Proteomic profiles in inclusion body myositis and polymyositis with mitochondrial pathology.

BACKGROUND: Idiopathic inflammatory myopathies (IIMs) are autoimmune muscle diseases with distinct clinical, histopathological, and molecular features. Among them, inclusion body myositis (IBM) is refractory to immunotherapy and characterized by combined inflammatory and degenerative changes. Polymyositis with mitochondrial pathology (PM-Mito) has been proposed as a prodromal stage of IBM, but molecular profile underlying this spectrum remains poorly defined. METHODS: Skeletal muscle biopsies from 38 IBM, 14 PM-Mito, 5 anti-synthetase syndrome (ASyS), 3 dermatomyositis (DM), 5 immune-mediated

Animal Models of Idiopathic Inflammatory Myopathies: Bridging Mechanistic Insights and Clinical Translation.

Idiopathic inflammatory myopathies (IIMs) are a heterogeneous group of autoimmune diseases with wide variation in pathogenesis, clinical presentation, and serological profiles, leading to substantial differences in diagnosis, classification, treatment response, and prognosis among patients. Although decades of progress have been made in understanding myositis-specific autoantibodies and molecular pathology, important gaps remain in elucidating disease mechanisms and identifying effective therapeutic targets. Therefore, animal models that recapitulate key characteristics of human IIMs provide a

Clinicopathological and serological spectrum of idiopathic inflammatory myopathies: Experience from a neuromuscular referral center in Cyprus.

BackgroundThe classification of idiopathic inflammatory myopathies (IIMs) has evolved substantially with the identification of myositis-specific autoantibodies (MSAs). However, how contemporary classification frameworks are applied in routine clinical practice remains variable. This study provides a clinicopathological and serological characterization of IIMs from a national neuromuscular referral center in Cyprus.MethodsWe conducted a retrospective, single-center cohort study of adult patients diagnosed with IIM between 2007 and 2025. Patients were classified using a clinicopathological and s

Complete remission and relapse in the treatment of idiopathic inflammatory myopathies: a retrospective cohort study.

OBJECTIVES: To investigate the achievement of complete remission (CR) and the management for relapsed cases in the treatment of idiopathic inflammatory myopathies (IIM). METHODS: A total of 132 patients were retrospectively analysed. CR was defined as no evidence of disease activity in all involved organs for a continuous period of six months. Relapse was defined as a dose increase of glucocorticoid (GC) more than 50% and an increase in prednisolone (PSL) equivalent to more than 0.4 mg/kg/day. Clinical data were collected by electrical chart review. RESULTS: Eighty-eight (66.7%) patients ach

Causal link between polymyositis and idiopathic pulmonary fibrosis in individuals of European ancestry: A Mendelian randomization and multi-omics study identifying mediators and drug targets.

Polymyositis (PM), an autoimmune muscle disorder, often leads to interstitial lung disease (ILD), including idiopathic pulmonary fibrosis (IPF). However, causality and mechanisms are unclear. This study aimed to investigate the causal effect of PM on IPF risk, identify potential mediating biomarkers through multi-omics analysis, and explore candidate therapeutic drugs. Two-sample Mendelian randomization (MR) was conducted using publicly available genome-wide association study (GWAS) summary statistics (PM: 244 cases/4,28,965 controls; IPF: 6257 cases/9,47,616 controls). Reverse-causality was e

Minimal residual disease flow cytometry of peripheral blood B-cell subsets in patients with idiopathic inflammatory myopathies following rituximab therapy: a single-centre, retrospective cohort study.

BACKGROUND: Rituximab treatment has shown inconclusive results in clinical trials and observational studies in patients with idiopathic inflammatory myopathies. This study aimed to assess whether complete depletion of distinct B-cell subsets was associated with clinical response in rituximab-treated patients with myositis using highly sensitive flow cytometry (HSFC). METHODS: This single-centre, retrospective cohort study included adults (aged ≥18 years) treated with rituximab who fulfilled the American College of Rheumatology (ACR) and European Alliance of Associations for Rheumatology (EULA

Mental- and physical-health comorbidity are strongly associated in idiopathic inflammatory myopathies: a multinational cross-sectional e-survey.

Comorbidities are associated with morbidity, function, and survival in idiopathic inflammatory myopathies (IIM), but subtype-specific patterns, mental-physical comorbidity relationships, and patient-reported outcomes remain incompletely characterized. We hypothesized that multimorbidity in IIM is heterogeneous, associated with demographic, clinical, and psychosocial factors, and linked to differences in physical function and fatigue. We studied 1028 adults with self-reported, specialist-confirmed IIM. Comorbidities were assessed using the Functional Comorbidity Index and categorized into basic

Sporadic inclusion body myositis: overreliance on histology may lead to misdiagnosis.

