The Th17/Treg axis: a key to understanding and treating autoimmune disorders.

In autoimmune rheumatic diseases (ARDs), T cells mistakenly attack the body's own joints, skin, blood vessels, and other tissues, leading to chronic inflammation and tissue damage. Among these, the immune balance between T helper 17 lymphocytes (Th17) and regulatory T lymphocytes (Treg) is a foundation for maintaining normal immune function in the human body. An immune imbalance between Th17 and Treg cells is one of the key pathogenic mechanisms in ARDs. The percentages of Th17 and Treg cells can serve as important indicators for the severity of autoimmune diseases and treatment response. Ther

Cellular therapies in systemic Sclerosis: immunopathogenesis-informed precision targeting with CAR-T and CAR-NK approaches.

Systemic sclerosis (SSc) is a rare, heterogeneous systemic autoimmune rheumatic disease characterized by immune dysregulation, vasculopathy, and excessive fibrosis. SSc is associated with high morbidity and mortality despite current immunosuppressive and anti-fibrotic therapies. Emerging cellular therapies, particularly chimeric antigen receptor (CAR) T-cell and natural killer (NK) cell approaches offer potential for deep immune reset and long-term remission. This review examines the immunobiological rationale for these therapies in SSc and evaluates their early clinical evidence, including co

Exosome immunotherapy in inflammatory rheumatic and musculoskeletal diseases: a novel immunotherapeutic frontier.

Chronic autoimmune inflammatory rheumatic and musculoskeletal diseases (iRMD such as rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), and systemic sclerosis (SSc)) are characterized by the dysregulation of the immune system, leading to chronic inflammation and tissue damage. Traditional therapeutic approaches have focused on immunosuppressive drugs and biologics targeting specific cytokines or immune cells. However, these therapies often have limited efficacy and significant side effects. Moreover, they are not globally accessible due to high drug costs, especially in poor as wel

Neutrophil reprogramming underlie vasculopathy and lung disease in systemic sclerosis.

The role of neutrophils in systemic sclerosis (SSc) remains incompletely understood. To address this, blood samples from 39 SSc patients, 39 healthy controls, and 22 systemic lupus erythematosus (SLE) patients were analyzed. Our results indicate that in SSc, neutrophils exhibited substantial activation, evidenced by granule mobilization, elevated plasma levels of Neutrophil Extracellular Trap (NET) byproducts, and upregulated TIE2 expression. In parallel, they underwent metabolic reprogramming, characterized by increased autophagy, likely to support the heightened energy demands of activation.

Renal involvement in systemic sclerosis.

Systemic sclerosis is a complex connective tissue disease characterized by immune dysregulation, widespread vasculopathy, and progressive fibrosis affecting the skin and internal organs. Among its visceral complications, renal involvement, manifesting as scleroderma renal crisis, remains organ and life-threatening if not detected. Although its mortality has markedly declined with the life-saving introduction of angiotensin-converting enzyme inhibitors, renal complications remain a major determinant of morbidity, mortality, and long-term health outcomes. Beyond scleroderma renal crisis, a spect

The relationship between epicardial adipose tissue and mortality in systemic sclerosis patients: 10-year follow-up results.

BACKGROUND: Systemic sclerosis (SSc) is a connective tissue disease with cardiac involvement characterized by autoimmunity, vascular damage, and fibrosis. Recent studies suggest that epicardial adipose tissue (EAT) may be associated with poor cardiovascular outcomes in patients with SSc and cardiovascular comorbidities. Our aim in this study was to investigate the relationship between epicardial adipose tissue thickness and mortality in patients with systemic sclerosis. METHODS: Sixty patients who presented to cardiology and rheumatology outpatient clinics between January 2013 and December 20

Novel pleiotropic loci link vascular and immune pathways in systemic sclerosis and primary Raynaud's phenomenon.

OBJECTIVE: Raynaud's phenomenon (RP) is the first noticeable symptom of systemic sclerosis (SSc), appearing even years before other signs. Vascular and immune pathways, hallmarks in SSc pathogenesis, are implicated in primary RP. Hence, we aimed to define the shared genetic architecture between these conditions to explore pathogenic mechanisms and whether pleiotropic loci could help identify primary RP patients at increased genetic risk of developing SSc. METHODS: We analysed genome-wide association study (GWAS) summary statistics for primary RP (4 986 cases and 850 981 controls) and SSc (10

3D-Mesenchymal stromal cells derived VASH2 alleviates oxidative stress-induced endothelial senescence by mediating α-tubulin detyrosination in systemic sclerosis.

