Sepsis-associated thrombotic microangiopathy-like syndrome following cardiac surgery: a case report.

Severe sepsis following cardiac surgery may be accompanied by a thrombotic microangiopathy (TMA)-like syndrome distinct from classic immune-mediated thrombotic thrombocytopenic purpura (TTP) and overt disseminated intravascular coagulation (DIC). We report a 62-year-old man who developed sepsis after mitral valve replacement, complicated by thrombocytopenia (nadir 45 × 10⁹/L), microangiopathic hemolysis (2.6% schistocytes), and multiple organ dysfunction. Laboratory findings revealed marked dysregulation of the von Willebrand factor (vWF)/ADAMTS13 axis, with markedly elevated vWF antigen level

Sjögren disease and idiopathic inflammatory myopathy overlap presenting as complement-mediated thrombotic microangiopathy/atypical haemolytic uremic syndrome with a complement factor B variant: A case report.

Complement-mediated thrombotic microangiopathy (CM-TMA) is often triggered by autoimmune diseases; however, reports of CM-TMA in patients with Sjögren disease (SjD) and idiopathic inflammatory myopathy (IIM) are limited. We describe the case of a 48-year-old woman with overlapping SjD and IIM who presented with interstitial lung disease. After initiation of combination therapy with calcineurin inhibitor, intravenous cyclophosphamide, and glucocorticoids, she developed progressive thrombocytopenia with schistocytosis, haemolytic anaemia, and acute renal dysfunction. Laboratory testing revealed

Fatal TAFRO syndrome in which adrenal hemorrhage preceded thrombocytopenia.

TAFRO syndrome is a rare systemic inflammatory disease characterized by thrombocytopenia, anasarca, fever, renal dysfunction, and myelofibrosis, and its follows a rapidly fatal clinical course. A few cases of TAFRO syndrome complicated by adrenal hemorrhage were recently reported; however, its pathogenetic significance remains largely unclear. The present case was a 59-year-old man presenting with left lower back pain. Imaging studies revealed left adrenal hemorrhage. Although an elevated C-reactive protein level was observed in the initial visit, there were no obvious abnormalities in the plat

Quercetin and bradykinin potentiating peptide protect against Busulfan induced liver and kidney damage in Sprague Dawley rats: an ultrastructural study.

Busulfan (BUS), a chemotherapeutic agent used to treat chronic myeloid leukemia, is associated with hepatotoxicity and nephrotoxicity. This study used transmission electron microscope (TEM) to characterize BUS-induced ultrastructural alterations in the liver and kidney of rats, and to evaluate the protective effects of quercetin (QCT), bradykinin potentiating peptide (BPP) and their combination. To our knowledge, these ultrastructural changes have not been previously reported. Rats received two BUS doses (10 mg/kg b.w.) at a 21-day interval. BUS caused marked hepatic alterations, including mit

Malignancy-Associated Microangiopathic Hemolytic Anemia Mimicking Thrombotic Thrombocytopenic Purpura in a Patient With Metastatic Breast Cancer: A Case Report.

Microangiopathic hemolytic anemia (MAHA) is a form of Coombs-negative hemolysis characterized by mechanical fragmentation of erythrocytes, producing schistocytes on the peripheral blood smear. When accompanied by thrombocytopenia and neurological features, the presentation closely resembles thrombotic thrombocytopenic purpura (TTP), a hematological emergency requiring urgent plasma exchange. However, advanced malignancy can produce an almost identical thrombotic microangiopathy (TMA) that responds poorly to plasma exchange, and for which the definitive treatment is control of the underlying ca

Potential Benefit of Ruxolitinib in Suspected Ocular and Neurological Transplant-Associated Thrombotic Microangiopathy After Allogeneic Stem Cell Transplantation: A Case Report and Review.

We describe a rare case of suspected delayed-onset TA-TMA with ocular and neurological involvement following allo-HSCT. The condition was refractory to standard therapies; neurological improvement was temporally associated with ruxolitinib initiation, suggesting a possible GVHD overlap. This report supports a multidisciplinary approach to complex post-transplant endothelial injury and supports further investigation of JAK inhibition in selected cases.

Kidney transplantation in catastrophic antiphospholipid syndrome survivors: Results from a nationwide series of 12 cases.

Catastrophic antiphospholipid syndrome (CAPS) is a rare, life-threatening subtype of APS that may progress to end-stage kidney disease. CAPS survivors may have limited access to kidney transplantation (KT). We conducted a retrospective nationwide study including all French KTs performed in recipients with a history of CAPS between 2008 and 2024. Twelve KTs were performed in 10 patients (median age, 45 [interquartile range {IQR}, 41-56] years), all from deceased donors. Anticoagulation therapy was resumed 4 (IQR, 0-5) hours after KT and reached therapeutic levels at 12 (IQR, 6-28) hours. Preven

Bilateral postpartum renal cortical necrosis complicated by peripartum cardiomyopathy after postpartum hemorrhage: a case report.