We describe a 57-year-old man who had initial isolated weakness in the deep finger flexors of his non-dominant hand, followed by quadriceps weakness and wasting a year later. Despite the distribution specific for sporadic inclusion body myositis (s-IBM) and early myogenic abnormalities found in deep finger flexors, the case was initially misdiagnosed as polymyositis based on the initial biopsy results, which revealed endomysial inflammation with partial invasion/compression of non-necrotic fiber, without rimmed vacuoles. As a result, the patient received unnecessary treatment, including immuno

Severe Peripheral Arterial Ischemia Leading to Digital Gangrene in Mixed Connective Tissue Disease: A Case Report.

Mixed connective tissue disease (MCTD) is a rare systemic autoimmune disorder characterized by overlapping clinical features of systemic lupus erythematosus, systemic sclerosis, and polymyositis, in association with anti-U1 ribonucleoprotein (anti-U1-RNP) antibodies. Although vascular manifestations such as Raynaud's phenomenon are common, severe peripheral ischemic complications are rare. We report the case of a 40-year-old woman with known MCTD who presented with a three-month history of progressive necrotic lesions affecting the lower extremities, associated with severe pain, impaired ambul

A systematic review of abatacept and belatacept in immune-mediated diseases.

BACKGROUND: Abatacept (ABA) and belatacept (BEL) are immunomodulatory fusion proteins in which the extracellular domain of cytotoxic T-lymphocyte-associated protein 4 (CTLA-4) is linked to the Fc fragment of human IgG1. Functionally, they bind to cell surface antigens CD80 and CD86 on antigen-presenting cells, thereby preventing CD28-mediated costimulatory signaling. This systematic review aims to evaluate the clinical efficacy of ABA and BEL in immune-mediated diseases, focusing on indications currently not approved by the United States Food and Drug Administration (FDA) or the European Medic

Polymyositis with scleroderma-like features and an anti-PM-Scl75 antibody detected by line blot but not by protein array or immunoprecipitation: A case report and literature review.

Anti-PM-Scl antibodies (Abs) are associated with systemic sclerosis (SSc)-myositis overlap syndrome. However, anti-PM-Scl75/100 reactivity detected by line blot (LB) assays is frequently not confirmed by protein array or immunoprecipitation, indicating that such results should be interpreted with caution in Japan. A 72-year-old woman developed limb fatigue and Raynaud's phenomenon, followed by muscle weakness, elevated serum creatine kinase levels, and gangrenous changes in several fingers. Magnetic resonance imaging and muscle biopsy findings were consistent with myositis. She tested positive

[Macrophage activation syndrome in a young patient with polymyositis].

Haemophagocytic lymphohistiocytosis/macrophage activation syndrome (HLH/MAS) is a life-threatening condition characterised by systemic hyperinflammation and is often triggered by infections, malignancies, or autoimmune disease. The HScore is a diagnostic tool used to estimate the likelihood of HLH/MAS based on clinical, biochemical, and histological findings. This case describes a patient with polymyositis developing MAS, indicated by an HScore of 219. He was treated with IV anakinra, steroids, and immunoglobulin, leading to significant improvement. This case emphasises the importance of early

Trigeminal neuropathy as the first symptom of mixed connective tissue disease: case report and literature review.

Mixed connective tissue disease (MCTD) combines features of systemic lupus erythematosus, scleroderma, and polymyositis. Neurologic involvement, including trigeminal neuropathy, is rare. The present report describes isolated trigeminal sensory neuropathy as the initial manifestation of MCTD. Quantitative sensory testing revealed progressive bilateral trigeminal sensory deficits. Laboratory evaluation showed elevated anti-U1 ribonucleoprotein (anti-U1-RNP) antibodies, and clinical examination revealed lymphadenopathy and hand swelling, confirming the diagnosis. No other systemic manifestations

Causal Associations Between Nine Autoimmune Diseases and Idiopathic Thrombocytopenic Purpura: A Mendelian Randomization Study.

INTRODUCTION: Clinical evidence suggests the presence of comorbidity between Autoimmune diseases (ADs) and secondary Idiopathic thrombocytopenic purpura (ITP), whereas the underlying causal relationship remains to be established. This study employed Mendelian Randomization (MR) analysis to investigate potential causal relationships between multiple common ADs and the risk of ITP. METHODS: Using criteria of P<5×10-8 , P<5×10-6 , and F-statistic>10, and excluding single nucleotide polymorphisms (SNPs) with confounding, palindromic sequences, and linkage disequilibrium as instrumental variables
