BACKGROUND: Transplant-associated thrombotic microangiopathy (TA-TMA) is a serious complication of hematopoietic cell transplantation (HCT) associated with high morbidity and mortality. The disease is driven by endothelial injury and complement dysregulation. Eculizumab, a complement C5 inhibitor, has been used off-label for TA-TMA, but reported outcomes are variable. Real-world response rates and predictors of treatment failure remain incompletely defined, particularly in children. Additionally, the optimal time point to assess treatment response has been identified as a key gap in the field.

INTRODUCTION: Thrombotic microangiopathy (TMA) with associated nephrotic syndrome is a unique and rare condition that is infrequently described in pediatrics.
CASE PRESENTATIONS: Four children with steroid-resistant nephrotic syndrome (SRNS), immune-mediated and of monogenic origin, presented with TMA during late stages of chronic kidney disease. All patients tested negative for Shiga toxin and had normal A Disintegrin-like and Metalloprotease with Thrombospondin type 1 repeats 13 (ADAMTS13) activity. Genetic and functional studies of complement dysregulation were negative. Three of the 4 pat

IgA Nephropathy (IgAN) is one of the most common causes of glomerulonephritis. Biopsy is necessary for definitive diagnosis, though due to variability in severity and progression patients may forego biopsy initially. However, certain clinical presentations and histopathological findings may be significant predictors of poor prognosis. We describe a case of IgAN initially presenting as malignant hypertension. Retrospective chart review suggested previous hospital encounters with features of IgAN, before loss to follow up. Subsequent biopsy evidence of co-occurrent IgA deposition and thrombotic

Liver transplantation remains the definitive treatment for end-stage liver disease and acute liver failure, yet a critical and persistent shortage of donor organs results in thousands of preventable deaths annually worldwide. Xenotransplantation has emerged as a potential solution to this structural deficit. This narrative review traces the evolution of liver xenotransplantation, from early non-human primate trials in the 1960s through the application of CRISPR/Cas9-driven multi-gene editing platforms in contemporary porcine donors. The immunological barriers that drove the transition from pri

Hemolytic uremic syndrome (HUS) is an uncommon but serious cause of thrombotic microangiopathy (TMA) in adults and can pose diagnostic challenges, especially in patients receiving chemotherapy, due to overlapping features with chemotherapy-induced TMA. We report the case of a 55-year-old woman with advanced ovarian cancer undergoing neoadjuvant chemotherapy who developed Shiga toxin-producing Escherichia coli (STEC)-associated HUS. The patient developed severe oligo-anuric acute kidney injury (AKI), requiring hemodialysis and multiple transfusions. Early recognition, prompt supportive care, an

INTRODUCTION: Daboia siamensis envenoming is a major cause of hematotoxicity and acute kidney injury in Southeast Asia. In Thailand, D. siamensis is a clinically important snake, yet detailed national data on its bites are limited. Hence, we aimed to determine the clinical characteristics, management, and outcomes of D. siamensis bites in Thailand, identify factors associated with acute kidney injury and hemodialysis, and analyze antivenom administration with reference to acute kidney injury and hemodialysis.
METHODS: We conducted a 10-year retrospective cross-sectional study using data from

BACKGROUND: Nonbacterial thrombotic endocarditis (NBTE) is an uncommon but important cause of systemic arterial embolism, most often occurring in advanced malignancy. Diagnosis is frequently difficult because vegetations may be small, transient, or not visualized on echocardiography, which may delay recognition.
CASE PRESENTATION: A 37-year-old woman presented with multifocal ischemic strokes and visceral infarctions. Initial transesophageal echocardiography (TEE) showed a small isoechoic structure on the mitral valve, interpreted as a possible vegetation. Despite empiric broad-spectrum antib

BACKGROUND: Multi-target tyrosine kinase inhibitors (TKIs) are cornerstone therapies for solid malignancies, yet their association with renal thrombotic microangiopathy (TMA)-a severe, irreversible adverse event-remains incompletely defined. This study delineates TKI-associated renal TMA's clinical features, onset patterns, and outcomes.
METHODS: We systematically searched PubMed, Embase, Cochrane Library, and Web of Science for case reports/series of renal TMA linked to 7 multi-target TKIs (sunitinib, sorafenib, etc.) published through January 2026. Key data were extracted and analyzed descr

