Defining interstitial lung disease related to idiopathic inflammatory myopathies: a systematic review protocol of the Myositis Clinical Trial Consortium (MCTC).

BACKGROUND: Idiopathic inflammatory myopathies (IIM) are systemic autoimmune diseases that include dermatomyositis, polymyositis, antisynthetase syndrome, and immune-mediated necrotizing myopathy in both adult and juvenile populations. Interstitial lung disease (ILD) is a common and serious complication of IIM, with a highly variable clinical course ranging from indolent disease to rapidly progressive respiratory failure, often influenced by underlying myositis-specific autoantibody profiles. Despite the substantial morbidity and mortality associated with IIM-ILD in both adults and children, h

Anti-PM/Scl antibody-positive myositis with muscle pathology resembling anti-synthetase syndrome.

Anti-polymyositis/scleroderma (PM/Scl) antibodies were originally thought to be autoantibodies associated with overlap syndromes involving idiopathic inflammatory myopathies and systemic sclerosis, but some patients with these antibodies present with clinical features similar to those of anti-synthetase syndrome (ASyS). Herein, we report the case of a 63-year-old Japanese woman with anti-PM/Scl antibody-positive, but anti-aminoacyl tRNA synthetases antibody-negative myositis with interstitial lung disease and dermatomyositis-like skin manifestations. Her muscle pathological findings resembled

Bladder carcinoma associated with anti-NXP2-positive inflammatory myopathy: a case-based review.

Anti-nuclear matrix protein 2 (NXP2) antibodies are increasingly recognized in adult idiopathic inflammatory myopathies (IIM) and have been associated with an elevated risk of malignancy. However, the spectrum of associated malignancies remains incompletely characterized. We report a case of anti-NXP2-positive myositis associated with bladder carcinoma in a 56-year-old man who presented with severe proximal muscle weakness, early dysphagia, and subcutaneous edema. Evaluation confirmed inflammatory myopathy, and myositis-specific antibody testing demonstrated anti-NXP2 positivity. Given the rec

Case Report: Balancing immunosuppression and infection-intravenous immunoglobulin in anti-HMGCR immune-mediated necrotizing myopathy complicated by severe pneumonia.

A 65-year-old male with a 3-year history of atorvastatin use presented with progressive proximal muscle weakness, dysphagia, and markedly elevated creatine kinase (2,277 U/L). Initially misdiagnosed with polymyositis, he deteriorated with severe pneumonia, septicemia, and type I respiratory failure requiring mechanical ventilation. Interleukin-6 rose from 12.83 to 610.9 pg/mL during sepsis. After confirmation of anti-HMGCR antibody positivity (18 arbitrary units [AU], cutoff >10 AU) via line blot assay, we discontinued methotrexate, maintained methylprednisolone at 40 mg, and initiated intrave

Evaluation of the prognostic value of visual and quantitative CT analysis in myositis-associated interstitial lung disease.

OBJECTIVE: To evaluate chest high-resolution CT (HRCT) images in idiopathic inflammatory myositis (IIM) using both quantitative CT and visual assessment. METHODS: HRCT images from a single-centre longitudinal IIM cohort were reviewed both visually and by quantitative imaging analysis (QIA), which quantified the extent of ground glass (QGG), fibrosis (QLF) and total interstitial lung disease burden (QILD). Visual review identified ILD presence and pattern and excluded other comorbid disease processes. Linear regression assessed correlations between QIA measures and pulmonary function tests (PF

Postpartum-onset anti-PM/Scl-positive dermatomyositis-systemic sclerosis overlap syndrome with reversible interstitial lung disease: a case report.

BACKGROUND: Polymyositis-scleroderma autoantibody (Anti-PM/Scl) associated connective tissue disease is a recognized overlap syndrome classically characterized by features of polymyositis- systemic sclerosis overlap. However, presentation with a dermatomyositis (DM) phenotype accompanied with interstitial lung disease (ILD) is less commonly reported. Pregnancy and the postpartum period are recognized immunological triggers for overlap syndromes, potentially leading to autoimmune disease. Postpartum-onset inflammatory myopathy with overlap features and significant pulmonary involvement poses su

Identification of Novel Risk Loci for Common B-Cell Lymphoma Subtypes Through Cross-Trait Analysis with Idiopathic Inflammatory Myopathies.

Objectives: The genetic architecture of B-cell lymphomas is not fully characterized. We aimed to identify novel candidate variants associated with four common B-cell lymphoma subtypes-diffuse large B-cell lymphoma (DLBCL), follicular lymphoma (FL), chronic lymphocytic leukemia (CLL), and marginal zone lymphoma (MZL)-through cross-trait analyses with correlated idiopathic inflammatory myopathies subtypes, dermatomyositis (DM) and polymyositis (PM). Methods: Leveraging shared genetic susceptibility with DM and PM, we applied a pleiotropy-informed conditional false discovery rate (condFDR) method

JAK inhibitors in areas outside of inflammatory arthritis: focus on connective tissue diseases.

