Juvenile dermatomyositis (JDM) is a rare autoimmune disease in children, characterized by muscle inflammation and skin manifestations. This case report describes a nine-year-old boy with JDM associated with anti-TIF1-γ antibodies and interstitial lung disease (ILD). The patient presented with progressive muscle weakness and characteristic skin lesions. Imaging confirmed ILD, and treatment with methylprednisolone pulses and rituximab resulted in significant improvement. This case emphasizes the need for pulmonary monitoring in JDM patients, regardless of antibody profile, and highlights the imp

Tumoral calcinosis is a debilitating complication of juvenile dermatomyositis (JDM) that results from dystrophic calcification in inflamed soft tissues. Although often asymptomatic, it can occasionally be complicated by secondary infections, requiring both medical and surgical interventions. Here, we report a rare case of tumoral calcinosis with superadded infection in a child with JDM, along with a brief pathophysiological and therapeutic overview. An 11-year-old girl with a known history of JDM presented with fever and localized pain in the right knee. Examination revealed local warmth, mild

Idiopathic inflammatory myopathies (IIM) are a heterogeneous group of rare systemic autoimmune rheumatic diseases. Despite advances in treatment, the definition of remission and low disease activity (LDA) in IIM remains inconsistent and lacks consensus and validation. This review summarizes existing published definitions, achievement rates, and predictive factors of remission/LDA in adult IIM, focusing on dermatomyositis (DM), polymyositis (PM), anti-synthetase syndrome (ASyS), and immune-mediated necrotizing myopathies (IMNM). Our literature review revealed a wide variability in remission def

BACKGROUND: Juvenile dermatomyositis (JDM) is an inflammatory vasculopathy characterized by muscle weakness and systemic inflammation. This study aimed to investigate the clinical utility of neutrophil activation markers, specifically calprotectin (S100A8/A9) and myeloperoxidase (MPO)-DNA complexes, as potential biomarkers for muscle inflammation and predictors of muscle outcomes in JDM.
FINDINGS: Plasma levels of calprotectin and MPO-DNA were quantified using ELISA in JDM (n = 36), juvenile idiopathic arthritis (JIA, n = 13), and healthy controls (HCs, n = 21). Disease severity and muscle fu

INTRODUCTION: Vaccinations have had a transformative impact on public health, reducing the incidence of many infectious diseases and increasing survival. However, there remains uncertainty about the potential of vaccines to trigger autoimmune diseases such as the idiopathic inflammatory myopathies (IIM). Myositis after vaccination (MAV) is a rare clinical entity, but given immunogenetic associations with other adverse events, we explored genetic risk factors, particularly human leukocyte antigen (HLA) alleles and GM/KM immunoglobulin allotypes, that may predispose individuals to develop MAV.

This article explores the role of physical activity monitors (PAMs) in addressing these challenges, focusing on their reliability, validity, and responsiveness in idiopathic inflammatory myopath (IIM) patients. PAMs provide continuous, objective data on physical activity. Evidence supports the use of PAMs as reliable tools in both adult and juvenile IIM populations, correlating with clinical disease measures and reflecting changes in disease activity. This article discusses current findings, limitations, and future directions for integrating PAMs into clinical practice and research, highlighti

Juvenile dermatomyositis (JDM) is an autoimmune systemic connective tissue disorder treated by various anti-inflammatory and immunomodulating agents. JDM typically requires immunomodulatory therapies including corticosteroids, disease-modifying antirheumatic drugs, intravenous immunoglobulins, and other steroid-sparing therapies. It is important to also employ nonpharmacologic interventions, which are integral in the treatment of JDM to optimize outcomes. These include photoprotection, physical therapy, occupational therapy, and mental health support. There are increasing number of exciting, e

Superimposed segmental juvenile dermatomyositis (SSJDM) is a rare and underrecognized variant of juvenile dermatomyositis (JDM) characterized by an initial, long-standing, well-defined lesion following Blaschko lines, later accompanied by classic cutaneous and systemic features. SSJDM likely results from early developmental genetic mutations causing loss of heterozygosity and type 2 cutaneous mosaicism. We describe an eight-year-old girl with morpheiform SSJDM and review the previously reported cases, analyzing clinical features, treatment, and outcomes. We also propose a clinical classificati

BACKGROUND: Neutrophils contribute to the pathogenesis of many autoimmune diseases. While the neutrophil to lymphocyte ratio (NLR) is associated with disease activity in adult dermatomyositis, its clinical utility in juvenile dermatomyositis (JDM) is unknown. METHODS: Demographic, medication use, autoantibody profile, disease activity markers, and laboratory data from 97 JDM patients was collected using retrospective chart review. Patients were age-stratified (0–9 vs. 9–21 years of age), as white blood cell count stabilizes at 9 years. The relationships between NLR with JDM disease activity me