BACKGROUND: Endothelial cell (EC) senescence driven by oxidative stress is a pivotal contributor to vasculopathy in systemic sclerosis (SSc), leading to impaired angiogenesis and loss of vascular integrity. Three-dimensional (3D) cultured human umbilical cord-derived mesenchymal stromal cells (MSCs) represent a promising therapeutic strategy for SSc due to their enhanced anti-fibrotic and immunomodulatory properties. However, their potential to mitigate EC senescence remains unclear. This study aims to investigate whether 3D-MSCs confer superior therapeutic effects over conventional MSCs by am

Topoisomerase 1-DNA complexes are preferentially recognised by a subset of anti-topoisomerase 1 autoantibodies and their corresponding B cell receptors in systemic sclerosis.

OBJECTIVES: Systemic sclerosis (SSc) is a deleterious disease. Its clinical management is complicated by strong interpatient heterogeneity. Progressive organ fibrosis is linked to autoantibodies against topoisomerase 1 (TOP1). Here, we hypothesised that the interaction between anti-TOP1 autoantibodies (ATAs) and TOP1 could influence SSc pathogenesis. TOP1 is a DNA-binding enzyme. Therefore, we studied the effects and functional consequences of TOP1-DNA binding on ATA recognition. METHODS: ATA monoclonal antibodies (ATA mAbs) and ATA-expressing B cell lines were generated from patient-derived

Effects of locally applied water-filtered infrared a irradiation adjunctive to iloprost and carbon dioxide hand baths in patients with systemic sclerosis and severe Raynaud's phenomenon - a randomized controlled trial.

BACKGROUND: Almost all patients with systemic sclerosis (SSc) experience Raynaud's phenomenon (RP) with risk of digital ulcers (DU). In severe RP, intravenous iloprost is standard, yet additive hyperthermic therapy may enhance vasodilation and microcirculation. However, whether more intensive or alternative hyperthermic modalities offer benefits beyond commonly used CO2 hand baths remains unclear. METHODS: In this randomized, controlled trial, all patients received a baseline therapy of iloprost plus CO2 hand baths. The intervention group (IG) received additional water-filtered infrared-A (wI

Cellular and molecular mechanisms of systemic sclerosis: Translation to targeted therapies and clinical practice.

Systemic sclerosis (SSc) is characterized by profound clinical heterogeneity due to dysregulated communication between vascular, immune, and stromal compartments. This review synthesizes genetic and epigenetic determinants of SSc with observations from transcriptomic, proteomic, and metabolomic studies. We highlight emerging cellular- and molecular-level insights into vasculopathy, immune dysregulation, and fibroblast activation in SSc. Finally, we discuss how these mechanistic discoveries inform the development of targeted, precision-medicine based therapeutic approaches in SSc.

Feasibility study of a smartphone app to monitor systemic sclerosis-related digital ulcers-a potential new tool for remote clinical monitoring.

OBJECTIVES: To assess the feasibility, in a clinical setting, of using a smartphone app that allows 'tracking' of SSc-related digital ulcers/lesions over time and to gauge patient opinion of the app's benefits, including a newly added lesion feedback function for patients. METHODS: Patients with SSc with active digital ulcers/lesions were recruited and assessed twice: on study entry and at the point of lesion healing. Participants photographed their lesions and recorded patient-reported outcome measures (PROMs) with the app twice weekly. The lesion feedback function showed lesion size (measur

Systemic sclerosis-related digital ulcers: Current understanding and updated management approaches- a primer for clinicians.

Digital ulcers (DUs) are a common and debilitating manifestation of systemic sclerosis (SSc), driven by complex vascular dysfunction and mechanical factors, and are associated with substantial pain, disability, infection risk, and higher burden of broader disease. Despite growing insights into the contribution of endothelial injury and impaired angiogenesis, as well as increasing use of imaging and biomarker-based assessments, SSc-related DUs remain an area of significant unmet clinical need. Management is multifaceted, encompassing wound care, vasodilator/active therapies, and selected interv
