Ultrasensitive serum levels of interleukin 13 in systemic lupus erythematosus patients: a cross-sectional study.

BACKGROUND: Systemic lupus erythematosus (SLE) is characterized by elevated levels of various cytokines. Interleukin (IL)-13 is a pleiotropic type 2 cytokine that plays a central role in allergic inflammation and tissue fibrosis. Until recently, serum measurement of IL-13 was not feasible because of its extremely low concentration. New highly sensitive techniques now allow its quantification at the femtogram level. In the present study, we aimed to evaluate the association between SLE-disease characteristics and ultrasensitive serum IL-13 levels. METHODS: In this cross-sectional study, a comp

Isotype-specific autoantibody profiles associated with clinical immunological signatures in IgG4-RD.

OBJECTIVES: IgG4-related disease (IgG4-RD) involves elevated serum IgG4 and several autoantigens, but the role of different antibody isotypes in pathogenesis remains unclear. This study systematically characterized isotype‑specific autoantibody responses against representative autoantigens and evaluated their clinical and immunological correlations. METHODS: We enrolled 61 patients with IgG4‑RD, 31 with systemic lupus erythematosus (SLE), 11 with Sjögren's syndrome (SS), and 30 healthy controls (HC). Serum levels of isotype‑specific autoantibodies were measured using in‑house ELISA. Correlati

Efficacy of nutraceuticals as adjunctive therapies in autoimmune diseases: A systematic review of clinical trials.

INTRODUCTION: Autoimmune diseases are characterized by a dysregulation of the immune system, resulting in chronic inflammation and tissue damage. Although immunosuppressive therapies have advanced, their limited efficacy and the adverse effects associated with long-term use restrict their clinical utility, highlighting the need for alternative treatments. This study aimed to synthesize current evidence on the efficacy of nutraceutical interventions for these disorders. METHODS: A comprehensive search was conducted on PubMed, Embase, and ClinicalTrials.gov to identify studies published between

Endoplasmic reticulum stress in autoimmune skin diseases: immune activation, cutaneous cell damage, and therapeutic advances.

Endoplasmic reticulum stress (ERS) arises when the protein-folding load, or perturbations in calcium and redox homeostasis, outpace the adaptive capacity of the endoplasmic reticulum (ER). The unfolded protein response (UPR) initially reduces secretory load and restores proteostasis. Persistent stress can redirect the same pathways toward inflammatory signaling, altered antigen processing, and potentially immunogenic cell death. We term this context-dependent transition ERS-immune tolerance breaking, and propose it as a unifying framework for understanding how unresolved ERS may contribute to

IgG N-glycosylation in autoimmune diseases.

IgG is the most abundant antibody in the human circulation. It carries a single conserved glycosylation site, Asparagine 297, in its Fc domain. This N-glycan is critical for the ability of the antibody to activate immune cells via binding to, and cross-linking Fc receptors in order to elicit functional responses. First documented in the context of rheumatoid arthritis (RA), the IgG N-glycan may undergo changes that are correlated to host health. We present a narrative in-depth comparison of changes incurred by IgG N-glycans in autoimmune diseases; RA, systemic lupus erythematosus, multiple scl

RNA therapeutics in rheumatoid arthritis, systemic lupus erythematosus, and systemic sclerosis: a compartment-aware translational framework.

Rheumatoid arthritis (RA), systemic lupus erythematosus/lupus nephritis (SLE/LN), and systemic sclerosis (SSc) remain major causes of chronic inflammation, disability, organ damage, and treatment burden despite therapeutic advances. RNA therapeutics offer programmable transcript suppression, network-level microRNA modulation, transient restoration of protective proteins, and targeting-ligand functions. Existing reviews largely catalogue modalities or individual delivery systems; this review instead asks whether a candidate forms an interpretable translational program across target biology, dis

Sodium-glucose cotransporter 2 inhibitors in patients with systemic lupus erythematosus and lupus nephritis: A systematic review and meta-analysis.

IntroductionSystemic Lupus Erythematosus (SLE) and lupus nephritis (LN) carry substantial cardiorenal risks. Conventional immunosuppressive therapies primarily target inflammation but leave a burden of residual renal and metabolic damage. Sodium-glucose cotransporter 2 inhibitors (SGLT2i) provide clear cardiorenal protection in general populations, but data specific to SLE remains limited. This systematic review and meta-analysis evaluated the efficacy and safety of SGLT2i in patients with SLE and LN.MethodsComprehensive searches across five electronic databases (PubMed, Cochrane CENTRAL, Web

The ups and downs of human UNC93B1: Autoimmune and viral conditions.

