Human blood vessel organoids recapitulate key mechanisms of transition from vasculopathy to fibrosis in systemic sclerosis.

OBJECTIVES: Systemic sclerosis (SSc) is an autoimmune disease that transitions from vasculopathy as an initiating pathogenic event to tissue fibrosis. The mechanisms of these transitions remain, however, poorly understood, mainly because complex multicellular human models of SSc vasculopathy are lacking. We aimed to develop a complex multicellular human model of SSc vasculopathy and use it to investigate the mechanisms underlying this process. METHODS: Blood vessel organoids (BVOs) were derived from induced pluripotent stem cells of patients with SSc and healthy controls. Organoids were expos

Neurofascialvascular training in systemic sclerosis: Clinical outcomes, proposed rationale, and home-programme feasibility.

BACKGROUND: Secondary Raynaud's phenomenon in limited cutaneous systemic sclerosis is driven by structural microvascular disease and is frequently accompanied by hand oedema, stiffness, and functional disability. Evidence for pragmatic physiotherapy approaches addressing both vascular symptoms and soft-tissue involvement remains limited. CASE: A 66-year-old man with limited cutaneous systemic sclerosis (diagnosed in 2009) and long-standing triphasic Raynaud's symptoms reported cold-induced digital pain, recurrent fissures with prior ulceration, puffy hands, reduced finger range of motion, and

Triple-Positive Serology in Rapidly Progressive Glomerulonephritis: A Diagnostic Dilemma.

The acute onset of malignant hypertension and acute kidney injury in patients with features suggestive of systemic sclerosis often raises concern for scleroderma renal crisis, a life-threatening complication requiring prompt recognition and angiotensin-converting enzyme inhibitor therapy. However, other autoimmune vasculitides, including antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis and antiglomerular basement membrane (anti-GBM) disease, can present similarly but require immunosuppressive therapy. We reported a diagnostically challenging case of a 72-year-old woman with int

Case Report: Switching secukinumab to bimekizumab in diffuse cutaneous systemic sclerosis after autologous hematopoietic stem cell transplantation: a case follow-up.

Diffuse cutaneous systemic sclerosis (dcSSc) is a heterogeneous autoimmune disease characterized by progressive skin fibrosis, vasculopathy and variable organ involvement. Our recent case report about this patient described clinical improvement under IL-17A inhibition with secukinumab for worsening of cutaneous involvement after autologous hematopoietic stem-cell transplantation. We present the extended disease course through 2025. In July 2025 the patient presented with a new flare characterized by cutaneous tightening, dysesthetic sensory symptoms and intermittent pruritus despite ongoing se

Microvascular ultrasonography for quantitative evaluation of nailfold vasculature in systemic sclerosis.

INTRODUCTION: Systemic sclerosis (SSc) is characterized by microvascular dysfunction affecting the digital vasculature. Microvascular ultrasonography (MVUS) is a technique capable of detecting low-velocity flow. This study aimed to evaluate nailfold microvasculature in SSc using the MVUS-derived color pixel percentage (CPP) quantitatively. METHODS: Seventy-two participants were included in this cross-sectional observational study. Thirty-seven participants had SSc, 19 participants were healthy controls, and 16 individuals were hypertensive, receiving calcium-channel blockers (CaCBs). Nailfold

Validity of Bioelectrical Impedance Analysis for the Assessment of Body Composition in Patients With Systemic Sclerosis.

BACKGROUND: Systemic sclerosis (SSc) is an autoimmune disease characterized by vasculopathy and progressive fibrosis. Reduced muscle mass is common and contributes to myopenia, sarcopenia and adverse clinical outcomes. Dual-energy X-ray absorptiometry (DXA) is the most widely used method in clinical research and is recommended by international guidelines as the reference standard for muscle mass evaluation in sarcopenia. However, its accessibility in routine clinical practice is limited. Bioelectrical impedance analysis (BIA) is a validated method for assessing body composition due to its affo

Simplified ultrasound scoring scale (LUNA score) for the detection of interstitial lung disease using point-of-care lung ultrasound in systemic autoimmune diseases.

BACKGROUND AND OBJECTIVE: Interstitial lung disease (ILD) is a frequent and severe complication of systemic autoimmune diseases (SADs). Its early diagnosis, based on periodic high-resolution computed tomography (HRCT), involves radiation exposure. Lung ultrasound (LUS) has shown good performance in ILD detection. The aim of this study was to design a new simplified lung ultrasound scoring scale based on the quantification of B-lines and pleural irregularities (LUNA score) for ILD detection using a point-of-care ultrasound device. PATIENTS AND METHODS: Between November 2020 and December 2021,

Prevalence and clinical significance of anti-NOR90 antibodies in systemic sclerosis: Results from a multicentre cohort and systematic literature review.

OBJECTIVE: To evaluate the prevalence and clinical associations of anti-Nucleolar Organizer Region 90 (anti-NOR90) antibodies in patients with systemic sclerosis (SSc). METHODS: We conducted a cross-sectional study of anti-NOR90-positive SSc patients from two tertiary hospitals, using the EUROLINE SSc profile kit and including only patients with repeated moderate or strong positivity. A comparative analysis was performed between patients with and without anti-NOR90 antibodies. Further, we performed a systematic literature search to summarise published evidence on anti-NOR90 antibodies in SSc.

Association of nailfold capillaroscopic features with cardiac block and arrhythmias due to systemic sclerosis-related primary heart involvement.

