In this overview the characteristics have been defined of those patients with acute myocardial infarction to whom early thrombolytic therapy would offer a major benefit. However, this concerns only the 20% of all patients who are admitted to the hospital with acute myocardial infarction within the specified time limit. It is still unclear whether later thrombolytic therapy with the newer agents might offer benefit to other patients with acute myocardial infarction. Presently, it seems unlikely that patients admitted to the hospital more than six hours after onset of symptoms will benefit from

The acute effects of earthquake-related psychological stress on total and cardiac mortality were investigated using mortality data from the city of Thessaloniki (Greece) which was hit on 19 and 20 June 1978, by two strong earthquakes, measuring 5.2 and 6.4 degrees on the Richter scale, respectively. During the a priori specified three-day period of 19, 20 and 21 June, mortality from cardiac and 'all other' pathological causes increased significantly. The estimated relative risks (and 95% confidence intervals) were 3.0 (1.5-5.9) for atherosclerotic heart disease, and 1.6 (1.1-2.3) for 'all othe

Left ventricular (LV) ejection fraction (EF) is known to be related to prognosis after acute myocardial infarction (AMI), but its role alone and in combination with other factors in the definition of a high-risk group has not been adequately specified. Several recent multicenter studies emphasize that LVEF together with features of ventricular ectopic activity during ambulatory electrocardiography define a group at high risk for death for up to 3 years. However, these high-risk groups comprised only a small fraction of the population (less than 7.5%) and failed to include 75% or more (less tha

By 1979, mortality rates for coronary heart disease had declined for 14 consecutive years in the United States. Preliminary data indicate a continued decline. This review of reports published in the last five years documents the consistency of most data with the following hypotheses: (1) Reductions in population levels of hypertension and cigarette smoking have contributed to the decline in mortality from coronary heart disease. (2) Improved medical care for acute myocardial infarction has also contributed to the decline. Data are lacking to specify the contributions of changes in other risk f

Electrocardiographic criteria have been derived from vectorcardiographic and angiographic correlation which allow division of patients with electrocardiographic "poor R-wave progression" or "reversed R-wave progression" into the following four etiologic groups: (1) anterior myocardial infarction; (2) left ventricular hypertrophy; (3) type-C right ventricular hypertrophy; and (4) the normal variant. The sensitivity, specificity, and predictive value of this approach to the electrocardiogram with poor or reversed R-wave progression were studied in a series of 33 patients examined at autopsy. Usi

Transitory ischemic accident is an episode that commences abruptly with subjective or objective neurological disturbances, persists for a brief time, and returns to normal within 24 hours of onset with complete recuperation of neurological function. Using this as a basic definition, the authors specify the characteristic symptoms of different types of transitory ischemic accident and review in retrospect 150 cases of ischemic infarction in the region of the median cerebral artery. Thirty-eight percent of the patients suffered transitory ischemic accidents prior to stroke. The symptoms included

Clinical, angiological, and biochemical examinations were performed in 981 men and 30 women with organic afflictions of peripheral arteries, and in 411 men and 50 women without any signs of peripheral arterial lesions. Their family histories were thoroughly recorded with particular reference to the occurrence of myocardial infarction, cerebral accidents before and after the age of 60 years, and death of these causes; further, of hypertension, diabetes mellitus, obliterations and gangraenes, in each patient's siblings, parents, and all four grandparents. Furthermore, the significance of positiv

Electromyographic biofeedback is becoming widely used to help patients regain voluntary control of specific muscles affected by neuromuscular disorders. Electromyographic feedback training has been employed in the rehabilitation of patients affected by poliomyelitis, cerebrovascular accident, torticollis, nerve injury, temporomandibular joint syndrome, bruxism, and other disorders. While EMG biofeedback appears to be a promising treatment technique, the research literature on its effectiveness consists mainly of uncontrolled case reports and clinical trials. It is concluded that new studies wi

The constant progress in surgical techniques during the last few years have prepared the way for important developments in the field of noninvasive cardiac exploration. Their non-traumatic character, appreciable in diagnostic and preoperative examinations, become indipensable whenever there is a need to repeat the examination in order to evaluate the effects of a treatment or to monitor progress both in the short and long term. In order that such methods be adopted in cardiac surgery it is important that not only must they be without risk but also that the results obtained be clearly presented
