Bridging Clinical and Preclinical Perspectives on Spina Bifida and Tethered Cord Syndrome: Presentation, Neurogenic Bowel and Bladder Management, and Animal Models.

Although spina bifida (SB) and tethered cord syndrome (TCS) are often discussed and studied as separate conditions, their co-occurrence is common, clinically consequential, and understudied, particularly with respect to neurogenic bladder and bowel dysfunction. SB arises from incomplete neural tube closure, whereas TCS involves abnormal fixation of the spinal cord, often leading to progressive neurologic deficits. SB affects approximately 3.63 per 10,000 live births in the United States, and TCS occurs in a significant proportion of affected individuals; however, the true incidence of their co

Radiologic Phenotyping in Patients with SOX10 Pathogenic Variants: Insights into Neck, Brain, and Temporal Bone Abnormalities.

BACKGROUND AND PURPOSE: The SOX10 gene is essential for developing neural crest-derived tissues, including the temporal bone, peripheral and central nervous systems, and melanocytes. Pathogenic SOX10 variants are linked to a broad clinical spectrum, including Waardenburg syndrome (WS) in isolation, Kallmann syndrome (KS), as well as an entity characterized by the combination of peripheral demyelinating neuropathy, central dysmyelinating leukodystrophy, Waardenburg syndrome, and Hirschsprung disease (PCWH). This study aims to characterize the full spectrum of abnormalities in the brain, tempora

CREST-associated ruptured right MCA aneurysm: A case report and review of the literature.

Intracranial aneurysms associated with systemic sclerosis and Calcinosis, Raynaud's phenomenon, Esophageal dysmotility, Sclerodactyly, and Telangiectasia (CREST) syndrome are rare, and their pathophysiological relationship remains uncertain. We report the case of a 24-year-old male with systemic sclerosis and clinical features suggestive of CREST syndrome who presented with subarachnoid hemorrhage caused by a ruptured right middle cerebral artery bifurcation aneurysm. The aneurysm was treated with microsurgical clipping, followed by stent-assisted coiling for a residual aneurysm neck identifie

A 62-Year-Old Woman With Progressive Dyspnea, Interlobular Septal Thickening, and Ground-Glass Opacities.

A 62-year-old White female individual who had never smoked and who had a diagnosis of idiopathic sclerosing mesenteritis in 2006 and antiphospholipid syndrome was referred to the interstitial lung disease clinic in January 2025 for evaluation of progressive dyspnea. In 2009, she experienced pericarditis that required a pericardial window. A PET/computerized tomography/CT scan that was performed at that time revealed diffuse low-grade metabolic activity that involve the mediastinum, abdomen, and pelvis. A bone biopsy from the left iliac crest showed scattered foamy macrophages. Based on a multi

Case Report: First description of an intracranial "NET" in ROHHAD syndrome; time to rename to the ROHHAD-NeCT syndrome?

In around 50% of patients with Rapid-onset Obesity Hypothalamic dysfunction, Hypoventilation and Autonomic Dysregulation (ROHHAD) syndrome, a neuro-endocrine tumor (NET) is found. Reported NETS are (ganglio)neuroblastoma or ganglioneuroma located extracranially. Here, we report for the first time a young patient with signs and symptoms fitting the criteria of ROHHAD syndrome in whom an intracranial ganglion cell tumor was found, pathologically resembling the reported extracranial NETs. Our finding leads to the discussion of whether the NET in ROHHAD-NET syndrome should be classified as true NE

Direct and indirect regulation of SIX1+EYA transcriptional activity by PA2G4, MCRS1, and SOBP.

Branchio-oto-renal (BOR) syndrome is an autosomal dominant condition characterized by variable malformations including hearing loss and renal dysfunction. Variants in SIX1 or its activating co-factor EYA1 are causative in about 50% of patients. Some patients carrying BOR variants also present with craniosynostosis, indicating that cranial skeletal dysmorphologies could be an under-diagnosed feature. To date, most studies on the role of SIX1 have focused on its role in the cranial placode-based development of the inner ear, whereas its role in the neural crest cells of the mandibular arch, whic

Life-Threatening Gastrointestinal Bleeding Revealing a Rare Coexistence of Ampullary Ganglioneuroma and Pancreatic Neuroendocrine Tumor: A Case Report.

Background and Clinical Significance: Ganglioneuromas are rare benign tumors of neural crest origin, with gastrointestinal involvement being uncommon and ampullary localization exceptionally rare. Pancreatic neuroendocrine tumors (NETs) are also uncommon neoplasms with variable biological behavior. The coexistence of these two entities is unusual, particularly in the absence of an identifiable hereditary syndrome. Case Presentation: A 38-year-old man presented with hematemesis and multiple episodes of melena over 18 h and was found to have significant anemia with borderline hemodynamic stabili

Cervical Ganglioneuroma in Neurofibromatosis Type 1 Presenting as a Carotid Space Mass With Postoperative Horner Syndrome.

Ganglioneuromas are uncommon benign tumours derived from neural crest cells and composed of mature ganglion and Schwannian stromal elements. They most frequently arise in the posterior mediastinum and retroperitoneum, while cervical involvement is rare. Although ganglioneuroma may occur in association with neurofibromatosis type 1, such presentations are infrequently reported. Surgical excision is generally curative; however, postoperative sympathetic dysfunction, including Horner syndrome, may occur because of the close anatomical relationship of these tumours to the cervical sympathetic chai

SOX10 Mutation of Waardenburg Syndrome With Hypogonadism: A Report of 2 Cases.

