Exploring the anti-fibrotic effects of safflower in systemic sclerosis based on metabolomics and gut microbiota analyses.

BACKGROUND: Systemic sclerosis (SSc) is a complex autoimmune disease characterized by inflammation, fibrosis, and multi-organ involvement. Traditional botanical drugs have long been used empirically to manage fibrotic and inflammatory conditions, yet their pharmacological effects remain incompletely understood. OBJECTIVE: The aim of this study was to investigate the effects of an aqueous extract of safflower on fibrotic and inflammatory phenotypes, gut microbiota composition, and fecal metabolic profiles in a bleomycin-induced murine model of systemic sclerosis. METHODS: A murine model of sy

Longitudinal Assessment of the Juvenile Systemic Sclerosis Severity Score: Application in the National Registry for Childhood Onset Scleroderma.

OBJECTIVE: Juvenile-onset systemic sclerosis (jSSc) is a rare, heterogeneous pediatric autoimmune disease. Existing severity tools are often adapted from adult systemic sclerosis and lack pediatric-specific validation. The Juvenile Systemic Sclerosis Severity Score (J4S) was developed to address this gap by capturing multiorgan disease burden. This study evaluated the clinical use of the J4S using data from the National Registry for Childhood Onset Scleroderma (NRCOS). METHODS: We applied the J4S to 58 patients in the NRCOS cohort, analyzing scores at baseline and at one-, two-, and three-yea

Prevention and management of gastrointestinal side effects in patients with systemic sclerosis-interstitial lung disease receiving anti-fibrotic therapy: a modified Delphi consensus study.

OBJECTIVES: Interstitial lung disease (ILD) is one of the most common manifestations of systemic sclerosis (SSc), with most patients requiring treatment with immunosuppressive or anti-fibrotic agents to control ILD progression. Since gastrointestinal (GI) tract complications are widespread in this patient setting, we focused on their prevention and management. METHODS: We conducted a modified Delphi study following best practices for consensus studies. We involved 20 expert rheumatologists from 8 Italian regions in two online rounds conducted between April and September 2024. RESULTS: An agr

Dermatoscopy limitation and the critical role of capillaroscopy in the evaluation of systemic sclerosis and Raynaud's phenomenon among African American Veterans.

INTRODUCTION: Nailfold capillaroscopy (NFC) is a crucial tool for assessing diagnostic microvascular abnormalities in patients with Raynaud's phenomenon (RP). While dermatoscopy is a widely accepted, cost-effective screening method for systemic sclerosis (SSc), its utility depends on optimal visibility for accurate interpretation. The objective of this study was to determine if skin pigmentation affects the interpretability of nailfold evaluation. METHODS: In an Institutional Review Board-approved study at the TVHS Hospital, patients undergoing evaluation for CTD-related RP were enrolled. Ima

Resting Oxygen Consumption Estimates in Scleroderma Can Lead to Underestimation of Cardiac Output.

Accurate resting oxygen consumption (rVO2) quantification is critical for Fick-derived cardiac output calculations. Yet, clinical practice predominantly uses empirical estimations, which can be inaccurate. We evaluated the established rVO₂ prediction equations against direct metabolic cart measurements in systemic sclerosis patients, revealing significant discordances with potential diagnostic implications.

Nailfold capillaroscopy and organ involvement in systemic sclerosis: a systematic review.

Systemic sclerosis (SSc) is connective tissue disease characterised by early microvascular damage and tissue fibrosis. Nailfold videocapillaroscopy (NVC) enables direct assessment of the microangiopathy and classification into "scleroderma patterns", offering potential prognostic value. This systematic review aims to analyse the most updated evidence linking NVC findings with organ-specific involvement in SSc. Uniquely, only longitudinal studies, with centralised reading using the European Alliance of Associations for Rheumatology (EULAR) Study Group on Microcirculation in Rheumatic Diseases c

Mechanical gating of tendon fibrogenic transcription in systemic sclerosis.

Fibrosis arises from aberrant tissue repair in systemic sclerosis and is characterized by progressive stiffening across multiple tissues, including tendons. How altered tissue mechanics sustain fibrotic remodeling remains poorly understood, in part because of limited experimental models that capture key biophysical features of fibrosis. Here we develop a modular cantilever-based mechano-culture platform that enables tendon-like constructs to be maintained under controlled static tension. We show that elevated matrix tension induces fibroblast-to-myofibroblast activation and scar-like phenotype

Increased incidence of sudden cardiac death in systemic sclerosis: data from a nationwide cohort study.

