A rare case of ultrastructurally proven toxoplasmic myositis masquerading as necrotizing myopathy with emphasis on pathobiology.

Toxoplasmosis is a zoonotic infection caused by the obligate protozoan Toxoplasma gondii. In an immunocompetent human host, the parasite forms bradyzoites (tissue cysts), mainly in skeletal muscle, eye, brain, or cardiac muscle, and remains latent. Once cysts are established, type 1 cytokine-based immunity controls the conversion to tachyzoites from bradyzoites. We propose that short-term immunosuppression can only cause a transient defect in this cycle of conversion control. The temporary lapse in conversion control releases few tachyzoites, and the host immunity in response gives rise to cyt

Unusually long-term survival in a feline with Fibrodysplasia ossificans progressiva (FOP-like) disease without ACVR1 gene mutations.

Fibrodysplasia ossificans progressiva (FOP) is a rare genetic disorder characterized by heterotopic ossification within the connective tissues that affects the muscles, tendons, and joint capsules. It is an autosomal inherited condition caused by mutations in ACVR1, which is associated with ectopic bone formation. A 5-year-old neutered male outdoor cat, clinically diagnosed with FOP-like disease three years prior, presented for follow-up evaluation owing to an unusually long survival. New blood samples and radiographs were obtained to assess disease progression and investigate the patient's ge

Case Report of an Atypical Presentation of Inclusion Body Myositis Masquerading as Polymyalgia Rheumatica.

INTRODUCTION: Idiopathic inflammatory myopathies are a group of rheumatologic disorders presenting with progressive muscle weakness and the presence of inflammatory infiltrates in muscle tissue on histopathology. Inclusion body myositis classically has an insidious onset and slow progression and affects the older population, most commonly men. Muscle weakness is usually asymmetric and involves the distal upper extremity muscle groups. CASE PRESENTATION: This case describes a 59-year-old man presenting with worsening symmetrical upper and lower extremity proximal muscle weakness and disabling

Identification of a distinctive gene signature associated with disease activity in granulomatous myositis.

OBJECTIVES: Granulomatous myositis (GM) is defined by focal collections of activated macrophages that fuse to form multinucleated cells that aggregate into granulomas within skeletal muscle. This study aimed to elucidate the pathophysiology of GM by defining its specific transcriptomic profile. METHODS: Bulk RNA sequencing was performed on 722 muscle biopsies, including 38 from patients with GM, other myopathies and healthy comparators. Spatial transcriptomics and immunohistochemistry assays were used to identify the location of specific transcripts and proteins within the tissue. RESULTS: W

Genetic Association Between Polymyositis/Dermatomyositis and Epilepsy: Insights From Mendelian Randomization and Bioinformatic Analyses.

BACKGROUND: Idiopathic inflammatory myositis (IIM), comprising polymyositis (PM) and dermatomyositis (DM), is a collective term for immune-mediated diseases characterized by skeletal muscle inflammation. Emerging evidence points to an increased incidence of epilepsy in patients with PM/DM. However, the causality and underlying mechanisms behind this association are unclear. Our study aimed to explore the potential causal link between PM/DM and epilepsy, with a focus on immune-mediated mechanisms, using Mendelian randomization (MR) and transcriptome analyses. METHODS: Initially, summary data f

Initial presentation with elevated transaminases and subsequent hematuria in limb-girdle muscular dystrophy type 2B: A case report.

RATIONALE: Limb-girdle muscular dystrophy type 2B (LGMD2B) is a degenerative muscle disorder induced by mutations in the dysferlin gene. Dysferlin is involved in membrane repair and vesicle fusion through its 7 C2 calcium-binding domains, which mediate these calcium-dependent processes. It is currently considered an untreatable neuromuscular condition with a poor prognosis. The estimated incidence of this disease is 1 to 6.5 per 100,000 individuals. The primary clinical features of LGMD2B include proximal muscle weakness and elevated serum creatine kinase (CK) levels; occasionally, patients ma

Immunotherapies in autoimmune inflammatory myopathies: Rationale and therapeutic updates.

Autoimmune inflammatory myopathies (AIM) constitute a heterogeneous group of acquired myopathies with endomysial inflammation as a shared feature highlighting a potentially autoimmune inflammatory process amenable to immunotherapies. The disorders can be best classified as dermatomyositis, necrotizing autoimmune myositis, antisynthetase syndrome-overlap myositis, and inclusion body myositis. Because of clinical and immunohistologic heterogeneity but often overlapping pathophysiologic features among all major subsets, a correct diagnosis is critical from the outset to apply the most suitable an
