Physical Growth Dynamics of Children With Juvenile Dermatomyositis.

Objective Growth failure is an often overlooked complication of juvenile dermatomyositis (JDMS) hence, the present study analyses the auxological status of adolescent JDMS patients. Methods Sixty serial observations with respect to height, weight, and skinfold thicknesses were made on 35 JDMS children aged 10 to 17 years in the Growth Laboratory/Clinic of the Department following a mixed-longitudinal growth research design. Information with regard to disease duration and disease activity (MMT-8 score) was recorded. Results The mean height and weight of JDMS patients showed a regular increase f

Imaging assessment of calcinosis in juvenile dermatomyositis: a narrative review.

Calcinosis is a severe manifestation of juvenile and adult idiopathic inflammatory myopathies, which can lead to pain, limited range of motion, disfigurement, and infection. It is more common in juvenile idiopathic inflammatory myopathies, especially in juvenile dermatomyositis (JDM). Calcinosis can be visible on cutaneous surfaces, although can also present in muscles and internal organs, making a thorough assessment difficult without imaging modalities. In this narrative review, we discuss different imaging modalities used in evaluating JDM-associated calcinosis including X-ray, computed tom

Juvenile Dermatomyositis Triggered by Influenza B: A Case Report on Viral-Induced Autoimmunity.

A previously healthy 13-year-old boy developed juvenile dermatomyositis (JDM) shortly after a confirmed influenza B infection, presenting with progressive proximal muscle weakness and classic cutaneous findings. Laboratory tests revealed elevated muscle enzymes and myositis-specific autoantibodies, supporting the diagnosis. The temporal association suggests a potential post-viral autoimmune trigger, highlighting influenza B as a possible environmental factor in JDM pathogenesis. This case reinforces the need for heightened clinical awareness and further research into virus-associated autoimmun

Targeting interferon responses in juvenile dermatomyositis: Siglec-1 as an in vitro biomarker for JAK inhibitor efficacy.

OBJECTIVES: For IFN-driven diseases, such as juvenile dermatomyositis (JDM), there is a critical need for targeted therapies. We aimed to develop an in vitro model, using Siglec-1 as read-out, to evaluate inhibition of IFN-mediated responses with different JAK inhibitors (JAKi). METHODS: Healthy donor (HD) PBMCs were cultured with type I and II IFNs, TLR agonists and plasma or serum from patients (JDM, DM, JSLE, COVID-19) and HDs. Siglec-1 expression on CD14+ monocytes was analyzed using flow cytometry. Inhibitory assays involved pre-incubation with JAKi (filgotinib, tofacitinib, baricitinib,

Why Recommended Visit Intervals Should Be Extracted When Conducting Longitudinal Analyses Using Electronic Health Record Data: Examining Visit Mechanism and Sensitivity to Assessment Not at Random.

Electronic health records (EHRs) provide an efficient approach to generating rich longitudinal datasets. However, since patients visit as needed, the assessment times are typically irregular and may be related to the patient's health. Failing to account for this informative assessment process could result in biased estimates of the disease course. In this paper, we show how estimation of the disease trajectory can be enhanced by leveraging an underutilized piece of information that is often in the patient's EHR: physician-recommended intervals between visits. Specifically, we demonstrate how r

Exploring the Association of Sociodemographic Factors and Primary Diagnosis With Transition Readiness in Adolescents With Rheumatic Disease.

OBJECTIVE: Transitioning from pediatric to adult care is challenging for adolescents with chronic health conditions. The Transition Readiness Assessment Questionnaire (TRAQ) is a validated tool for measuring transition readiness in pediatric patients with chronic diseases. This study examines the association of sociodemographic factors and primary diagnosis with transition readiness in adolescents with rheumatic disease using TRAQ scores. METHODS: We conducted a retrospective chart review of 882 adolescents with rheumatic diseases, aged 14 to 19 years, from September 2019 to December 2021. TR

Tofacitinib monotherapy as maintenance treatment in juvenile dermatomyositis: a case report.

Juvenile dermatomyositis (JDM) is a multisystem autoimmune disease associated with considerable morbidity and mortality, having a significant impact on patient health. Early diagnosis and appropriate treatment play a key role in optimising outcomes. JDM management remains challenging, particularly in severe or refractory cases, due to the limited evidence regarding therapeutic options. Given these challenges, Janus kinase inhibitors, including tofacitinib, have emerged as promising alternatives, yet evidence in paediatric populations is still scarce. We present the case of a child with JDM who

Treating juvenile dermatomyositis to target: Paediatric Rheumatology European Society/Childhood Arthritis and Rheumatology Research Alliance-endorsed recommendations from an international task force.

OBJECTIVES: Despite the recent prognostic improvement, a sizeable proportion of patients with juvenile dermatomyositis (JDM) respond suboptimally to contemporary therapies. This study aimed to develop recommendations for treating JDM to target. METHODS: A Steering Committee formulated a set of provisional recommendations based on evidence derived from a systematic literature review and a retrospective chart review of patients. These were discussed, amended, and voted on by an international Task Force, including 28 paediatric rheumatologists, 2 specialists in neuromuscular diseases, 1 dermatol

Rheumatologic manifestations in children with underlying inborn errors of immunity.

BACKGROUND AND OBJECTIVE: In recent years, many studies have been conducted on the possible link between rheumatologic diseases and inborn errors of immunity. Rheumatologic diseases may occur as manifestations of an underlying immunodeficiency disorder, and may appear before the more-common infectious manifestations more typically seen in immunodeficiency disorders. In this study, we have attempted to study such symptoms and uncover their relationship with inborn errors of immunity. METHODOLOGY: In this retrospective descriptive-analytical study, 381 cases of IEIs in children that were referr

Anti-melanoma differentiation-associated gene 5 antibody associated rapidly progressive interstitial lung disease in a pediatric patient: a case report.

BACKGROUND: Rapidly progressive interstitial lung disease presents as a severe complication of juvenile dermatomyositis, particularly when associated with anti-melanoma differentiation-associated gene 5. We report a pediatric case that underscores the necessity for clinicians to maintain a high index of suspicion for early identification and management. CASE PRESENTATION: A previously healthy 7-year-old White girl presented with a 6-week history of generalized weakness, fever, joint pain, and abdominal pain. Initial examination revealed hypoxia, tachypnea, and hepatosplenomegaly. Laboratory t

Oral JAK inhibitors for pediatric inflammatory skin disease.

PURPOSE OF REVIEW: JAK inhibitors have emerged as a promising new therapy for inflammatory skin disease, including in pediatric populations. However, due to their boxed warnings and lack of long-term safety and efficacy data, their specific role in the treatment ladder for various conditions remains to be determined. RECENT FINDINGS: JAK inhibitors are currently approved for pediatric atopic dermatitis and alopecia areata with evidence of both efficacy and safety. They are currently being investigated for use in children with vitiligo, psoriasis, juvenile dermatomyositis, and hidradenitis sup