OBJECTIVE: To evaluate incidence and risk factors of sudden cardiac death (SCD) in systemic sclerosis (SSc). METHODS: Individuals with SSc from the Australian Scleroderma Cohort Study meeting ACR/EULAR criteria were included. International Classification of Diseases (ICD)-10 code cause of death was sourced from the Australian National Death Index. "Presumed" SCD was defined as primary cause of death being an acute cardiovascular pathology (e.g., cardiac arrest, myocardial infarction). Between-group comparisons (presumed SCD vs. overall cohort) and multivariable Cox proportional hazard modelli

Real-World Adherence to Systemic Sclerosis Quality Indicators: A Single-Center Quality of Care Study.

OBJECTIVE: Systemic sclerosis (SSc) is associated with substantial morbidity and mortality, prompting the development of internationally endorsed quality indicators (QIs). However, real-world adherence to these standards remains incompletely characterized. We evaluated adherence to baseline and longitudinal SSc QIs and identified predictors of adherence in a specialized clinical practice. METHODS: We conducted a retrospective cohort study including all patients with SSc managed at a tertiary SSc clinic between January 2016 and December 2024. Adherence to QIs was assessed across baseline scree

Nailfold capillaroscopy improves cardiovascular risk stratification in SSc: an adjustment of the SCORE2 algorithm.

OBJECTIVE: SSc is characterized by micro- and macrovascular damage, increasing cardiovascular (CV) risk. The SCORE2 algorithm underestimates CV risk in systemic autoimmune diseases. Nailfold capillaroscopy (NFC) is used to assess SSc-related microvascular damage. We aimed to evaluate whether incorporating NFC findings into SCORE2/SCORE2-OP improves CV risk stratification in SSc. METHODS: Retrospective multicentre study including 276 patients with SSc. Baseline 10-year CV risk was estimated using SCORE2 or SCORE2-OP. NFC at diagnosis was assessed. Cox regression identified NFC variables associ

The immune landscape of systemic sclerosis: from pathogenic mechanisms to precision therapeutic breakthroughs.

Systemic sclerosis (SSc) is a chronic autoimmune disease characterized by immune dysregulation, microvascular damage, and multi-organ fibrosis. Recent breakthroughs in single-cell and spatial multi-omics technologies have profoundly revealed the high heterogeneity of the SSc immune microenvironment, including extensive aberrant activation of innate immunity (e.g., dendritic cells, macrophages, neutrophils) and adaptive immunity (T cells, B cells), and their interaction with fibroblasts and endothelial cells through an "immune-stromal-vascular" network that collectively drives the fibrotic proc

Automatic assessment of lung involvement in systemic sclerosis using deep learning.

BACKGROUND: Systemic sclerosis (SSc) is a relatively uncommon connective tissue disorder, commonly manifesting as interstitial lung disease (ILD) and affecting both the lung parenchyma and the modification of the space between endothelium and epithelium. Imaging modalities like computed tomography (CT) scans are essential for diagnosing and revealing specific abnormal findings (ILD patterns) in SSc, such as reticulation and Ground-glass opacity (GGO). To enhance diagnostic precision and minimize human error, we leverage deep learning (DL) techniques. MATERIALS AND METHODS: In our study, we co

A practical clinical approach to the diagnosis and management of morphea (localized scleroderma).

Morphea, also referred to as localized cutaneous scleroderma, encompasses a heterogeneous group of inflammatory and fibrosing skin disorders characterized by variable depth of tissue involvement and different clinical course. The diagnostic delay remains common due to insidious onset, broad differential diagnosis, and lack of validated biomarkers of disease activity. Clinical assessment relies therefore on expert evaluation, supported by clinical scoring systems and selected imaging techniques. Therapeutic strategies depend on disease subtype, activity, depth, and risk of irreversible damage,

Definition, diagnosis, treatment for scleroderma primary heart involvement.

Systemic sclerosis (SSc) is an autoimmune disease characterized by chronic inflammation leading to fibrosis in the skin and organs, including the heart. It is classified as SSc-primary heart involvement (pHI), defined as heart dysfunction primarily caused by SSc, excluding other causes. Several disease manifestations, including conduction abnormalities, cardiac arrythmias, myocarditis/pericardial involvement, heart failure (systolic and diastolic) and valvular dysfunction, are categorized within SSc-pHI. SSc-pHI has a high prevalence, high mortality, and increased risk for sudden cardiac death
