Clinically validated assay for rapid determination of type I and type II interferon activity in systemic inflammatory diseases.

BACKGROUND: Type I interferons (IFN-I) and type II interferon (IFN-γ) contribute to the pathogenesis of inflammatory diseases, but measurement of these cytokines remains challenging in the clinical setting. OBJECTIVE: We aimed to develop a clinical test that rapidly captures the activity of IFN-I and IFN-γ. METHODS: We compared RNA sequencing data from healthy controls and patients with macrophage activation syndrome, multisystem inflammatory syndrome in children, and systemic lupus erythematosus to evaluate biomarkers of IFN-I and IFN-γ signaling. We utilized a flow cytometry assay to measu

Transition From Juvenile Dermatomyositis to Spondyloarthritis: A Novel Overlapping Inflammatory Phenotype.

OBJECTIVE: Juvenile dermatomyositis (JDM) is a rare childhood inflammatory myopathy, whereas spondyloarthritis (SpA) is an inflammatory arthropathy characterized by enthesitis and peripheral or axial involvement. We describe a series of patients diagnosed with JDM in childhood who later fulfilled classification criteria for SpA, a sequential phenotype that has not been well characterized. METHODS: This institutional review board-exempt retrospective study (2024-180) reviewed demographics, clinical features, laboratory tests, imaging, treatments, and outcomes in a convenience sample of patient

Distinct by Design: Unraveling the Unique Clinical and Transcriptomic Identity of Juvenile Scleromyositis Overlap Compared to Juvenile Systemic Sclerosis and Juvenile Dermatomyositis: Implications for Care and Pathogenesis.

OBJECTIVE: To characterize clinical and transcriptomic differences in juvenile scleromyositis overlap (jOverlap) compared to juvenile systemic sclerosis (jSSc) and juvenile dermatomyositis (JDM), focusing on autoantibody profiles, organ involvement, treatment, and peripheral blood gene expression. METHODS: Peripheral blood bulk RNA sequencing was performed on children with jSSc (n = 25), JDM (n = 25), and jOverlap (n = 26), and healthy controls (HCs; n = 21) from two tertiary referral centers. Disease category was assigned by treating physicians. Clinical data and autoantibody profiles were c

Nailfold video capillaroscopy as a non-invasive biomarker in juvenile dermatomyositis: A longitudinal analysis of microvascular changes and clinical relevance over one year.

OBJECTIVE: Juvenile dermatomyositis (JDM) is a rare, immune-mediated vasculopathy characterized by muscle inflammation and cutaneous involvement. This study aimed to evaluate longitudinal changes in nailfold video capillaroscopy (NVC) findings and their associations with clinical disease activity and serologic markers in JDM patients over one year. METHODS: A prospective, observational study was conducted at two tertiary pediatric rheumatology clinics. Twenty children diagnosed with JDM were enrolled and followed for one year with clinical evaluations and NVC assessments performed at four-mon

Comprehensive Analysis Reveals Potential Molecular Targets in Juvenile Dermatomyositis.

Juvenile dermatomyositis (JDM) is a rare autoimmune disease primarily affecting children, characterized by muscle weakness and skin lesions. This study identifies 145 genes significantly associated with JDM through differential gene expression analysis, weighted gene coexpression network analysis (WGCNA), protein-protein interaction network analysis, and miRNA and transcription factor (TF) prediction, using blood and muscle microarray sequencing datasets. Functional enrichment analysis indicates that these genes are involved in crucial biological processes, including cytokine-mediated signalin

Immunomodulation in paediatric inflammatory interstitial lung diseases: towards mechanism-based targeted therapies, a focus on the anti-cytokines.

Childhood interstitial lung diseases (chILDs) associated with inflammatory diseases represent a heterogeneous group of rare conditions. To date, therapeutic approaches predominantly rely on broad-spectrum immunosuppressive agents, often independently of the underlying mechanism. During the last ten years, next generation sequencing techniques allowed the identification of monogenic auto-inflammatory diseases with the subsequent development of mechanism-based targeted immunomodulatory therapies. Among these emerging therapies, anti-cytokine agents occupy an increasingly prominent role in inflam

CAR T-cell therapy and indications for pediatric rheumatology: Structured overview of applications, risks, and considerations.

This chapter offers an overview of CAR T-cell therapy and its emerging role in pediatric rheumatology, particularly the potential impact on severe refractory autoimmune conditions in children. It examines criteria for patient eligibility, safety concerns, and potential long-term effects for young rheumatology patients, while stressing the importance of personalized and prolonged follow-up. The possibility of achieving drug-free remission in pediatric cases appears to be a possible in refractory patients; however, further studies should focus on standardizing pre-treatment protocols for childre

A test of a conceptual model of uncertainty, benefit and burden in children and young people with juvenile dermatomyositis.

OBJECTIVES: Mental health concerns are highly prevalent in children and young people (CYP) with chronic conditions. This is further exacerbated by perceived feelings of illness uncertainty and illness burden, which have negative implications for CYP with childhood-onset chronic conditions and their families. Understanding the mechanisms that reduce health-related quality of life (HRQoL) and identifying resilience factors is important to improve outcomes for these vulnerable populations. The current study aimed to assess HRQoL in CYP with juvenile dermatomyositis (JDM) and to test a new concept

Imaging Findings of Pediatric Rheumatic Disorders: JIA in the PRINTO Era and Autoimmune Interferonopathies.

Treatment strategies for pediatric rheumatic diseases have changed substantially over the past two decades, driven by the development of biologics and cytokine-targeted molecular therapy. Therapeutic approaches that modulate both innate and adaptive immune responses have improved prognosis in these immune-mediated disorders, and early diagnosis with timely intervention is associated with better outcomes. However, affected children often present with non-specific symptoms, and reliable biomarkers remain limited in juvenile idiopathic arthritis (JIA); thus, the diagnostic delay remains common. I

Comorbidities in Juvenile-Onset Rheumatic Diseases: A Systematic Review and Meta-Analysis.

Background/Objectives: Patients with childhood-onset rheumatic diseases may be at additional risk of developing other health conditions. This systematic review aimed to (i) identify and describe the comorbidities associated with three significant childhood-onset rheumatic diseases-Juvenile Idiopathic Arthritis (JIA), Juvenile-onset Systemic Lupus Erythematosus (jSLE), and Juvenile Dermatomyositis (JDM); (ii) describe comorbidity prevalence and incidence reported as apparent in childhood or adulthood, and (iii) compare these comorbidity estimates with control groups. Methods: PubMed, Web of Sci

A systematic review of juvenile dermatomyositis complicated by macrophage activation syndrome.

BACKGROUND: Macrophage activation syndrome (MAS) is a rare but potentially life-threatening complication of juvenile dermatomyositis (JDM). Data on its clinical presentation, diagnosis, treatment, and outcomes remain limited. OBJECTIVE: To systematically review the literature on MAS occurring in patients with JDM. METHODS: A systematic search of PubMed and Embase was performed for studies published between 1997 and 2025 reporting JDM-associated MAS/hemophagocytic lymphohistiocytosis. Three reviewers independently screened studies according to PRISMA guidelines. Due to marked heterogeneity an
