STAT4 gene polymorphism may be associated with microscopic polyangiitis susceptibility in a Chinese Guangxi population: A case-control analysis based on propensity score matching.

BACKGROUND: Microscopic polyangiitis (MPA) is a severe multisystem autoimmune disease featured by small-vessel vasculitis with few or no immune complex, also has a significant genetic predisposition. Growing evidence has confirmed that STAT4 gene is tightly associated with multiple autoimmune diseases, but its contribution to MPA onset is still elusive. OBJECTIVE: The aim was to investigated the association between STAT4 gene polymorphisms (rs7572482, rs7574865 and rs12991409) and MPA susceptibility in a Guangxi population of China. METHODS: 260 MPA patients and 295 healthy adult volunteers

Proteinase 3 (PR3)-Positive Perinuclear Anti-neutrophil Cytoplasmic Antibodies (pANCA) Vasculitis With Concurrent Invasive Aspergillosis Infection.

Anti-neutrophil cytoplasmic antibodies (ANCA)-associated vasculitides are a group of rare diseases that cause inflammation of small to medium vessels. They comprise granulomatosis with polyangiitis (GPA), microscopic polyangiitis (MPA), and eosinophilic granulomatosis with polyangiitis (EGPA). As the signs and symptoms are varied, the diagnosis of vasculitis is complex and challenging; however, there are type-specific clinical presentations that can be factored into the existing classification criteria. The difficulty faced in diagnosis is compounded due to ANCA being present in other chronic

Real-life use of the PEXIVAS reduced-dose glucocorticoid regimen in granulomatosis with polyangiitis and microscopic polyangiitis.

BACKGROUND: The PEXIVAS (Plasma exchange and glucocorticoids in severe antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis) trial showed that a reduced-dose glucocorticoid regimen (redGC) was non-inferior to a standard-dose regimen (standGC) with respect to death or end-stage kidney disease (ESKD) in patients with ANCA-associated vasculitis (AAV). However, the primary endpoint did not include disease progression or relapse, cyclophosphamide was the main induction therapy and rituximab (RTX)-treated patients tended to have a higher risk of death or ESKD with redGC. We aimed to eval

Case report: Spontaneous renal hemorrhage in anti-neutrophil cytoplasmic antibody-associated vasculitis.

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a systemic necrotizing vasculitis that predominantly affects small vessels. In this report, we present a typical case of granulomatosis with polyangiitis (GPA) complicated by spontaneous renal hemorrhage (SRH), a rare but potentially severe condition. The patient developed SRH during immunosuppressive therapy but recovered following conservative treatment. We then conducted a systematic literature review on SRH in the context of AAV, and analyzed clinical features, management strategies, and patient prognosis. A total o

Microscopic polyangiitis With Pauci-Immune Glomerulonephritis Associated With Gastrointestinal Tuberculosis.

Introduction: Tuberculosis (TB) is a prevalent disease in Guatemala, present in 20-25 cases per 100 thousand inhabitants. Extrapulmonary TB (EPTB) accounts for only 10%-17% of TB cases. The diagnosis of EPTB is challenging, especially in low-resource settings, because TB can present with clinical characteristics of rheumatological, oncological, or other infectious diseases. Occasionally, mycobacterial infection stimulates the immune system, inducing the generation of antibodies that may lead to autoimmune diseases secondary to primary TB infection, such as vasculitis. To the best of our knowle

Interstitial lung disease, antineutrophil cystoplasmic antibodies and microscopic polyangiitis.

This mini review explores the association of interstitial lung disease (ILD) with antineutrophil cystoplasmic antibodies (ANCA) and the clinical syndrome of microscopic polyangiitis (MPA). Reports on radiographic and histopathologic findings as well as genetic predispositions are reviewed. Based on this evidence a concept for the pathogenesis of the relationship of ILD, MPO-ANCA and MPA is proposed. Finally, a practical clinical management approach to patients presenting either with ILD and a positive ANCA test result, or to patients with MPA found to have pulmonary abnormalities qualifying as

Current Diagnosis and Treatment of Anti-Neutrophil Cytoplasmic Antibody-Associated Vasculitis: A Review Including a Comparison of Characteristics in Europe and Japan.

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a disease entity characterized by systemic vasculitis positive for ANCAs, which often leads to severe organ damage such as diffuse bronchoalveolar hemorrhage and rapidly progressive glomerulonephritis. It is known that the incidence and characteristics of AAV vary depending on region, and differences in the peak age of onset, the ratio of positive rates of MPO-ANCA to PR3-ANCA, and occurrence rates of GPA and MPA may have resulted in different approaches to clinical practice. It may also be necessary to modify therapeut

Clinical Characteristics of Anti-Neutrophil Cytoplasmic Antibody-Associated Vasculitis Patients Proven by Muscle Biopsy and Its Relevance to Kidney Involvement.

Muscle symptoms including myalgia are common in patients with anti-neutrophil cytoplasmic antibody-associated vasculitis (AAV). Muscle biopsy (MB) is sometimes used in the diagnosis of AAV, but which AAV patients benefit from MB remains unclear. We retrospectively assessed the clinical characteristics of Japanese AAV patients who underwent MB and examined the relationship between MB showing positive results for muscular vasculitis and kidney involvement. We enrolled all 38 of the 118 patients with AAV who also underwent MB. Clinical characteristics and histopathological findings on kidney biop

Urinary Kim-1 Correlates with Interstitial Nephritis Activity in Patients with Microscopic Polyangiitis.

BACKGROUND: Microscopic polyangiitis (MPA) is a type of necrotizing vasculitis that primarily affects small vessels and belongs to the spectrum of anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAVs). While previous studies have identified potential prognostic biomarkers, further research is needed to validate a reliable marker for risk stratification in clinical practice. Kidney injury molecule-1 (Kim-1), a transmembrane protein expressed on proximal tubular epithelial cells, has been implicated in tubular damage. This study investigated the potential of Kim-1 as a bioma

Thoracic Manifestations of ANCA-associated Vasculitis: Review of the 2022 American College of Rheumatology-European Alliance of Associations of Rheumatology Classification Criteria.

Antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis (AAV) is a rare disease that manifests as necrotizing vasculitis involving small vessels without immune complex deposition. Granulomatosis with polyangiitis (GPA), eosinophilic granulomatosis with polyangiitis (EGPA), and microscopic polyangiitis (MPA) are included in this disease entity. Diagnosis and differentiation of AAV is challenging because of the diverse and overlapping clinical manifestations and lack of pathognomonic findings. Therefore, AAV classification criteria have been developed to increase the likelihood of d

BACKGROUND: Antineutrophil cytoplasmic antibody (ANCA) is an autoantibody against the cytoplasmic components of neutrophils and monocytes. ANCA related vasculitis includes granulomatous polyangiitis (GPA), microscopic polyangiitis (MPA), and eosinophilic granulomatous polyangiitis (EGPA). The detection of ANCA has important clinical significance for the diagnosis, differential diagnosis, classification, condition monitoring, and prognosis of these diseases. The treatment and prognosis of ANCA associated vasculitis are closely related to the titer and activity of ANCA, so the detection of ANCA
