoarthritis.

Osteoarthritis (OA) is a common joint disorder characterized primarily by cartilage degeneration and osteophyte formation, leading to a substantial decline in patients' quality of life. Temporomandibular joint OA (TMJOA) is a degenerative lesion within temporomandibular joint disorders, accounting for approximately 8%-16% of diagnosed cases. Its clinical manifestations include joint pain, limited mouth opening, joint noises, and related symptoms. Cellular senescence plays a pivotal role in OA pathogenesis. Senescent processes contribute to functional impairment of chondrocytes, synovial cells,

Traditional Chinese Medicine Bone-Setting Techniques Research Progress for the Treatment of Knee Osteoarthritis.

This paper systematically reviews the biological mechanisms and clinical applications of traditional Chinese medicine (TCM) manual therapy in the treatment of knee osteoarthritis (KOA), explores the scientific basis for its multipathway intervention in improving joint function, and analyzes the current research limitations. KOA is a degenerative joint disease characterized by articular cartilage degeneration, synovitis, and osteophyte formation. Its pathogenesis is closely related to mechanical stress imbalance, abnormal elevation of proinflammatory factors (e.g., IL-1β and TNF-α), and chondro

Dorsal C1-2 Spinal Cord Compression After Reduction of Atlantoaxial Posterior Dislocation Associated With Os Odontoideum: A Case Report and Literature Review.

BACKGROUND: Atlantoaxial posterior dislocation associated with os odontoideum is uncommon, and experience with its surgical management remains limited. Posterior reduction and C1-2 fixation can usually restore atlantoaxial alignment and stability. However, neurological safety after reduction depends not only on the quality of realignment but also on the residual space available for the spinal cord. In patients with congenital atlas hypoplasia or posterior arch osteophytes, reduction may unmask or aggravate dorsal C1-2 canal compromise. This mechanism of postoperative neurological deterioration

Cervical diffuse skeletal hyperostosis as a rare cause of dysphagia: Two surgical cases.

BACKGROUND: Cervical diffuse idiopathic skeletal hyperostosis (DISH), also known as Forestier's disease, is an uncommon but potentially reversible cause of dysphagia due to mechanical compression of the hypopharynx or upper esophagus. CASE DESCRIPTION: Two male patients presented with progressive dysphagia caused by anterior cervical DISH. Radiological evaluation demonstrated extensive anterior osteophyte formation resulting in significant hypopharyngeal and esophageal compression. Both patients underwent anterior cervical osteophytectomy, which led to immediate and complete resolution of dys

Neural Shape Modeling Reveals Early and Progressive Femoral Bone Shape and Cartilage Thickness Changes After Anterior Cruciate Ligament Reconstruction.

Early and longitudinal shape changes in femoral bone and cartilage after anterior cruciate ligament reconstruction (ACLR) remains poorly understood. It is also unknown whether post traumatic shape changes follow similar trajectory as progressive alterations seen in idiopathic osteoarthritis (OA). Two neural shape models (NSMs)-B-NSM (bone-only) and BC-NSM (bone + cartilage) encoded femoral bone and cartilage shapes from 17 ACLR, contralateral, and healthy control knees at 3 weeks and 3, 9, 18, and 30 months post-ACLR. Knee OA severity shape scores (B-Score, BC-Score), bone surface area, and ca

Can Deep Learning Methods Differentiate Temporomandibular Joint Disorders From Healthy Joints? A 3D Artificial Intelligence Algorithm Study Based on CBCT Images.

BACKGROUND: Given the high difficulty of interpreting complex bone-related temporomandibular joint disorders (TMD) on CBCT scans, developing advanced artificial intelligence models has become essential for accurate diagnosis and detailed subcategorization. OBJECTIVES: This study aimed to develop deep learning models capable of performing a wide range of tasks in the evaluation of the temporomandibular joint on CBCT scans. METHODS: This study followed a four-stage design. First, an nnU-Net v2-based model was developed for automatic segmentation of mandibular condyles on CBCT images. Second, a

Ten Years of Lumbar Transforaminal Epidural Steroid Injections in a Tertiary Referral Center: Trends in Clinical Indications and Outcomes.

INTRODUCTION: Transforaminal epidural steroid injection (TFESI) is commonly applied for lumbar radicular pain. High-level evidence supports the use in disc-related radiculopathy, while evidence for TFESI in neuroforaminal stenosis remains consensus-based. METHODS: A retrospective repeated cross-sectional study was conducted covering January 2013-December 2022. The dataset comprised 11,208 unique patients reimbursed for TFESI. Trends in clinical indications were analyzed using automated keyword-based text recognition. Representative cohorts from 2014 (n = 266) and 2022 (n = 299) were randomly

Surgical management of ventral epidural cerebrospinal fluid leak: a scoping review and illustrative case.

INTRODUCTION: Spontaneous intracranial hypotension (SIH) is a neurological disorder caused by cerebrospinal fluid (CSF) leaks, most often spinal in origin, and is characterized by orthostatic headache, nausea, and cognitive impairment that can markedly reduce quality of life. Epidural blood patching (EBP) is first-line therapy, but spinal leaks due to ventral dural tears are less likely to respond and often require surgical repair. This scoping review synthesizes current evidence on surgical management of ventral spinal CSF leaks from dural tears and presents an illustrative case of osteophyte

Mechanistic insights into gut microbiota dysbiosis in osteoarthritis based on the gut-joint axis.

Osteoarthritis (OA) is a highly prevalent degenerative joint disease characterized by articular cartilage degradation, extracellular matrix disruption, subchondral bone sclerosis, and osteophyte formation, pathological features that collectively impair joint function and significantly reduce patients' quality of life. Therefore, elucidating the systemic pathogenic mechanisms of OA is essential for enabling early diagnosis and implementing disease modifying interventions. Recent studies demonstrate that the gut microbiota, as a key regulatory factor, actively participates in OA pathogenesis via

Tonsillar Fullness Mimicking Malignancy Secondary to a Cervical Osteophyte: A Case Report.

Tonsillar asymmetry and firmness on examination often raise concern for underlying malignancy, particularly squamous cell carcinoma or lymphoma. However, rare benign etiologies may mimic these findings and lead to diagnostic uncertainty. We present the case of an 83-year-old female who presented with globus sensation and was found to have left-sided tonsillar fullness with firmness on physical exam, raising concern for malignancy. Flexible laryngoscopy was unremarkable. Computed tomography of the neck demonstrated a prominent anterior cervical osteophyte at the C1-C2 level, causing mass effect