INTRODUCTION: Impaired microvascular function has been implicated as a complication affecting multiple organs in systemic sclerosis (SSc). We investigated the correlation between microvascular damage on nailfold video capillaroscopy (NVC) and cardiac arrhythmias or conduction blocks in patients with primary heart involvement (pHI). MATERIAL AND METHODS: One hundred ten patients with SSc underwent an NVC study. Thirty-one patients with arrhythmias or conduction blocks were included in the study group. No patients in this group had pulmonary arterial hypertension, systemic hypertension, or isch

Pulmonary Function Decline in Very Early Diagnosis of Systemic Sclerosis: A Retrospective Longitudinal Single-Center Study.

OBJECTIVE: The very early diagnosis of systemic sclerosis (SSc; VEDOSS) criteria aim to identify patients at risk of evolving into SSc. The predictive value of diffusing capacity of the lungs for carbon monoxide (DLco) and forced vital capacity (FVC) remains poorly characterized. This study aimed to evaluate whether baseline pulmonary function test (PFT) alterations, especially DLco, predict progression to SSc and to examine longitudinal PFT changes in patients with stable VEDOSS over time. METHODS: A retrospective longitudinal study was conducted including 73 patients with VEDOSS. Patients w

Activity-Based Functional Exercise for Disability in Systemic Sclerosis: Protocol for a Feasibility Randomised Controlled Trial.

INTRODUCTION: Systemic Sclerosis (SSc) is associated with substantial disability in affected patients. Although exercise interventions for SSc have been developed, most focus on isolated body segments rather than global disability. Thus, this study aims to assess the feasibility of a functional activity-based exercise programme designed to address disability in individuals with SSc. METHODS: This feasibility study protocol is designed as a randomised controlled mixed-methods trial. It will include individuals with SSc aged 18-65 years and with a score ≥ 1 on the Scleroderma Health Assessment

TAPSE/sPAP Ratio in Systemic Sclerosis: Evidence From Pulmonary Hypertension and Implications for Early Risk Stratification-A Systematic Review.

PURPOSE: Cardiopulmonary involvement is a major determinant of morbidity and mortality in systemic sclerosis, particularly when pulmonary hypertension and right ventricular dysfunction develop. Increasing evidence suggests that prognosis in PH is more closely related to the ability of the right ventricle to adapt to increased afterload rather than to pulmonary pressure alone. The ratio between tricuspid annular plane systolic excursion and systolic pulmonary artery pressure has emerged as a simple echocardiographic surrogate of the interaction between right ventricular contractility and pulmon

Time-Controlled Refrigerated Stem Cell Therapy Mitigates Scleroderma Fibrosis via Modulation of Mitochondrial Autophagy and Gut Metabolism.

Scleroderma is a chronic autoimmune disease characterized by progressive fibrosis, associated with high morbidity and mortality, limited therapeutic efficacy, and significant systemic side effects. Therefore, there is an urgent need to develop novel treatment strategies with improved tissue targeting and safety profiles. In this study, a refrigerated-treated mesenchymal stem cell (RT-MSCs) system was established by culturing human umbilical cord-derived MSCs under controlled low-temperature conditions. The antifibrotic effects of RT-MSCs were evaluated through both in vivo and in vitro experim

Echocardiographic Evaluation of Global Cardiac Function in Systemic Sclerosis-Associated Pulmonary Arterial Hypertension: An Integrated Review of Parameters and Techniques.

Systemic sclerosis (SSc) is a connective tissue disease frequently complicated by pulmonary arterial hypertension (PAH), a leading cause of morbidity and mortality. Global cardiac function is a key determinant of prognosis and treatment response in SSc-PAH, necessitating precise and dynamic assessment. Echocardiography provides a comprehensive, noninvasive modality for this purpose. This narrative review synthesizes recent advances in echocardiographic evaluation of global cardiac function in SSc-PAH. It covers established parameters of right and left ventricular function, emerging indices suc

[The Role of Nailfold Capillaroscopy in Raynaud's Phenomenon and Autoimmune Diseases].

Connective tissue diseases are systemic autoimmune disorders for which diagnosis is often delayed owing to nonspecific early clinical manifestations. However, Raynaud's phenomenon is a common and early symptom that prompts etiological evaluation. In this context, nailfold capillaroscopy plays a pivotal role. It enables the detection of characteristic microvascular abnormalities, particularly the scleroderma pattern, which is strongly associated with systemic sclerosis and included in both classification and early diagnostic criteria. This review outlines the principles of capillaroscopy interp

Heel fat pad atrophy correlates with cutaneous fibrosis severity but not functional disability in systemic sclerosis: a single-center, cross-sectional, hypothesis-generating study.

To explore, in a hypothesis-generating framework, the relationship between heel fat pad thickness (HFPT), measured by plain radiography, and clinical parameters of disease severity and disability in systemic sclerosis (SSc), and to assess the specificity of HFPT atrophy by comparing SSc patients with healthy controls and disease controls. In this single-center, cross-sectional study, 98 SSc patients were enrolled. HFPT was measured on lateral foot radiographs with excellent reliability (intra-observer ICC = 0.96; inter-observer ICC = 0.94, 95% CI: 0.88–0.97). Clinical data included demographic

Skin Transcriptomics Reveal Shared Molecular Mechanisms for Skin and Lung Involvement in Systemic Sclerosis.

OBJECTIVE: Systemic sclerosis (SSc) is characterized by dysregulated immunity, microvascular damage, and progressive fibrosis. SSc-related interstitial lung disease (SSc-ILD) is the leading disease-specific cause of death; it is therefore important to identify biomarkers associated with SSc-ILD. We hypothesized that the skin transcriptome would provide mechanistic clues that could further our understanding of lung impairment in SSc-ILD. METHODS: We conducted a multistage analysis of SSc transcriptomes using weighted gene correlation analysis, gene set enrichment analysis, and single-cell RNA
