Intralesional Adipose-Derived Stem Cells Reverse Established Dermal Fibrosis and Modulate Angiogenesis-Related Readouts in a Murine Systemic Sclerosis Model.

BACKGROUND: Systemic sclerosis (SSc) is characterized by progressive dermal fibrosis and microvascular dysfunction, and no approved therapy reliably reverses established skin fibrosis or durably restores microvascular perfusion. Adipose-derived stem cells (ASCs) possess anti-fibrotic, immunomodulatory, and vascular-related parameters properties, but their therapeutic impact in a strictly therapeutic (rather than preventive) SSc-like setting remains incompletely defined. METHODS: Bleomycin-induced systemic sclerosis model was induced in male C57BL/6 mice by daily subcutaneous bleomycin injecti

Membranous Nephropathy Preceding Systemic Sclerosis: An Unusual Presentation of Systemic Sclerosis sine Scleroderma.

Background. Membranous nephropathy (MN) is generally primary, but it can also occur as a secondary form in association with infections, neoplasms or autoimmune diseases. Systemic Sclerosis (SSc), especially in its sine scleroderma forms or in its early stages, rarely manifests itself as MN. Case report. A 60-year-old woman with onset of nephrotic syndrome and histological picture of MN, in the absence of systemic manifestations. The patient subsequently developed episodes of acute renal failure, recurrent proteinuria and clinical-serological signs suggestive of autoimmune connective tissue dis

Deficiency of Med1 in keratinocytes augments dermal fibrosis and inflammation in scleroderma.

Scleroderma is primarily characterized by widespread fibrosis and vascular dysfunction, with the abnormal activation of fibroblasts (FBs) being the most critical pathological features. While FB's activation has traditionally been attributed to infiltrating immune cells such as macrophages and T cells, emerging evidence highlights that keratinocytes (KCs) in scleroderma exhibit significant functional impairment. KCs secrete multiple key inflammatory factors that contribute to disease progression. Previous studies have identified the coactivator Mediator subunit 1 (Med1) as essential for regulat

WNT5A induces profibrotic activation of human macrophages via pSTAT3.

OBJECTIVES: WNT5A is a non-canonical WNT ligand that is overexpressed in autoimmune systemic sclerosis (SSc). While the pathogenic effects of WNT5A have been established in SSc fibroblasts, its contribution to profibrotic SSc macrophage activation is unknown. The goal of this study was to determine the effects of WNT5A on fibrotic macrophage activation. METHODS: Human monocyte-derived macrophages were cultured in the presence of recombinant WNT5A and assessed for changes in immunophenotype, STAT3 phosphorylation, and cytokine secretion. The ability of WNT5A-treated macrophages to induce healt

Factors associated with variations in the 6-minute walk distance during the follow-up of patients with systemic sclerosis.

INTRODUCTION: The 6-minute walk distance (6MWD) is commonly used to assess functional capacity in patients with systemic sclerosis (SSc), but its ability to reflect changes in cardiopulmonary status during follow-up remains uncertain. This study aimed to investigate whether variations in the 6MWD accurately mirror changes in organ involvement over time in SSc patients. METHODS: We conducted a longitudinal study on a global cohort of 227 consecutive SSc patients, including a subgroup of 88 patients with ILD or PH, over a two-year follow-up period. Statistical analyses were performed to assess

Scoping literature review on applied definitions of very early diagnosis of systemic sclerosis (VEDOSS) and pre-systemic sclerosis in original articles.

CONTEXT: The very early diagnosis of systemic sclerosis (VEDOSS) criteria were developed to diagnose systemic sclerosis (SSc) at a very early stage. However, several studies have applied these criteria inconsistently, often using them to identify patients at a predisease stage. This use has created confusion between the concept of VEDOSS and pre-SSc. With growing interest in predisease states in autoimmunity, a consensus definition of pre-SSc is mandatory. OBJECTIVE: As a preliminary step towards such definition, this scoping review aimed to map existing definitions of pre-SSc and VEDOSS in t

Characterization of Pulmonary Functional Abnormalities in Systemic Sclerosis Using Xenon MRI.

