Evaluation of the Long-Term Safety of Avacopan in Antineutrophil Cytoplasmic Antibody-Associated Vasculitis in the Real World (AvacoStar): Protocol for a Noninterventional Prospective Cohort Study.

BACKGROUND: Established treatments for granulomatosis with polyangiitis (GPA) or microscopic polyangiitis (MPA) include the use of immunosuppressive agents for remission induction followed by maintenance therapy. However, patients continue to experience disease progression, organ damage, and adverse events related to current therapies. Avacopan, an oral selective C5a receptor antagonist, was approved by the European Commission in January 2022 for the treatment of adult patients with severe, active GPA or MPA in combination with rituximab (RTX) or cyclophosphamide (CYC). In the pivotal phase 3

Small vessel vasculitis diagnosed with conchotome muscle biopsy: A case series.

A percutaneous conchotome muscle biopsy, which is a semi-open technique, provides significant diagnostic yield, especially for inflammatory myopathies. In patients with small vessel vasculitis, muscle involvement could sometimes be the sole or most prominent organ involvement. While open muscle biopsy has been reported to be useful for the diagnosis of vasculitis, the diagnostic utility with conchotome muscle biopsy for vasculitis remains unclear. Here, we report four cases in which a conchotome muscle biopsy made a significant contribution to their diagnosis of small vessel vasculitis. All ca

Association Between Serum Syndecan-1 Levels and Relapse in Patients With Antineutrophil Cytoplasmic Antibody-associated Vasculitis: A Medical Record-based Cohort Study in Japan.

BACKGROUND/OBJECTIVES: Despite previous reports on the risk factors for relapse in patients with antineutrophil cytoplasmic antibody-associated vasculitis (AAV), none have evaluated the relationship between serum syndecan-1 levels and AAV relapse. Therefore, this study aimed to investigate the association between serum syndecan-1 levels at AAV diagnosis and subsequent relapses in patients with AAV, hypothesizing that higher levels would be associated with increased risk of relapse. METHODS: This single-center, medical record-based cohort study included 60 consecutive patients with microscopic

[Vasculitides and anti-GBM disease].

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAV)-granulomatosis with polyangiitis (GPA), microscopic polyangiitis (MPA), eosinophilic granulomatosis with polyangiitis (EGPA)-as well as anti-glomerular basement membrane disease (anti-GBM disease) are systemic small-vessel vasculitides with high mortality. Diagnosis relies on serology/laboratory testing, organ screening, and exclusion of differential diagnoses. Biopsy must not delay therapy initiation in fulminant cases. In GPA and MPA, loss of T‑ and B‑cell tolerance and ANCA-mediated activation of neutrophils are pivot

Serum citrullinated histone H3 as a biomarker of disease activity and renal outcome in microscopic polyangiitis and granulomatosis with polyangiitis.

BACKGROUND: Citrullinated histone H3 (CitH3), a hallmark component of neutrophil extracellular traps, has emerged as a circulating indicator of pathological neutrophil activation. We evaluated the utility of serum CitH3 as a biomarker for disease activity and risk of end-stage kidney disease (ESKD) development in patients with microscopic polyangiitis (MPA) and granulomatosis with polyangiitis (GPA). METHODS: A total of 65 patients with MPA and GPA were included. At diagnosis, serum CitH3 levels were measured and disease activity was assessed using the Birmingham Vasculitis Activity Score (BVA

Long-term outcomes remain unchanged despite reduced glucocorticoid exposure in ANCA-associated vasculitis: the multicentre REVEAL cohort study.

BACKGROUND: The long-term prognosis of patients with anti-neutrophil cytoplasmic antibody-associated vasculitis (AAV) has historically been poor. In recent years, the widespread use of novel targeted therapies has led to an increased emphasis on reduced-dose glucocorticoid (GC) strategies; however, owing to regional differences in the frequency and clinical characteristics of AAV subtypes, contemporary real-world evidence on long-term outcomes in Japanese patient populations remains limited. OBJECTIVES: The primary outcome was to elucidate subtype-specific clinical characteristics and five-ye

Gut microbiota dysbiosis exacerbates microscopic polyangiitis via toll-like receptor 7.

OBJECTIVES: Anti-neutrophil cytoplasmic antibody-associated vasculitis (AAV) is closely associated with factors such as genetic predisposition, environmental pollution, medications, and infections. Some bacteria have been definitively linked to AAV. Although gut microbiota dysbiosis has been confirmed in individuals with AAV, the specific role of this dysbiosis and the associated mechanisms contributing to AAV pathogenesis remain unexplored. The most common type of AAV in China is microscopic polyangiitis (MPA). METHODS: We used propylthiouracil/phorbol 12-myristate 13-acetate (PMA/PTU) to es

A Diagnostic Tightrope: Carbimazole-Induced Microscopic Polyangiitis With Co-Existent Antiphospholipid Syndrome and Diffuse Alveolar Hemorrhage.