BACKGROUND: HIV is the most common cause of secondary thrombotic thrombocytopenic purpura (TTP) in South Africa.
OBJECTIVES: To assess the clinical presentations and outcomes of patients treated for HIV-associated and idiopathic TTP.
METHOD: We conducted a retrospective cohort study of patients consecutively diagnosed with TTP from 2010 to 2020 at Groote Schuur Hospital. Patients were identified by reviewing hospital and Western Cape Blood Services records. Kaplan-Meier curves and log-rank tests were used to evaluate remission rates, both overall and by HIV status and treatment group. Logist

Avermectin, a commonly used agricultural pesticide and antiparasitic agent, typically causes acute human toxicity characterized by central nervous system depression and gastrointestinal disturbances. However, its association with thrombotic microangiopathy has not been previously reported. We describe a 60-year-old woman who developed anuria after ingesting 300 mL (15 g) of avermectin. Laboratory investigations demonstrated thrombocytopenia, microangiopathic hemolytic anemia, and acute kidney injury. Renal biopsy revealed thrombotic microangiopathy-like lesions. Thrombotic thrombocytopenic pur

Transplantation-associated thrombotic microangiopathy (TA-TMA) is a severe complication following hematopoietic stem cell transplantation, with its pathogenesis not yet fully understood and a lack of highly effective treatment options currently available. This article reports a case of a female patient with acute myeloid leukemia (AML) who was diagnosed with TA-TMA three months after a second allogeneic hematopoietic stem cell transplantation (allo-HSCT). After promptly discontinuing cyclosporine (CSA), and following ineffective treatments with anti-infective therapy, glucocorticoids, and plas

Malignant hypertension is traditionally defined as severe systemic blood pressure elevation accompanied by end-organ damage. In kidney transplant recipients, however, kidney allografts may be particularly vulnerable to hypertensive injury even at lower blood pressure levels, owing to impaired autoregulation associated with denervation and calcineurin inhibitor exposure. We report a 52-year-old kidney transplant recipient presenting with severe kidney injury (serum creatinine, 4.90 mg/dL) 16 months after transplantation. Although systemic blood pressure remained within the range of 140-170/90-1

INTRODUCTION: Renal resistive index (RRI), derived from Doppler ultrasound sonography (DUS), is widely utilized for monitoring in kidney allotransplantation. However, its applicability in kidney xenotransplantation (KXTx) has not been thoroughly investigated. This study assesses the potential of RRI as an early predictive indicator of thrombotic microangiopathy (TMA), a common diagnosis, often associated with graft loss in KXTx.
METHODS: Five consecutive pig-to-baboon KXTx were included in this prospective study. The first three cases comprised the study group, while the subsequent two served

Atypical hemolytic uremic syndrome (aHUS) is a complement-associated thrombotic microangiopathy (TMA). Thalassemia is a hemolytic anemia triggered by defects in the beta-globin gene. Mutations in the complement factor H (CFH) gene are associated with the development of aHUS; however, CFH gene mutations coexisting with thalassemia have rarely been reported. CFH gene mutations are susceptibility factors that can trigger complement overactivation during pregnancy. When aHUS is combined with thalassemia, large amounts of hemoglobin are released from fragmented erythrocytes, leading to amplificatio

BACKGROUND: Transplant-associated thrombotic microangiopathy (TA-TMA) is a severe and potentially fatal complication following allogeneic hematopoietic stem cell transplantation (allo-HSCT). Early identification of patients at risk is essential for timely intervention and improved outcomes. This study aimed to identify biomarkers capable of predicting the early risk of TA-TMA in patients with severe aplastic anemia (SAA) undergoing allo-HSCT.
METHODS: In a prospective nested case-control study of patients with SAA undergoing allo-HSCT, blood samples were collected before pretreatment, before

Atypical hemolytic uremic syndrome (aHUS) is a rare, life-threatening complement-mediated thrombotic microangiopathy (TMA) associated with high morbidity, mortality, and substantial healthcare resource utilization (HCRU). Although complement inhibitors such as eculizumab and ravulizumab have dramatically improved outcomes, aHUS continues to impose significant economic and operational burden on healthcare systems. In the Gulf region, payers and health systems face increasing pressures driven by rising costs, delayed diagnosis, fragmented access, and limited real-world evidence necessary for inf