Waardenburg syndrome (WS) is a complex genetic disorder primarily characterized by auditory and pigmentary abnormalities, resulting from neural crest cell migration disorders. We reported 2 genetically confirmed SOX10-mutant WS cases illustrating critical management principles. Both patients underwent early auditory intervention (case 1: cochlear implantation at age 3; case 2: hearing aids from age 2), resulting in preserved age-appropriate language acquisition. Each case manifested delayed puberty with biochemical evidence of hypogonadotropic hypogonadism, necessitating gonadotropin therapy t

A novel activating somatic mutation in EPAS1, coding for HIF-2α, in a patient with a paraganglioma and sickle cell disease.

Pheochromocytomasand paragangliomas (collectively referred as PPGLs) are highly heritable neoplasms arise from chromaffin cells of neural crest tissues; 40% of patients with PPGLs harbour germline pathogenic variants (PV), which up to 45% of patients exhibit somatic mutations in similar susceptibility genes. Endothelial PAS domain-containing protein-1 [also known as hypoxia inducible factor-2α, HIF-2α] is encoded by EPAS1, and along with other hypoxia-inducible factors (HIFs) acts as a key mediator in the cellular response to hypoxia. Gain-of-function mutations in EPAS1 have been linked to the

The vascular-osteogenic interface in craniofacial development: a structured review of emerging associations in congenital malformations.

Craniofacial skeletal morphogenesis depends on tightly coordinated interactions between vascular development and osteogenesis. Although congenital craniofacial malformations are traditionally conceptualized as bone-intrinsic disorders, emerging molecular evidence suggests that dysregulated vascular pathways may contribute to skeletal maldevelopment. Here, we synthesize emerging experimental and genetic evidence linking perturbations in vascular development to congenital craniofacial osseous phenotypes. A structured literature search was conducted in PubMed and Embase to identify experimental a

SPEN deficiency contributes to the development of orofacial clefts in humans and mice.

Haploinsufficiency of SKI, PRDM16, RERE, PAX7, and GRHL3 have been implicated in the development of orofacial clefting (OFC) associated with chromosome 1p36 deletions based on human and/or mouse data. Haploinsufficiency of SPEN, a 1p36 gene that encodes a transcriptional repressor, causes Radio-Tartaglia syndrome, a neurodevelopmental syndrome in which high/narrow palates are common, and OFC is occasionally observed. We show that Spen-null embryos have abnormal palatal shelf elevation and extension leading to the development of cleft palate. Mesenchymal cell proliferation in the medial halves

Friend or foe: divergent immunomodulatory effects of metabolites derived from the virucidal zinc finger inhibitor SAMT-247.

BACKGROUND: Topical administration of SAMT-247, a mercaptobenzamide thioester zinc finger inhibitor targeting the HIV nucleocapsid Gag zinc finger protein, generates multiple metabolites in mucosal tissues, including Met-A, Met-B, Met-C, and Met-D. Prior studies established that SAMT-247, delivered either as a vaginal gel or via an intravaginal ring (IVR), synergizes with the ΔV1DNA/ALVAC-SIV/ΔV1gp120/alum vaccine regimen to markedly reduce vaginal SIVmac251 acquisition risk. This enhanced protection is associated with augmented protective mucosal immunity and reduced inflammatory responses th

GC content mismatch of transgene destabilizes RNA virus genomes.

GC content-the proportion of guanine and cytosine nucleotides-varies widely among organisms and viruses. Although GC content is recognized to have biological significance, its functional role in viruses remains poorly understood. Here, we examined the impact of GC-content bias using three positive-sense single-stranded RNA viruses with distinct GC profiles: severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), Japanese encephalitis virus (JEV), and hepatitis C virus (HCV). Variants of the NanoLuc (Nluc) reporter gene, engineered with synonymous mutations to alter GC content, were inser

Asymmetric Bilateral Axenfeld-Rieger Anomaly Misdiagnosed as Unilateral Iridocorneal Endothelial Syndrome.

Axenfeld-Rieger anomaly is a rare, congenital disorder characterized by neural crest-derived defects of the ocular anterior segment, traditionally presenting with high bilateral symmetry. This case report highlights a significant diagnostic pitfall involving a profoundly asymmetric presentation of bilateral Axenfeld-Rieger anomaly in a systemically healthy 23-year-old male student, complicated by late-onset secondary congenital glaucoma and an associated cystoid macular edema. Due to severe, unilateral microcystic epithelial and stromal corneal edema confined strictly to the left eye, the unde

Cognition in Cardiovascular Disease.

PURPOSE OF REVIEW: Acute cerebrovascular events once dominated efforts to protect the brain from cardiovascular disease. As stroke prevention and treatment have advanced, attention has broadened to the more continuous vascular processes that contribute to cognitive decline across later life. This review organizes cardiovascular brain injury by route, direct structural injury versus indirect mediated pathways, and tempo, acute events versus chronic processes, with vessel caliber determining lesion topography. RECENT FINDINGS: Vascular and Alzheimer pathology commonly coexist in older adults wi

Data-driven subtypes of mild cognitive impairment and their modifiable determinants: latent profile analysis of the BIOCIS cohort.

BACKGROUND: "Dry mouth" syndrome (chronic hyposalivation) can be caused by a number of pathophysiological conditions such as acute and chronic stress exposure, abnormal body weight (both too high and too low ones), eating disorders (such as anorexia nervosa), metabolic syndrome(s), Sjögren's and Sicca syndromes, drugs and head/neck radiotherapy application. In turn, the chronic hyposalivation as a suboptimal health condition significantly reduces quality of life, may indicate a systemic dehydration, provokes and contributes to a number of pathologies such as a strongly compromised protection o