Janus kinase (JAK) inhibitors have emerged as a transformative therapeutic class across autoimmune diseases by targeting multiple cytokine networks implicated in inflammation and fibrosis. Beyond inflammatory arthritis, increasing evidence supports their potential efficacy in connective tissue diseases such as idiopathic inflammatory myopathies, systemic sclerosis, primary Sjögren's disease, and systemic lupus erythematosus. Through modulation of type I/II interferon and interleukin signalling, JAK inhibition exerts broad anti-inflammatory and antifibrotic effects, translating into clinical im

Anti-PM/Scl Antibody-Positive Dermatomyositis With Rapidly Progressive Interstitial Lung Disease in a 19-Year-Old Male: Clinical Implications of a Presumptive Diagnosis Based on Line Immunoassay Positivity.

Anti-PM/Scl antibodies are classically linked to systemic sclerosis-polymyositis overlap, but a dermatomyositis phenotype with interstitial lung disease has also been described. However, the clinical course and therapeutic approach for short-term progressive interstitial lung disease in anti-PM/Scl-positive dermatomyositis remain incompletely defined. A 19-year-old Japanese male developed cough and exertional dyspnea after several weeks of progressive myalgia and proximal muscle weakness. Marked creatine kinase elevation, a high-titer nucleolar-pattern antinuclear antibody, Gottron papules, an

Spectrum of clinical phenotypes associated with myositis-specific and myositis-associated antibodies in juvenile idiopathic inflammatory myositis: Our experience from North India.

BACKGROUND: Myositis-specific autoantibodies (MSAs) and myositis-associated autoantibodies (MAAs) are increasingly being identified in juvenile inflammatory myositis (JIIM). The current study's objective was to characterize the incidence and clinical-phenotypic profile of MSA/MAA in a cohort of JIIM from North India. METHODS: We reviewed the medical records of the Pediatric Rheumatology Clinic of a tertiary care referral centre in Chandigarh, North India (1992 to 2024) and analysed the clinical details of children with JIIM. MSA/MAAs were assayed using a 16-antigen kit immunoblot assay (Eurol

Total Knee Arthroplasty in a patient with anti-synthetase syndrome and severe knee stiffness: A case report.

Anti-synthetase syndrome (ASyS) is a rare autoimmune disorder with diverse clinical manifestations, often initially misdiagnosed as rheumatoid arthritis. Delayed diagnosis may result in severe joint destruction, disabling polymyositis, and even mortality from rapidly progressive respiratory illness. With this context, we present a rare case of ASyS in a 53-year-old female exhibiting the classical triad of chronic arthritis, myositis, and interstitial lung disease (ILD) - features seldom observed together. She experienced severe arthritis in both knees with pronounced varus deformity, impairing

Identification of genetic variants associated with idiopathic inflammatory myopathies via cross-trait analysis with B cell lymphomas.

The genetic architecture of idiopathic inflammatory myopathies (IIMs) remains incompletely defined. When increasing sample size is not feasible, cross-trait analysis of genetically correlated diseases offers an effective strategy for discovering risk loci. Using summary statistics of IIM and B cell lymphoma subtypes, we applied conditional false discovery rate (condFDR) and multi-trait analysis of genome-wide association studies (GWASs) (MTAG) to detect genetic associations with IIM risk. Single-nucleotide polymorphisms (SNPs) outside the human leukocyte antigen (HLA) region meeting significan

CTRP1 regulates skeletal muscle differentiation through quality control of mitochondrial dynamics and function.

Mitochondrial dysfunction is a hallmark of myopathies and impaired skeletal muscle differentiation. Here, we demonstrate that C1q/TNF-related protein 1 (CTRP1) is essential for maintaining mitochondrial dynamics and supporting myogenic differentiation. Loss of CTRP1 in myoblasts and in skeletal muscle-specific-knockout (CTRP1 KOΔACTA) mice led to impaired myotube formation, reduced muscle fiber cross-sectional area, and decreased muscle strength. CTRP1 deficiency also shifted the muscle fiber composition from oxidative type IIA to glycolytic type IIB fibers, indicating a compromised mitochondr

Clinical features and prognostic factors of hospitalized patients with acute exacerbation of interstitial lung disease secondary to dermatomyositis/polymyositis.

INTRODUCTION/OBJECTIVES: Acute exacerbation of interstitial lung disease secondary to dermatomyositis or polymyositis (DM/PM-AEILD) is an intractable, life-threatening disease with unsatisfactory therapeutic efficacy, particularly in patients positive for anti-melanoma differentiation-associated gene 5 (anti-MDA5) antibody (Ab). Identifying prognostic risk factors for DM/PM-AEILD might help clinicians to stratify high-risk patients at an early stage and deliver targeted standardized treatment. METHODS: We retrospectively collected clinical data of 35 hospitalized patients diagnosed with DM/PM