BACKGROUND: Xenon MRI is increasingly used to evaluate patients with interstitial lung disease (ILD) and pulmonary hypertension (PH), both of which are common manifestations of systemic sclerosis (SSc). As such, Xe-MRI may be suited to interrogate lung function impairment in SSc. PURPOSE: To characterize xenon MRI signatures in SSc, toward evaluating the utility of xenon MRI as a method to elucidate mechanisms of regional lung function impairment in this population. STUDY TYPE: Prospective. POPULATION: Of 25 participants initially imaged, 21 participants (18 females) with SSc were included.

Aurora A kinase activation contributes to the fibrotic phenotype in systemic sclerosis through primary cilia shortening.

BACKGROUND: Systemic sclerosis (SSc) is a severe autoimmune disease characterised by progressive fibrosis driven by fibroblast activation. Primary cilia, key hubs for profibrotic signalling, are markedly shortened in SSc fibroblasts, but the mechanisms underlying this phenotype remain unclear. This study aimed to define the signalling pathways responsible for primary cilia shortening and fibroblast activation in SSc. METHODS: Primary dermal fibroblasts from SSc patients and healthy controls were analysed for cilia incidence and length by immunofluorescence, profibrotic marker expression by qP

Mycophenolate Mofetil Treatment Reduces the Risk of Treatment Escalation Due to Vascular Complications in Limited Cutaneous Systemic Sclerosis: Emulation of a Target Trial From the Italian Rheumatology Society SPRING Registry.

OBJECTIVE: Mycophenolate mofetil (MMF) use in limited cutaneous systemic sclerosis (lcSSc) is relatively uncommon because of the lower fibrotic burden and the predominance of vascular complications. In vitro observations and clinical data from transplanted patients suggest a protective effect of MMF on endothelial function. Our aim was to evaluate the reasons for prescribing MMF treatment in patients with lcSSc and its impact on the need for escalation of vascular complication-related treatments during follow-up. METHODS: Patients with lcSSc enrolled in the Italian Systemic Sclerosis Progress

Compromised muscle energetics and intracellular O2 availability during dynamic contraction in the plantar flexor muscles of patients with systemic sclerosis.

The fundamental intracellular mechanisms underlying exercise intolerance and muscle weakness in patients with systemic sclerosis (SSc) are poorly understood. Thus, in six SSc patients (55 ± 11 yr) and six age-matched healthy controls (50 ± 13 yr), we assessed both skeletal muscle metabolism and intracellular O2 availability in vivo during submaximal contractions. Specifically, during dynamic plantar flexion exercise at 40% of maximal aerobic power, we used a multimodal approach combining phosphorus (31P) and proton (1H) MRS of deoxymyoglobin to examine both skeletal muscle energy pathways and

NAT10-dependent ac4C mRNA modification programs fibroblast pathogenicity in systemic sclerosis.

Systemic sclerosis (SSc) is characterized by progressive fibrosis driven by persistent activation of dermal fibroblasts, yet the epigenetic and epitranscriptomic mechanisms sustaining fibroblast pathogenicity remain incompletely defined. Here, we investigated the role of N-acetyltransferase 10 (NAT10)-mediated N4-acetylcytidine (ac4C) RNA modification in fibroblast activation and fibrotic remodeling. Integrative analyses of single-cell transcriptomics, human SSc skin, in vitro assays, in vivo models, and fibroblast-specific knockout mice revealed that NAT10 is upregulated in pathogenic fibrobl

A novel stratification framework based on vascular-fibrotic complications identifies distinct risk endotypes in systemic sclerosis: A retrospective cohort study.

BACKGROUND: Systemic sclerosis (SSc) is highly heterogeneous, and the traditional skin-based classification inadequately guides organ-specific management or predicts prognosis. OBJECTIVE: To develop and validate a novel stratification framework based on dominant vascular and fibrotic complications to identify distinct risk endotypes. METHODS: This retrospective cohort study included 281 SSc patients categorized at baseline into 4 subgroups: Vascular-dominant, Fibrotic-dominant, Mixed (both), and Limited (neither). Clinical, serological, and immunophenotypic data were compared. Longitudinal p
