Initial success with B cell-targeted chimeric antigen receptor (CAR) T cells for the treatment of systemic lupus erythematosus and other rheumatic diseases has generated enthusiasm for the broad application of this technology outside of the field of oncology. Paediatric patients with severe rheumatic diseases require lifelong therapy with a substantial toxicity burden and a high cost of care. Paradigm-shifting treatments, including CAR T cells, are desperately needed. Although CAR T cell therapy shows promise for paediatric rheumatic diseases, there are unique aspects of care compared with adu

Juvenile dermatomyositis (JDM) is the most popular subtype of juvenile idiopathic inflammatory myopathies clinically characterised by skin manifestations and myositis. Some patients with JDM present predominantly with skin symptoms without significant muscle weakness. Furthermore, some patients exhibit residual skin disease even after showing improvement of muscle symptoms with systemic glucocorticoids (GCs) and immunosuppressants. Topical GCs and/or tacrolimus are recommended for the patients with skin predominant JDM. Furthermore, systemic use of GCs and methotrexate (MTX) are indicated in r

Panniculitis is a rare clinical finding in dermatomyositis. There are few reported cases in the medical literature. In this report, we describe a 17-year-old male patient with anti-MDA5 positive hypomyopathic dermatomyositis who, eight months after diagnosis, presented with indurated nodules on the right forearm and right thigh despite methotrexate and monthly intravenous immunoglobulin treatment. A skin biopsy revealed lobular panniculitis with lymphocytic infiltrate. His lesions were successfully controlled with hydroxychloroquine and azathioprine. This article presents a case regarding the

AIM: To analyze a broad spectrum of cytokine in the serum of patients with JDM.
BACKGROUND: Juvenile dermatomyositis (JDM) is the most common subtype of idiopathic inflammatory myopathies characterized by muscle and skin involvement. The etiology of JDM is unclear. A variety of cytokines play a role in the pathogenesis of JDM. Interferons, galectin-9, CLCX10, and neopterin are the most promising biomarkers.
OBJECTIVE: This study describes the associations between clinical symptoms, cytokine, and interferon profiles in children with JDM.
MATERIALS AND METHODS: Ten patients (6 girls and 4 boy

Juvenile dermatomyositis (JDM) is a rare, idiopathic inflammatory myopathy of childhood and first-line therapy typically includes corticosteroids and immunosuppressive agents, but complete and sustained remission of the disease is not always achieved. This case report describes an 8-year-old girl with refractory JDM who failed multiple therapies, including corticosteroids, disease-modifying agents, and JAK inhibitors. After the introduction of anifrolumab, a monoclonal antibody targeting the type I interferon receptor, the patient experienced significant improvement in both skin and muscle sym

OBJECTIVE: Juvenile dermatomyositis (JDM) is an uncommon autoimmune condition in children, often leading to prolonged disease burden and significant morbidity. Despite global advancements in understanding JDM, studies from the Middle East, particularly Oman, remain scarce. This study aims to characterize JDM from an Omani national cohort, evaluating clinical manifestations, laboratory features, disease course, and treatment outcomes.
METHODS: A retrospective review of all JDM patients diagnosed and managed by pediatric rheumatologist in tertiary centers in Oman was conducted. Patient demograp

Alternative splicing (AS) and alternative polyadenylation (APA) are widespread in the immune system. However, their precise role in juvenile dermatomyositis (JDM) remains unclear. Based on the polyadenylation sequencing platform, we described APA landscape of refractory JDM before and after autologous hematopoietic stem cell transplantation (AHSCT) treatment and revealed a shortened APA event of RNA splicing genes in refractory JDM. Notably, the use of the proximal polyadenylation site (PAS) in CELF2 is a distinctive feature of refractory JDM, which transitions to the distal PAS following AHSC

PURPOSE OF REVIEW: This review provides updates on juvenile dermatomyositis pathogenesis and treatment.
RECENT FINDINGS: JDM pathogenesis research updates in genetic risk factors include C4 copy number. Studies clarify myositis-specific autoantibodies' (MSA) role in disease pathogenesis and more myositis-associated antibody (MAA) clinical associations. Recent studies validate an interferon (IFN)-regulated gene score and an IFN-related monocyte surface protein marker, SIGLEC-1. Vasculopathy and mitochondrial dysfunction evidence increases, both with ties to IFN. Studies point to not only T and

BACKGROUND: Interstitial lung disease (ILD) is a frequently observed pulmonary manifestation in juvenile dermatomyositis (JDM), and may significantly impact patient outcomes. Therefore, early lung involvement identification is essential. Radiomics is a new image analysis technique and might offer valuable information for the diagnosis of interstitial lung disease in juvenile dermatomyositis (JDM-ILD).
METHODS: We retrospectively analyzed clinical data of 56 children with JDM, and all participants gave written informed consent. These children were divided into the JDM group (n = 32) and JDM-IL

BACKGROUND: Juvenile Dermatomyositis (JDM) is a systemic autoimmune disease in children, characterised by skin and muscle inflammation, with incidence of 1.6 to 4 cases per million children annually. Calcinosis, affecting 20% to 70% of JDM patients, can lead to significant morbidity. The association of calcinosis with anti-Mi-2 antibodies is rare and complicates management.
CASE: We present a rare case of a 12-year-old girl diagnosed with JDM, extensive calcinosis, and positive anti-Mi-2 antibodies. The patient exhibited significant muscle weakness, skin manifestations, and painful calcinosis

OBJECTIVES: This study aimed to investigate the spatial transcriptomic landscape of muscle tissue from patients with treatment-naive juvenile dermatomyositis (JDM) in comparison to healthy paediatric muscle tissue.
METHODS: Muscle biopsies from 3 patients with JDM and 3 age-matched controls were analysed using the Nanostring GeoMx Digital Spatial Profiler. Regions of interest were selected based on muscle fibres without immune cells, immune cell infiltration and CD68+ macrophage enrichment. Differential gene expression, pathway analysis and pathways clustering analysis were conducted. Key fin
