Clinicopathologic spectrum and prognostic determinants in biopsy-proven thrombotic microangiopathy.

BACKGROUND: Thrombotic microangiopathy (TMA) represents a heterogeneous clinicopathologic entity characterized by endothelial injury, microvascular thrombosis, and variable clinical outcomes. Data on prognostic factors in biopsy-proven TMA remain limited. METHODS: This retrospective single-center study included 46 adult patients with biopsy-proven TMA diagnosed between 2000 and 2025. Demographic, clinical, laboratory, histopathologic, and genetic parameters were analyzed. The composite adverse outcome was defined as death or progression to end-stage renal disease (ESRD). Logistic regression a

Allogeneic haematopoietic stem cell transplantation for severe Still's disease: a retrospective report of three paediatric patients.

OBJECTIVES: Severe forms of systemic JIA (sJIA), also called paediatric-onset Still's disease are associated with two major life-threatening complications: macrophage activation syndrome (MAS) and severe lung disease. Patients are usually resistant to conventional synthetic (cs) DMARDs, biologic (b) DMARDs, and targeted synthetic (ts) DMARDs. Recently, allogeneic haematopoietic stem cell transplantation (HSCT) has been performed in a small number of patients with refractory and life-threatening disease. We aimed to report outcomes and complications of allogeneic HSCT in patients with severe, r

Postpartum TMA requiring dialysis with discordant complement tests: a case report.

Pregnancy induces vascular, coagulation, and immune alterations that predispose to thrombotic microangiopathy (TMA). Within this spectrum-preeclampsia/ hemolysis, elevated liver enzymes, and low platelets syndrome (HELLP) and complement-mediated TMA (CM-TMA)-overlapping features complicate diagnosis, whereas management and prognosis diverge. A 39-year-old woman developed severe hypertension at 35 weeks' gestation, underwent emergency cesarean section for preeclampsia, and subsequently became anuric. She had schistocytic hemolysis, thrombocytopenia, elevated lactate dehydrogenase (LDH), and tra

Complement-targeted therapies for transplant-associated thrombotic microangiopathy: recent advances.

Transplant-associated thrombotic microangiopathy (TA-TMA) is a life-threatening complication of hematopoietic stem cell transplantation in which systemic complement activation induces endothelial injury, leading to microangiopathic hemolytic anemia, thrombocytopenia, and organ dysfunction. TA-TMA often progresses rapidly and has historically been associated with high mortality. Over the last decade, increasing recognition of complement activation as a central driver of TA-TMA has shifted management from largely empirical supportive care toward mechanism-based therapy. This review summarizes re

Safety and real-world effectiveness of eptacog beta with emicizumab prophylaxis: an interim analysis of the ATHN 16 study.

Eptacog beta, a recombinant factor VIIa bypassing agent, has demonstrated safety and efficacy in the management of bleeding events (BEs) in people with hemophilia and inhibitors. Data describing eptacog beta use in patients receiving emicizumab prophylaxis remains limited. This interim analysis of the American Thrombosis and Hemostasis Network 16 study, a phase 4, multicenter, US based, open-label study, evaluates the safety and real-world effectiveness of eptacog beta for the treatment of BEs experienced on emicizumab prophylaxis. Nineteen participants with hemophilia A and inhibitors who agr

Paroxysmal Cold Haemoglobinuria: A Chilling Case of Acute Renal Failure.

We describe a rare case of a 64-year-old man with acute renal failure secondary to paroxysmal cold haemoglobinuria (PCH), presenting with haemoglobinuria, jaundice, and fevers. PCH is an intravascular haemolytic anaemia with polyphasic IgG antibody binding of red blood cells at cold temperatures, with complement-mediated haemolysis on rewarming. Renal biopsy confirmed acute tubular necrosis without thrombotic microangiopathy. The patient received supportive management including intermittent haemodialysis with a warm circuit to prevent further haemolysis and achieved full renal recovery. Optima

Atypical Hemolytic Uremic Syndrome Presenting with Diffuse Alveolar Hemorrhage: A Rare Extra-Renal Manifestation.

We report the case of a 72-year-old male patient with chronic kidney disease (CKD) who presented with diarrhea and subsequently developed thrombotic microangiopathy (TMA) and diffuse alveolar hemorrhage (DAH). Diagnostic workup and clinical course led to a final diagnosis of atypical hemolytic uremic syndrome (aHUS). The patient responded to plasmapheresis and corticosteroid therapy. This case highlights a rare presentation of aHUS with DAH in a patient with pre-existing CKD, and the importance of prompt recognition and multidisciplinary management.

