Severe systemic lupus erythematosus with thrombotic microangiopathy: rescue therapy with pegcetacoplan.

This report describes the case of a 12-year-old female patient with severe lupus nephritis and refractory thrombotic microangiopathy, who required dialysis for several months despite receiving intensive immunosuppressive treatment, including terminal complement blockade with eculizumab. Following a switch in therapy from eculizumab to pegcetacoplan, there was a significant reduction in haemolytic activity and steady improvement in kidney function. After more than 8 months of haemodialysis, the patient could be weaned off dialysis. This case study highlights the therapeutic potential of pegceta

[Once upon a time in nephrology - Episode 3: A brief history of preeclampsia].

Preeclampsia is a maternal syndrome of placental origin that complicates 2–5% of pregnancies. It remains one of the leading causes of maternal mortality worldwide and is a major contributor to fetal and neonatal morbidity and mortality. It is classically defined, after 20 weeks of gestation, by the combination of arterial hypertension and proteinuria ≥ 0.3 g/24 h. In its severe form, it may induce endothelial injury consistent with thrombotic microangiopathy in the liver and kidneys and can lead to fatal posterior cerebral edema. For many years, preeclampsia was referred to as the “disease of

Thrombotic microangiopathy with features of thrombotic thrombocytopenic purpura in a patient with Vibrio parahaemolyticus bacteremia: a rare case report.

We report the first documented case of thrombotic microangiopathy highly suggestive of thrombotic thrombocytopenic purpura (TTP) associated with Vibrio parahaemolyticus bacteremia in an immunocompetent adult. A 62-year-old man developed acute gastroenteritis following seafood ingestion and rapidly progressed to bacteremia with severe thrombocytopenia, microangiopathic hemolytic anemia, acute kidney injury, and neurological impairment. Laboratory evaluation revealed  ~ 3% schistocytes, markedly elevated lactate dehydrogenase, indirect hyperbilirubinemia, preserved coagulation parameters, and a

Pulmonary Tumor Thrombotic Microangiopathy in a Patient With Male Breast Cancer: A Report of a Rare Case.

Pulmonary tumor thrombotic microangiopathy (PTTM) is a rare but serious complication that can occur in patients with advanced cancer. It often leads to rapidly worsening respiratory failure and right heart failure and is typically diagnosed only postmortem. While PTTM has been reported in various cancers, it is extremely rare in male patients with breast cancer, which is itself an uncommon malignancy. We report the case of a man with hormone receptor-positive breast cancer who underwent surgery followed by adjuvant chemotherapy. Several months later, he developed progressive dyspnea and increa

Hemoglobinuria-associated acute kidney injury in hemolytic uremic syndrome without renal thrombotic microangiopathy.

BACKGROUND: Hemolytic uremic syndrome (HUS) is classically defined by the triad of microangiopathic hemolytic anemia, thrombocytopenia, and acute kidney injury (AKI) and is most often associated with renal thrombotic microangiopathy (TMA). However, the clinical triad may rarely occur in the absence of histologically demonstrable renal TMA. CASE PRESENTATION: A 64-year-old woman with hypertension treated with an ACE inhibitor presented with asthenia, nausea, dark urine, and oliguria. Laboratory evaluation revealed AKI (serum creatinine 4.5 mg/dL), thrombocytopenia (46 × 10⁹/L), and intravascul

Beyond HELLP: An Unusual Cause of Severe Thrombocytopenia in Pregnancy.

Thrombotic thrombocytopenic purpura (TTP) is a rare yet potentially fatal thrombotic microangiopathy that poses significant risks during pregnancy. It is characterized by microangiopathic hemolytic anemia, profound thrombocytopenia, and widespread microvascular thrombosis. We present a case of pregnancy-associated TTP presenting with isolated severe thrombocytopenia, highlighting the diagnostic challenge. A 27-year-old, G3P1 woman at 31+6 weeks of gestation presented with hypogastric pain and a resolved episode of vomiting. Laboratory evaluation revealed severe thrombocytopenia (platelets: 9 ×

Circuit Survival in Children with Hemolytic Uremic Syndrome Requiring Continuous Renal Replacement Therapy.

INTRODUCTION: Hemolytic uremic syndrome (HUS) is a significant cause of acute kidney injury (AKI) in children that is caused by Shiga toxin-producing Escherichia coli. Continuous renal replacement therapy (CRRT) is frequently used for managing AKI in critically ill pediatric patients. The filter lifespan is a crucial factor affecting its effectiveness and cost. This study aimed to investigate the impact of HUS on the lifespan of CRRT filters compared with that of other etiologies. MATERIALS AND METHODS: This retrospective longitudinal study was conducted in a pediatric intensive care unit in

Beyond glomeruli: interstitial fibrosis and tubular atrophy predict poor renal outcomes in lupus nephritis.

OBJECTIVES: To evaluate the prevalence, associated factors, and prognostic performance of NIH activity index–defined interstitial inflammation (NIH-TII) and interstitial fibrosis and tubular atrophy (IFTA) in lupus nephritis (LN). METHODS: This retrospective cohort study analyzed 195 renal biopsy episodes from 135 LN patients. Tubulointerstitial lesions were graded according to histopathological severity. NIH-TII was assessed semiquantitatively using the interstitial inflammation component of the modified NIH activity index. RESULTS: Among 195 renal biopsies, class IV LN was most frequent (49.