Microscopic polyangiitis (MPA) and drug-induced vasculitis represent significant diagnostic challenges. Carbimazole-induced antineutrophil cytoplasmic antibodies (ANCA)-associated vasculitis is a rare, life-threatening complication of thionamide therapy. We describe a 29-year-old female patient on long-term carbimazole therapy for 15 years who presented with acute type-1 respiratory failure due to diffuse alveolar hemorrhage. Investigations confirmed myeloperoxidase (MPO)-ANCA-positive microscopic polyangiitis with co-existent definitive antiphospholipid syndrome and atypical "immune-complex-m

Hydralazine-Induced Antineutrophil Cytoplasmic Antibody Vasculitis Causing Gastroduodenal Artery Perforation.

Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitides are rare disorders characterized by inflammation of small blood vessels, including capillaries, venules, and arterioles. The three main subtypes of primary ANCA-associated vasculitides (i.e., granulomatosis with polyangiitis, microscopic polyangiitis, and eosinophilic granulomatosis with polyangiitis) can present with diverse and nonspecific symptoms, making diagnosis difficult. We report the case of secondary, drug-induced ANCA vasculitis in a female patient who presented with hemoptysis and dyspnea complicated by acute kidney

Genetic burden across core genes of the PI3K-AKT-mTOR pathway is associated with susceptibility to microscopic polyangiitis: a Chinese cohort study.

BACKGROUND: The PI3K-AKT-mTOR signaling pathway plays a central role in immune regulation and has been implicated in autoimmune diseases. However, the contribution of genetic variation within key components of this pathway to microscopic polyangiitis (MPA) remains incompletely understood. METHODS: We conducted a genetic association study in a Chinese cohort including MPA patients and controls. Four single nucleotide polymorphisms (SNPs) within core genes of the PI3K-AKT-mTOR pathway (PIK3CA, AKT1, and MTOR) were analyzed. A cumulative genetic burden score was constructed by summing the number

ANCA vasculitis: what is in the pipeline.

PURPOSE OF REVIEW: Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is now recognized as a complex immune-mediated inflammatory disorder rather than a purely pauci-immune small-vessel vasculitis. Advances in understanding ANCA specificity, complement activation, neutrophil extracellular trap formation, genetic susceptibility, and environmental triggers have reshaped concepts of disease classification, prognosis, and therapy. A focused review of these developments is timely because they are directly informing current treatment strategies, disease stratification, and futur

Successful reintegration of a very elderly patient with rapidly progressive glomerulonephritis due to microscopic polyangiitis after multidisciplinary treatment: a case report.

ANCA (anti-neutrophil cytoplasmic antibody)-associated vasculitis (AAV) is known to cause rapidly progressive renal dysfunction, as well as high mortality, especially in older patients. In Japan, a high prevalence of microscopic polyangiitis (MPA) with myeloperoxidase (MPO)-ANCA among older patients has been reported; however, standard immunosuppressive therapy does not always lead to improved outcomes in this population. Here we report the case of a 93-year-old woman who developed rapidly progressive glomerulonephritis (RPGN) due to MPA and successfully returned home after multidisciplinary t

Microscopic polyangiitis-associated interstitial lung disease with acute exacerbation after SARS-CoV-2 infection: a case report and literature review.

Microscopic polyangiitis (MPA) is a type of systemic inflammatory small vessel vasculitis that is frequently accompanied by interstitial lung disease (ILD). Acute exacerbations (AE) of ILD are fatal complications in patients with MPA and can be triggered by various factors, including infections. We report a case of AE of MPA-ILD following severe acute respiratory syndrome Coronavirus 2 (SARS-CoV-2) infection. An 81-year-old male with MPA during remission maintenance therapy was hospitalised for Coronavirus disease 2019 pneumonia. Treatment for SARS-CoV-2 infection improved his respiratory symp

Systemic sclerosis with concurrent ANCA-positive IgA nephropathy successfully managed with rituximab and mycophenolate mofetil: a case report.

Systemic sclerosis (SSc) is an autoimmune disease characterized by vasculopathy and fibrosis, whereas antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a small-vessel vasculitis that frequently involves the kidneys and lungs. Although an overlap between SSc and AAV has been reported, ANCA-positive IgA nephropathy (IgAN) in SSc has not been described. Clinically, ANCA-positive IgAN resembles AAV, posing challenges for disease classification and management. However, the optimal treatment strategy for such overlapping cases has not yet been established. We report the case