A 36-year-old woman developed postpartum haemorrhage after an uncomplicated vaginal delivery, followed by disseminated intravascular coagulation, thrombotic microangiopathy, and persistent anuric kidney failure. Contrast-enhanced computed tomography showed bilateral reversal of corticomedullary enhancement, consistent with renal cortical necrosis. Despite kidney replacement therapy and 13 litres of ultrafiltration, she remained oxygen-dependent, prompting cardiac evaluation on postpartum day 11, which revealed severe left ventricular systolic dysfunction (ejection fraction 25%) consistent with

Scleroderma Renal Crisis-Associated Thrombotic Microangiopathy Preceding Skin Sclerosis in Diffuse Cutaneous Systemic Sclerosis: A Case Report.

A prompt and accurate diagnosis is essential because the management of thrombotic microangiopathy (TMA) depends on its underlying etiology. We report a case of diffuse cutaneous systemic sclerosis (dcSSc) in which scleroderma renal crisis-associated TMA preceded the development of overt skin sclerosis. A 53-year-old woman presented with TMA and severe renal dysfunction. She tested positive for anti-RNA polymerase III antibodies and subsequently developed skin sclerosis, leading to a diagnosis of dcSSc. Angiotensin-converting enzyme inhibitor therapy resulted in a clinical improvement in the pa

Biopsy-proven Systemic Lupus Erythematosus and Antineutrophil Cytoplasmic Antibody-associated Vasculitis Overlap Syndrome Developed during Propylthiouracil Therapy: A Case-based Review.

A 31-year-old woman who had been receiving propylthiouracil (PTU) treatment for Graves' disease was referred to our hospital for the evaluation of hematuria and proteinuria. Laboratory tests showed positivity for myeloperoxidase-antineutrophil cytoplasmic antibody (ANCA), proteinase 3-ANCA, and anti-double-stranded DNA antibodies. A kidney biopsy revealed immunocomplex-mediated mesangial proliferative glomerulonephritis, consistent with lupus nephritis (LN) diagnosis. Additionally, tubulitis, peritubular capillaritis without immune complex deposition, and medullary angiitis were observed. We h

When two rare diseases meet: atypical hemolytic uremic syndrome and alkaptonuria in two siblings.

Atypical hemolytic uremic syndrome (aHUS) is a rare complement-mediated thrombotic microangiopathy (TMA), whereas alkaptonuria (AKU) is an uncommon inherited metabolic disorder. Their coexistence has not previously been described. We report two siblings from a consanguineous family with CD46-associated aHUS and AKU to highlight an unusual diagnostic challenge and the importance of recognizing coexisting inherited diseases. Two brothers experienced recurrent episodes of TMA characterized by microangiopathic hemolytic anemia, thrombocytopenia, and acute kidney injury, while maintaining preserved

[A Case of Nephrotic Syndrome Developing Early After Initiation of Fruquintinib Therapy].

Fruquintinib (FRU) is a vascular endothelial growth factor receptor (VEGFR)-1, 2, 3 tyrosine kinase inhibitor (TKI) approved for metastatic colorectal cancer. While hypertension, fatigue, and hand-foot syndrome are common adverse events, severe proteinuria and nephrotic syndrome are rare and has not been reported in clinical trials. A woman in her 60s with recurrent rectal cancer began fruquintinib at 5 mg/d as a 4th-line treatment. She had a history of hypertension and was taking amlodipine. On day 8 of the first cycle, she developed hypertension (grade 2) . On day 22, proteinuria (grade 3) w

Clinical Practice Guideline: The Diagnosis, Treatment, and Follow-Up Management of Thrombotic Thrombocytopenic Purpura.

BACKGROUND: Thrombotic thrombocytopenic purpura (TTP) is a life-threatening thrombotic microangiopathy. The global incidence of immune-mediated TTP (iTTP) is 1-6 cases per million people per year. The new (S3-level) clinical practice guideline is the first in the German-speaking world to compile evidence- and consensus-based recommendations on the diagnosis, treatment, follow-up management, and care of patients with immune-mediated thrombotic thrombocytopenic purpura (iTTP) and congenital thrombotic thrombocytopenic purpura (cTTP). METHODS: This clinical practice guideline was developed under

Current standards and novel concepts in thrombotic thrombocytopenic purpura.

Thrombotic thrombocytopenic purpura (TTP) is a rare and rapidly life-threatening thrombotic microangiopathy. Disseminated microvascular thrombosis and resulting ischemia are the critical events determining outcome, if left untreated. The pathogenic hallmark is a severe deficiency of the plasma metalloprotease ADAMTS13, resulting in impaired cleavage of ultra-large von Willebrand factor (vWF) multimers leading to platelet aggregation. The vast majority of TTP cases are acquired and immune-mediated, depleting ADAMTS13 through autoantibodies. In contrast, congenital deficiency of ADAMTS13 affects

Subglottic stenosis in granulomatosis with polyangiitis: clinical, histopathological and therapeutic considerations with presentation of two cases.

Granulomatosis with polyangiitis (GPA), once known as Wegener's granulomatosis (WG), is an uncommon systemic vasculitic disorder characterized by inflammation of the vascular walls, particularly affecting the respiratory tract. One serious complication is subglottic stenosis (SGS), a narrowing of the airway just below the vocal cords. While uncommon, it can be life-threatening and may show up as stridor, dyspnea, or voice changes. Sometimes, it is the first or only sign that someone has GPA. Diagnosing this condition takes a combination of clinical evaluation, imaging, endoscopy, and blood tes