Expression of Human Thrombomodulin Prevents Early Thrombocytopenia and Thrombotic Microangiopathy in Pig-to-Nonhuman Primate Orthotopic Liver Xenotransplantation.

BACKGROUND: Severe thrombocytopenia and coagulation dysregulation remain major barriers to survival in pig-to-primate liver xenotransplantation models. METHODS: Porcine livers with five genetic modifications (GTKO, CMAHKO, β4GalNT2KO, hCD55, and hTBM) were orthotopically transplanted into four Tibetan macaques under two conventional immunosuppressive protocols. Longitudinal monitoring included graft and multi-organ function, hematology, coagulation, and immune responses. Histopathological and immunohistochemical analyses were performed to characterize graft and recipient pathology. RESULTS:

Safety and Efficacy of Therapeutic Plasma Exchange in Pediatric Non-Neurological Diseases.

INTRODUCTION: Therapeutic plasma exchange (TPE) is a recognized treatment in pediatric immune-mediated and toxicological disorders, though evidence remains limited. This study assesses the safety and efficacy of TPE in children with non-neurological diseases treated at a tertiary hospital over 27 years. We focused on non-neurological indications to avoid excessive data heterogeneity. METHODS: This retrospective analysis included 97 children (51 nephrological, 46 non-nephrological), who underwent 380 TPE procedures between 1997 and 2023. Indications were categorized according to ASFA 2023 guid

The complement factor B inhibitor iptacopan for relapsed and refractory immune thrombotic thrombocytopenic purpura.

BACKGROUND: Thrombotic thrombocytopenic purpura (TTP) is a rare and life-threatening thrombotic microangiopathy, classified as congenital TTP (cTTP) and immune TTP (iTTP). iTTP represents the most common clinical subtype, accounting for approximately 95% of cases. Primary clinical features include microangiopathic hemolytic anemia (MAHA), thrombocytopenia, neuropsychiatric symptoms, fever, and renal involvement. Reduced ADAMTS13 activity and dysregulation of the complement pathway contribute to the pathogenesis of iTTP. First-line therapy for newly diagnosed iTTP, comprising plasma exchange an

Modifier-Sensitive Phenotypic Divergence in XMEN Disease (MAGT1 Deficiency): Neurodegenerative and Immuno-Hematologic Trajectories.

Background: X-linked immunodeficiency with magnesium defect, Epstein-Barr virus (EBV) infection, and neoplasia (XMEN) disease is a rare inborn error of immunity caused by loss-of-function mutations in MAGT1, leading to impaired N-linked glycosylation. Although chronic EBV viremia is a hallmark of XMEN disease, the mechanisms underlying its marked clinical heterogeneity remain poorly understood. Methods: We performed an in-depth clinical, immunological, and genetic characterization of two siblings carrying a pathogenic MAGT1 variant (c.369_370insCC; p.Gly124fs), validated and deposited in ClinV

Thrombocytopenia in Pregnancy: Clinical Challenges, Maternal-Fetal Risks, and Management Strategies.

Thrombocytopenia affects up to 10% of pregnant women and represents the second most common blood disorder during pregnancy. Its causes include immune-mediated thrombocytopenia, hemolysis, elevated liver enzymes and low platelet count (HELLP) syndrome, preeclampsia (PE), and benign pregnant thrombocytopenia. Diagnosis is crucial because the cause dictates the effect on maternal health, pregnancy management, and neonatal outcomes. This narrative review examines the range of thrombocytopenia during pregnancy, primarily focusing on diagnostic evaluation, underlying pathophysiological causes, and d

Thrombotic microangiopathy overlap syndrome in pregnancy.

In these two cases, we highlight the concept of pregnancy-associated thrombotic microangiopathy (TMA) overlap syndrome, where two different TMAs occur together, likely one driving another. Thrombotic microangiopathies occurring in pregnancy threaten the lives of both mother and fetus, and yet precise diagnosis may be difficult because of overlapping clinical and laboratory features. The three main syndromes to be considered are thrombotic thrombocytopenic purpura (TTP) - an acute disorder caused by severe ADAMTS13 deficiency; preeclampsia/HELLP syndrome - characterised by haemolysis, elevated

Challenges and perspectives in xenoliver research: lessons from decedent and primate models.

Liver xenotransplantation is a potential strategy to address the shortage of donor livers, either as a temporary bridge to allotransplantation, bridge to native liver recovery, or destination therapy. Advances in genetic engineering and immunosuppression have enabled liver xenografts to evade hyperacute rejection, yet durable graft survival remains elusive. Profound thrombocytopenia, consumptive coagulopathy, and xenotransplantation-associated thrombotic microangiopathy limit long-term graft survival. Evidence from nonhuman primates, decedents, and recent clinical models suggests that hematolo

First-in-human nuclease-free homologous recombination-dependent gene editing in pediatric patients with methylmalonic acidemia: results of a phase 1/2 study.

Gene-based editing can potentially correct the genetic defect in methylmalonic acidemia (MMA). SUNRISE, a first-in-human phase 1/2 open-label study, evaluated the safety/tolerability (primary endpoints) of liver-targeted hLB-001 in four pediatric participants (ages 20-114 months) with mitochondrial methylmalonyl-CoA mutase (MMUT)-deficient MMA. We designed a single-infusion adeno-associated viral capsid (hLB-001) to nondisruptively integrate functional MMUT at the 3' end of the albumin (ALB) locus to produce both albumin and MMUT. All four participants experienced at least one treatment-emerge
