BACKGROUND: Systemic sclerosis (SSc) is a systemic autoimmune disease characterized by vasculopathy, immune system dysregulation, and fibrosis. There is growing evidence regarding the role of platelets in innate immunity and inflammatory processes, as they are an important source of mediators that may be key factors in the pathogenesis of SSc. Here we study the intraplatelet and plasma levels of various growth factors (GFs) and angiogenic factors in patients with SSc.
METHODS: The sample included 23 SSc patients and 16 controls. We determined the subjects' levels of factors (TGF-β, VEGF-A, PD

OBJECTIVE: Autologous hematopoietic cell transplantation (HCT) is an effective treatment for a subset of patients with systemic sclerosis (SSc). Unfortunately, relapse is a significant problem, with no available tests to predict relapse. We studied whether relapse is associated with pre- or post-HCT serum levels of SSc-related autoantibodies.
METHODS: The cohort comprised 38 consecutive evaluable patients with SSc who underwent HCT at a single center and were followed for a median of 33 months. Sixteen patients (42%) relapsed at a median of 14 months post-HCT. Autoantibody levels were determi

OBJECTIVES: This study aimed to analyse survival in SS-associated pulmonary hypertension (SSc-PH) patients with or without interstitial lung disease (ILD) stratified by PH monotherapy and PH combination therapy, respectively.
METHODS: Among the 6003 patients registered with the German Network for Systemic Sclerosis (DNSS) registry, SSc patients with incident PH diagnosed after registry entry were included. Kaplan-Meier analysis was performed to estimate the overall survival, and the log-rank test was used to assess differences between groups stratified by the presence of ILD or PH treatment.

Interstitial lung disease (ILD) is a major source of morbidity and the leading cause of mortality in people with systemic sclerosis (SSc). There have been advancements in the treatment of ILD. In this article, we review the American College of Rheumatology (ACR) and the American College of Chest Physicians (CHEST) guidelines for the screening, monitoring and treatment of SSc-ILD. We highlight additional pharmacologic and non-pharmacologic interventions that should be considered in those with SSc-ILD. Finally, we summarize the ACR guidelines for vaccinations in those with SSc-ILD, and recommend

A 62-year-old woman was diagnosed with diffuse cutaneous systemic sclerosis (dcSSc), complicated by scleroderma renal crisis, resulting in maintenance dialysis. Mycophenolate mofetil (MMF) and low-dose prednisolone stabilized her interstitial lung disease (ILD). Five years after the dcSSc diagnosis, she developed brain immunodeficiency‑associated lymphoproliferative disorders. Consequently, MMF was discontinued. Three months after MMF cessation, she developed a cough and productive sputum. Imaging revealed new ground-glass opacities (GGOs) in both lungs, indicating an acute exacerbation of sys

OBJECTIVE: Systemic sclerosis (SSc) is a rare and heterogeneous disease. SSc pathobiology is complex and it occurs in a genetically susceptible host. Our aim was to describe potential environmental and occupational exposures relevant to SSc.
METHOD: An online survey was developed in English to collect detailed information about relevant current/past environmental and occupational exposures. Individuals with SSc were invited to participate through international patient associations and social media. Participation was voluntary and no identifiable information was collected. We used descriptive

BACKGROUND: Systemic sclerosis is an immune-mediated inflammatory disease with marked sex differences in prevalence and severity. Although genome-wide association studies (GWAS) have advanced the understanding of systemic sclerosis genetics, sex-aware approaches remain scarce. We aimed to address this gap by examining autosomal sex-specific genetic factors in systemic sclerosis.
METHODS: Based on a chromosomal definition of sex, we conducted a sex-stratified autosomal meta-analysis of GWAS in systemic sclerosis. We retrieved patient-level and control data from a previous GWAS for systemic scl

Chronic systemic inflammation is increasingly recognized as a key contributor to cardiovascular disease, autoimmune disorders, and metabolic syndromes. Systemic inflammatory conditions share common pathophysiological pathways leading to myocardial injury, and biomarkers. However, these markers lack specificity and offer no information on localized myocardial damage, underscoring the need for advanced imaging biomarkers capable of detecting early cardiac involvement. Cardiovascular magnetic resonance (CMR) has emerged as a powerful and safe modality for comprehensive cardiac evaluation. This re

INTRODUCTION: Our understanding of the pathogenesis of systemic sclerosis-associated gastrointestinal (SSc-GI) disease is limited. This has hindered progress in the management of SSc-GI disease as objective measures to assess the extent of disease and monitor treatment response are lacking. We propose a conceptual model for understanding pathogenic mechanisms of SSc-GI symptoms. A multi-dimensional model of understanding the etiology of symptoms may enable an improved understanding of patients' symptom experience and opportunities for development of therapies.