Tacrolimus-Induced Thrombotic Microangiopathy in a Kidney Transplant Recipient After Treatment for Acute Allograft Rejection.

BACKGROUND Tacrolimus is widely used in solid-organ transplantation but has been associated with thrombotic microangiopathy (TMA), a rare yet potentially life-threatening complication that can result in graft dysfunction or loss. The reported incidence ranges from 1% to 4.7% in transplant recipients. Diagnosis is challenging because clinical manifestations overlap with rejection, infection, and other secondary causes, making timely recognition critical. CASE REPORT A 48-year-old kidney transplant recipient developed anemia, thrombocytopenia, and acute kidney injury 23 days after initiating tac

Clinical Pig Organ Transplantation: What Have We Learned So Far?

Since January 2022, eleven clinical xenotransplants into living patients involving gene-edited pig hearts (n = 2) or kidneys (n = 9) have provided initial data on patient selection, organ sourcing, and immunosuppression. Lessons from cardiac cases indicate that (i) patient selection must prioritize candidates with a realistic recovery potential from preexisting debility, (ii) immunosuppressive regimens should be initiated 5-7 days pretransplant to ensure therapeutic blood levels, (iii) the administration of products that potentially contain anti-pig antibodies should be avoided, and (iv) highl

An "All-in-One" Kidney Biopsy in AIDS: Crescentic Immune Complex-Mediated Glomerulonephritis, CD8-Driven Interstitial Nephritis, and Superimposed Thrombotic Microangiopathy.

INTRODUCTION: Human immunodeficiency virus (HIV)-associated kidney disease encompasses a diverse spectrum of glomerular, tubulointerstitial, and vascular pathologies. While HIV-associated nephropathy and immune complex-mediated glomerular disease in the setting of HIV are well-recognized entities, their simultaneous presentation with thrombotic microangiopathy (TMA) and diffuse infiltrative lymphocytosis syndrome (DILS) has not been previously documented. We report a novel case of advanced HIV infection manifesting as crescentic lupus-like glomerulonephritis (GN), CD8-predominant tubulointerst

Combined Islet and Kidney Xenotransplantation for Diabetic Nephropathy: Investigation of Pre-Vascularized Composite Grafts versus Sequential Islet-After-Kidney Transplantation in a Pig-to-Nonhuman Primate Model of Xenotransplantation.

AIMS: Combined renal and islet xenotransplantation could provide a durable treatment for end-stage diabetic nephropathy. In this feasibility-focused study, we evaluated two complementary approaches for clinical translation: (1) pre-vascularized composite islet-kidney (I-K) grafts and (2) sequential islet-after-kidney xenotransplantation with vascularized thymic lobe (VTL) co-transplantation as an adjunct immune tolerance strategy. METHODS: Composite I-K grafts were generated in nine MHC-matched, minor-antigen-mismatched miniature swine pairs by implanting adult porcine islets beneath the rena

Renal toxicity during VEGF-pathway inhibition in cancer: a practical review of proteinuria, hypertension, and kidney-limited thrombotic microangiopathy.

Vascular endothelial growth factor (VEGF)-pathway inhibitors improve outcomes across solid tumors, but proteinuria, hypertension and kidney dysfunction can be difficult to translate into oncology decisions. Most abnormalities are manageable on-target toxicities; a smaller subset may signal glomerular microvascular injury or kidney-limited thrombotic microangiopathy, sometimes without overt hemolysis or thrombocytopenia. This narrative review synthesizes mechanistic, clinical, pharmacovigilance and biopsy-based evidence to support an oncology-facing approach to renal toxicity during VEGF/VEGF r

A case report of atypical hemolytic uremic syndrome with a CFH mutation complicated by recurrent posterior reversible encephalopathy syndrome.

RATIONALE: Atypical hemolytic uremic syndrome (aHUS) is a rare thrombotic microangiopathy (TMA) characterized by microangiopathic hemolytic anemia, thrombocytopenia, and organ dysfunction, particularly affecting the kidneys and the central nervous system. It is caused by genetic variants or autoantibodies involved in dysregulation of the complement system. Pathogenic variants of the complement factor H (CFH) gene are associated with severe disease and may lead to life-threatening multiorgan failure during the acute phase. We report a case of CFH mutation-positive aHUS complicated by recurrent

Multimodality imaging of myocardial injury and coronary microvascular dysfunction in congenital thrombotic thrombocytopenic purpura.