Mouse Unc93b1 was identified in 2006 by forward genetics as an ER membrane protein essential for the activity of the endosomal nucleic acid-sensing receptors TLR3, TLR7, and TLR9. In vitro studies in mouse and human cells have revealed its cellular functions, including ER-based stabilization and trafficking of endosomal TLR proteins (TLR3, TLR7, TLR8, and TLR9) and regulation of signaling thresholds through competitive trafficking control (TLR7 and TLR9). Human genetic studies have clarified the physiological role of UNC93B1 in natural conditions. Biallelic loss-of-function UNC93B1 mutations i

Clinical characteristics of adult patients with autoimmune rheumatic diseases coexisting with persistent isolated low IgM: a retrospective study in China.

OBJECTIVE: Selective IgM deficiency (sIgMD) is an uncommon immunological phenotype characterized by persistently reduced serum IgM, but its characteristics within autoimmune rheumatic diseases (ARDs) remain poorly defined, particularly in Chinese populations. This study aimed to characterize a persistent isolated low IgM phenotype compatible with sIgMD from an ARD-centered perspective, focusing on its clinical features, and infection spectrum in a cohort of 257 patients. METHODS: A retrospective study of adult inpatients diagnosed with both ARDs and persistent isolated low IgM was conducted.

Renal-Limited Thrombotic Microangiopathy Presenting as Acute on Chronic Kidney Failure in the Absence of Schistocytosis.

We report a rare presentation of renal-limited thrombotic microangiopathy (TMA). TMA is a clinicopathologic syndrome resulting from endothelial injury and activation of the coagulation and platelet pathways, leading to the formation of platelet-rich microthrombi within small vessels. This loss of blood flow due to the thrombi can result in tissue ischemia and end-organ injury. Systemic TMA classically manifests with the triad of microangiopathic hemolytic anemia (MAHA), thrombocytopenia, and organ dysfunction. However, TMA may be limited predominantly to the kidneys, resulting in acute or prog

Chimeric Antigen Receptor T-cell (CAR-T) Therapy in Autoimmune Diseases: A Narrative Review.

Autoimmune diseases are chronic immune-mediated disorders that, in their systemic forms, such as systemic lupus erythematosus (SLE), systemic sclerosis (SSc), idiopathic inflammatory myopathies, and antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV), remain associated with progressive organ damage, impaired quality of life, and excess mortality despite conventional immunosuppression. A substantial proportion of patients experience refractory disease, recurrent flares, and glucocorticoid dependence, underscoring the need for therapies that address the underlying disease proc

Mixed Connective Tissue Disease (MCTD): A Rare Presentation of Probable Central Nervous System (CNS) Vasculitis.

Mixed connective tissue disease (MCTD) may rarely involve the central nervous system (CNS), and CNS vasculitis can present with acute seizures, altered consciousness, and focal neurological deficits. We report a 27-year-old woman with a seven-year history of arthralgia, morning stiffness, intermittent fever, and malaise who developed acute seizures, altered sensorium, and hemiparesis approximately two weeks before hospital admission. Laboratory evaluation demonstrated severe anemia, thrombocytopenia, elevated inflammatory markers, and positive anti-U1 ribonucleoprotein (U1-RNP) antibodies. Cer

Single-cell mass cytometry cartography highlights systemic lupus erythematosus and Sjögren's disease immune differences.

OBJECTIVES: Autoimmune diseases such as primary Sjögren's disease (pSjD) and systemic lupus erythematosus (SLE) share clinical and serological features, including type I interferon signatures, autoantibody formation, and chronic inflammation, but the underlying immunopathology is not the same. To support the development of targeted therapies, we aimed to systematically characterize disease-specific alterations in peripheral immune cell subsets of pSjD and SLE compared to healthy donors (HD). METHODS: We analyzed peripheral blood mononuclear cells from 15 HD, 12 pSjD and 21 SLE patients by mas

Translating the 2024 ACR lupus nephritis guidelines to pediatric practice.

Lupus nephritis (LN) is more common and often more aggressive in children than in adults, yet most randomized trials informing current LN management have enrolled adults. The 2024 American College of Rheumatology (ACR) guideline is the first ACR guideline on lupus nephritis to explicitly state that its recommendations apply to both adults and children, although its evidence base remains largely adult-derived. This review uses the 2024 ACR guideline as an organizing framework, cross-referenced with the 2024 KDIGO and 2025 EULAR recommendations, to examine which aspects of LN management translat