OBJECTIVES: Nailfold videocapillaroscopy (NVC) is the gold-standard to identify scleroderma patterns. Our aim was to establish the minimum requirements; number of fingers, length of nailbed samples, location of nailbed areas, to diagnose the existence of systemic sclerosis patterns (SSc-P) and further identify non-SSc-P and SSc-P subtypes reliably.
METHODS: NVCs were taken during routine explorations by capillaroscopists from 12 hospitals, who made four diagnosis per patient according to the examination of the following: 32 images (4 images/finger, mean 59.43mm); 16 images (2 medial images/fi

BACKGROUND: Systemic sclerosis (SSc) is characterized by an increased mortality. Various mortality risk factors are included in the DETECT algorithm, a screening tool for SSc-associated pulmonary arterial hypertension. We tested the DETECT score as a predictor of all-cause mortality in SSc.
METHODS: SSc patients from the European Scleroderma Trial And Research (EUSTAR) cohort, with available data for calculating the DETECT and the SCOpE (currently proposed risk algorithm) scores and follow-up were included. Patients from the University Hospital Zurich served as derivation cohort, the remainin

Systemic sclerosis (SSc) is characterized by fibrosis and vasculopathy affecting the skin and internal organs, leading to multiorgan dysfunction. Injury of microvascular endothelial cells (ECs) in SSc impairs blood flow and causes tissue ischemia, leading to vascular complications such as Raynaud's, digital ulcers, and pulmonary hypertension (PH). PH in SSc presents as group 1 pulmonary arterial hypertension or as group 3 PH related to hypoxia and interstitial lung disease (ILD), both major causes of mortality. Analysis of multiome data from SSc ILD-PH lungs inferred transcription factors regu

PET/CT imaging with radiolabeled fibroblast activation protein inhibitors (FAPI), as an emerging modality for imaging fibroinflammatory diseases, targets activated fibroblasts involved in active tissue remodeling and fibrogenesis. Although initially introduced for oncologic indications, FAPI PET imaging provides a distinctive molecular insight into various fibroinflammatory conditions, including rheumatologic disorders, interstitial lung disease, inflammatory bowel disease, thyroid inflammation, and multiorgan immune‑mediated diseases. Unlike 18F-FDG PET/CT, which reveals the metabolic demand

Systemic sclerosis (SSc), especially its life-threatening interstitial lung disease (SSc-ILD), urgently requires targeted therapies. This review highlights how macrophage heterogeneity, transcending the obsolete M1/M2 paradigm, acts as a central orchestrator of the pathogenic "fibro-immune axis" linking vascular injury, immune dysregulation, and fibroblast-mediated fibrosis. Single-cell RNA sequencing and spatial multi-omics reveal distinct, context-dependent macrophage subsets with specialised roles across SSc tissues: secreted phosphoprotein-1 (SPP1+) macrophages dominate SSc-ILD, promoting

OBJECTIVES: To characterise the clinical features, predictors, and prognosis of patients with systemic sclerosis (SSc) who had both scleroderma renal crisis (SRC) and pulmonary arterial hypertension (PAH).
METHODS: In this retrospective cohort study, we reviewed 234 patients diagnosed with SSc at our institute between January 2013 and April 2024. Clinical characteristics and prognosis were compared among patients with both SRC and PAH, PAH alone, SRC alone, and neither PAH nor SRC. Predictors of the coexistence of PAH and SRC were identified using multivariate logistic regression analysis.

OBJECTIVES: Cardiovascular death is the second leading cause in systemic sclerosis (SSc), with coronary microvascular dysfunction (CMVD) playing a key role. PET-derived myocardial blood flow (MBF) and flow reserve (MFR) offer a validated, non-invasive way to assess CMVD. We studied the prevalence of impaired MFR in SSc and its association with vasodilator or immunomodulatory therapy.
METHODS: SSc patients who underwent dynamic 82-Rubidium PET myocardial perfusion imaging (MPI) at Yale New Haven Hospital (July 2016 to April 2025) were studied. Patients were matched 3:1 with controls without au