Congenital thrombotic thrombocytopenic purpura is an ultra-rare autosomal recessive disorder associated with mutations in the ADAMTS13 gene. The gene product is an enzyme responsible for cleavage of ultra-large von Willebrand factor multimers, and severe deficiency results in a thrombotic microangiopathy. We present the case of subclinical cardiac involvement characterized by myocardial fibrosis/scarring and impaired myocardial flow reserve in an affected patient who temporarily discontinued enzyme replacement therapy. This case highlights the complementary value of cardiac magnetic resonance

Update in treatment options for congenital and immune thrombotic thrombocytopenic purpura.

Thrombotic thrombocytopenic purpura (TTP) is a thrombotic microangiopathy resulting either from congenital deficiency (cTTP) or acquired (immune) deficiency (iTTP) of A Disintegrin and Metalloprotease with ThromboSpondin-type 1 motif, member 13 (ADAMTS13). Deficiency of ADAMTS13 leads to disseminated platelet thrombosis and organ dysfunction. High mortality of cTTP is prevented by plasma infusion to replace the deficient protease, or more recently by infusion of recombinant ADAMTS13. Standard treatment of iTTP includes steroids, plasma exchange, and rituximab, with or without caplacizumab. Alt

Interstitial Fibrosis and Tubular Atrophy as a Predictor of Renal Outcomes in Patients with Malignant Hypertension-Related Thrombotic Microangiopathy: A Multicentric Cohort Study.

INTRODUCTION: Interstitial fibrosis and tubular atrophy (IFTA) are common pathological findings in patients with malignant hypertension (mHTN)-related thrombotic microangiopathy (TMA), but their predictive value for renal outcomes in this population remains unclear. METHODS: This study included patients with mHTN-related TMA confirmed by renal biopsy, admitted to seven different Chinese hospitals from January 2008 to October 2024. Patients were stratified into mild/moderate, severe, and extremely severe IFTA groups based on renal biopsy findings. The primary outcome was renal replacement ther

Performance of the CellaVision advanced RBC application for schistocyte counting in the emergency clinical laboratory.

OBJECTIVES: Schistocyte quantification is essential for diagnosing microangiopathic hemolytic anemia (MAHA) and thrombotic microangiopathies (TMA). Early exclusion is critical for guiding appropriate management. However, the reference standard remains time-consuming manual microscopy, highly operator-dependent. In this study, the CellaVision Advanced RBC Application (ARBCA), an AI-based red blood cell morphology tool, was evaluated for schistocyte quantification in an emergency laboratory setting. METHODS: A total of 169 peripheral blood smears were analyzed in two tertiary-care laboratories.

Drug-Induced Thrombotic Microangiopathy in Patients with Cancer : A Clinicopathologic Series.

KEY POINTS: Gemcitabine, bevacizumab, and doxorubicin were the most common drugs implicated in drug-induced thrombotic microangiopathy in cancer. Cessation of the offending drug was the mainstay of management. The median overall survival was 2.1 years, and the median kidney failure‑free survival was 1.8 years after biopsy-confirmed diagnosis. BACKGROUND: Drug-induced thrombotic microangiopathy (TMA) is a major sequela of cancer therapies. The diagnosis of drug-induced TMA is often presumptive, making accurate clinical characterization difficult. METHODS: We retrospectively reviewed clinical

Postsurgical Thrombotic Microangiopathy after Thoracoabdominal Aortic Aneurysm Repair: A Case Report.

Postsurgical thrombotic microangiopathy (TMA) is a highly lethal complication. We report a 71-year-old woman who developed TMA following open thoracoabdominal aortic aneurysm repair. From postoperative day 2, she presented with progressive thrombocytopenia, acute kidney injury, and hemolytic anemia. Therapeutic plasma exchange (PE) and hemodialysis were promptly initiated upon clinical suspicion, without waiting for ADAMTS13 results. In conclusion, we experienced a case of postsurgical TMA triggered by profound surgical stress, in which prompt initiation of PE was effective. The patient succes
